1 SEPTEMBER 21, 2018 ADDENDUM NO. 2 FOR ASBESTOS ABATEMENT, LEAD BASED PAINT MITIGATION AND MICROBIAL REMEDIATION SERVICES FOR CHICAGO O HARE AND MIDWAY INTERNATIONAL AIRPORTS SPECIFICATION NO For which Bids were due in the office of the Chief Procurement Officer, Department of Procurement, Bid & Bond Room 103, City Hall, 121 N. LaSalle Street, Chicago, IL at 11:00 a.m., Central Time on September 21, The following revisions and/or changes are incorporated into the above-referenced Specification (the "Contract Documents") as noted. All other provisions and requirements as originally set forth, except as amended by this previous addenda, remain in full force and are binding. Any additional work required by this Addendum shall conform to the applicable provisions of the original Contract Documents. BIDDER MUST ACKNOWLEDGE RECEIPT OF THIS ADDENDUM IN THE SPACE PROVIDED ON THE PROPOSAL EXECUTION PAGE. SECTION 1. NOTICE OF REVISIONS AND/OR CHANGES BID OPENING HAS BEEN POSTPONED TO OCTOBER 4, 2018 Change 1: Change 2: Change 3: The Bid Opening Date has been postponed to October 4, For which Bids are due in the Department of Procurement, Bid & Bond Room, Room 103, City Hall, 121 N. Section Definitions, delete the existing definition for rmal Work Hours replace it in its entirety with the following revised Section , definitions: rmal work hours first up to eight (8) hours worked in a day at the City worksite, Monday through Friday. Section Definitions, add the following definition: n-normal work hours hours worked at the City worksite in excess of eight (8) hours per day Monday through Friday, all day on Saturdays, Sundays and Designated City Holidays. 2 Change 4: Delete the existing Section 5.6 Contract Extension Option, and replace it in its entirety with the following revised Section 5.6: Contract Extension Option Change 5: The City may extend this Contract following the expiration of the contract term for up to 181 Calendar Days or until such time as a new contract has been awarded for the purpose of providing continuity of services and/or supply while procuring a replacement contract subject to acceptable performance by the Contractor and contingent upon the appropriation of sufficient funds. The CPO will give the Contractor notice of the City s intent to exercise its option to renew the Contract for the approaching option period. Delete the existing Section 5.21 Unit Pricing for Asbestos Abatement, and replace it in its entirety with the following revised Section 5.21: Unit Pricing for Asbestos Abatement The Proposal Pages (Article 9) illustrate the criteria under which various line items have been identified for the asbestos abatement work. The asbestos work must consist of gross removal, glovebag removal, non-friable removal, O&M activities, or other asbestos related work activities necessary for project performance. The scope of abatement activities and abatement methods will be determined at the discretion of the Commissioner and shall be verified by Contractor prior to project performance. Work activities to be performed in occupied areas that will require the construction barrier systems and additional security measures will be determined at the discretion of the Commissioner prior to project performance. Additionally, each category of asbestos abatement work, as listed on the Proposal Pages, will be done either during normal working hours or during non-normal working hours and may require emergency response services on occasion. Each and every unit price will include Overhead and Profit, including, but not limited to, the following costs: Administrative, management, and supervision (including field superintendent and foremen); coordination; correlation; travel and transportation and per diem personnel expense; project recordkeeping; billing; permit fees; notification of cognizant government agencies; preparation and reproduction of plans and logs; employee required health services and recordkeeping; all employee benefits and insurance including, but not limited to, employee wages and fringe benefits, Social Security and Medicare taxes (i.e., FICA), federal and state unemployment taxes, asbestos/pollution liability insurance, builder s risk insurance, public liability insurance, and workers compensation insurance; all equipment, supplies, tools, apparatuses for personal air quality monitoring; scaffolding and ladders; signs; ACM removal; HEPA filters and vacuums; NAMs including proper containment; labeling, removal, transportation, and disposal of waste in an authorized refuse area, all as necessary to perform the work of asbestos abatement. 3 Change 6: Section 5.48 Additional Unit Information for All Projects, add the following section: E. item 60 - Allowance for reimbursement of overtime costs for bid line items 2 through 40, 43 through 50 and 52 though 55 This item shall be used to reimburse the Contractor for additional wages paid to workers and working supervisors, in excess of the normal straight time rate when the Contractor is required to work overtime. Overtime compensation only applies to workers and working supervisors in direct charge of specific operations associated with the overtime Work. Straight Time Labor costs should be included in the unit price for the various line items. Overtime to be paid under this Allowance shall be paid as REIMBURSEMENT to the Contractor for the difference between the overtime wages paid for work in excess of eight (8), hours Monday through Friday, all day Saturday, Sundays and Designated Holidays and the straight time hourly rate normally paid to the employee. The Commissioner reserves the right to request copies of workers' pay checks to verify the wages actually paid to the employees. The City will pay no more than a 50% premium (time and one half) on the straight time hourly rate paid for hours worked in excess of eight (8) hours Monday through Friday and all day Saturday and no more than a 100% premium (Double Time) on Sundays and Designated City Holidays and the prevailing wage rate for regular time. The Contractor must have written authorization of the Commissioner prior to commencing any Work that would call for payment under this Allowance. Failure to obtain authorization may result in disallowance of overtime payment. Any Work requiring payment of $5,000 or more from this Allowance shall additionally require approval of the Chief Procurement Officer. The Contractor is not entitled to any remaining balance from the Allowance upon completion of individual Work items or termination of this Contract. The quantity of overtime for line items 2 through 40, 43 through 50 and 52 through 55 Commissioner approved overtime payment will be measured based on labor costs recorded on daily Work reports supported by Certified Payroll provided by the Contractor for trade personnel and working foremen in direct charge of specific operations associated with the overtime Work Change 7: Bid value for line item 60 has been included on the proposal pages. Bidder must not remove or change figure. Delete the existing Article 9 Proposal Pages and replace it in its entirety with the revised Article 9 Proposal Pages attached to this Addendum. 1 as Attachment 1.
