2 Leadership Message Dear Friends, Your Life. Our Passion. That s our mantra. It sums up our mission and what drives us. Since 1978, Hospice of Palm Beach County, Hospice of Broward County and Hospice by the Sea have cared for 200,000 families in South Florida. These three hospices are now one branch of Trustbridge, a community nonprofit that provides support for families facing serious illness, 24 hours a day. Our other services include skilled and compassionate home health care, unparalleled caregiver support and extensive bereavement programs. While we have a new brand and look, we retain the legacy of the meaningful work we have done in our communities for nearly 4 decades. We are one provider one team bound in our longstanding mission. Our passion and commitment to the people we care for drive us to reach for industry-leading goals, employing innovative methods in clinical care and our business practices. With the goal of improving patient care and employee satisfaction, we utilize Lean Six Sigma methods and tools to streamline our internal processes. This collaborative team approach enlists front-line staff and leadership in an ongoing effort to improve performance. Our unmatched levels of hospice and home health care have been nationally recognized for excellence, earning the Joint Commission National Quality Seal of Approval for both Palm Beach and Broward counties. We are the only hospice with this distinction in both counties. To deliver vital services in the most effective ways possible, we have our own in-house pharmacy and durable medical equipment division. Recent innovations in our pharmacy include the addition of a robot that fills 50% of our prescriptions. Our CareConnection department is dedicated to meeting all patient needs with immediate response, 24 hours a day. Our goal for new patients is to have a nurse walking through their door within 4 hours of their initial contact. Our Knowledge Center combines robust internal staff orientation, education and training programs. It has been recognized nationally for its high-quality education practices and materials. The Center is now home to the nation s first hospice Simulation Lab, which utilizes computerized mannequins to train clinical employees in palliative-care scenarios. We re proud of our passion and our commitment to those we serve. 5 Making a True Difference in the Lives of Patients and Families Our Foundations (Hospice of Palm Beach County Foundation, Hospice by the Sea Foundation and Hospice of Broward County Foundation) are the philanthropic arm of Trustbridge. Thanks to donors like you, we are able to go beyond what is covered by Medicare, Medicaid and private insurance to offer specialized services and exceptional care to every family who needs it. Your donations help in so many ways! 40.1% Charitable Care As your local, not-for-profit hospice, we are committed to providing care for everyone in our community, including those without insurance coverage or the ability to pay. Thank you for helping us to fulfill our mission. 22.0% Community Outreach/Employee Health Community outreach initiatives, employee health programs and the updating of our inpatient care centers were made possible by your support. 12.4% Music Therapy Music can create a physical and emotional connection that relieves pain, anxiety, depression and other symptoms. Thanks to your generosity, we offer one of the nation s largest music therapy programs, providing 7,957 music therapy visits in % Extraordinary Comfort Measures Trustbridge is one of only 4% of hospices nationwide that admit patients into hospice care who need to continue treatments for their comfort (chemotherapy, radiation, intravenous medications and blood transfusions). Your donations fund the comfort measures so these families can benefit from the world of support that hospice offers. 7.1% Physician and Employee Training Your generosity provides funding for the Fellowship in Hospice and Palliative Medicine program, our partnership with University of Miami Miller School of Medicine. In addition, we are able to offer training and education programs for our physicians and nursing staff (including the nation s first hospice Simulation Lab, which provides computerized mannequins for training). 6.8% Grief Support In 2016, we were able to help 1,389 adults with individual and group counseling. We served 333 children and teens with individual and group counseling, and 3 camp programs: Camp Good Grief, a 3-day overnight camp; Camp Stingray, a week-long day camp; and Club Seahorse, a 1-day program offering therapeutic experiences with miniature horses and the arts. *See detailed statistics on the Caring for the Community page of this report.
