Winn, Contracts & Procurement 1214 Aalis Dr, Taholah, Washington QIN Administration Building, Lobby ASBESTOS ABATEMENT SPECIFICATIONS DISPOSITION OF MATERIAL: The contractor must observe all applicable laws for disposal of debris. Burning on site is strictly prohibited. All necessary measures must be taken to safeguard life and property. Qualifications Desired: "Responsible and qualified" means the Contractor, offeror or vendor has: adequate financial resources to perform the services required in the Contract and to secure the necessary insurance, qualifications and bonds; the ability to comply with the performance schedule; a satisfactory record of performance; Washington state license for asbestos removal; a satisfactory record of business ethics and integrity; the

2 necessary skills and expertise to perform the services provided for in the Contract; the necessary production, construction and technical equipment and facilities to perform the services provided in the Contract; no past violations of Quinault Indian Nation laws or regulations within the last five (5) years. Nation s Tribal Employment: (TERO) The Nation s Tribal Employment Right Ordinance (TERO) provides the Nation s Indian Preference policyshall apply to this contract. A TERO plan must be submitted by the successful bidder, and a onetime TERO fee of 1.75% of the total amount of the contract shall be paid by the successful bidder prior to commencing work on the Quinault Reservation. ABATEMENT: Contractors should visit sites and have all skills and certifications necessary to complete this project in a safe manner. Scope of work includes: 1. ORCAA Asbestos Removal Permit / L & I Notification 2. Setup and tear-down of Negative Pressure Containment 3. Proper removal, packaging, and disposal of asbestos containing materials at the listed address. 4. Air Monitoring: Including Clearance Sample. 5. Listed address do not have power or water. Contractor responsible for providing power and/or water. 6. Contractor responsible for securing residence daily. 7. A copy of the asbestos survey and approved Quinault Nation Demolition Permit, as well as any subsequent amendments, must be kept on site and available for review by Agency inspection personnel. 8. Provide proof of removed, packaged asbestos containing materials has been properly disposed. End of specifications. The contractor shall submit and date invoices to the Quinault Indian Nation, PO Box 189, Taholah, WA, 98587, Community Development Department, upon completion and approval of the work. Washington State B&O taxes do not apply to construction charges performed within the boundaries of the Quinault Indian Reservation. All other applicable taxes and fees may apply. 21 Asbestos projects within Clallam, Grays Harbor, Jefferson, Mason, Pacific, and Thurston counties REQUIRE A NOTIFICATION and require that the following conditions be met prior to the demolition or renovation. Olympic Region Clean Air Agency (ORCAA) regulations define an asbestos project as any activity involving the abatement, renovation, demolition, removal, salvage, clean up, or disposal of asbestoscontaining materials, or any other action that disturbs or is likely to disturb any asbestos-containing materials. It includes the removal and disposal of stored asbestoscontaining materials or asbestos-containing waste material. This term does not include the application of duct tape, rewettable glass cloth, canvas, cement, paint, or other nonasbestos materials to seal or fill exposed areas where asbestos fibers may be released. ORCAA defines ACM as more than 1 percent (1%) of asbestos. Notification is not required for removal and disposal of non-friable asbestos caulking, window glazing or roofing if it meets all the requirements in ORCAA Regulation The following is merely a reference guide and not a substitute for agency regulations: 1. Certified asbestos contractors can be found on ORCAA s website, on the Washington State Labor and Industries website, as well as conducting an internet search. 2. Asbestos samples must be sent to a NVLAP Laboratory (National Voluntary Laboratory Accreditation Program) per 40 CFR A list of labs can be found on ORCAA s website. 3. The start date for asbestos abatement projects must be at least 14 days from the submission date of the complete application and payment. 4. It is the responsibility of the building owner and/or asbestos contractor to ensure all ACM identified (or suspected) in the survey and proposed to be removed, has been removed and properly disposed of in accordance with ORCAA s Regulations. 5. A copy of the asbestos survey, Asbestos Notification, and any subsequent amendments must be kept on site and be available for review by Agency inspection personnel. 6. Use the Completion Notification and Amendment Form to make changes to the original notification. 7. The original asbestos notification will expire on the Completion Date. To change the completion date, an Amendment and Completion Notification form must be received PRIOR to expiration. If the notification expires and the project is not complete, you must submit and pay for another asbestos notification. Under no circumstances will a project be extended beyond 1 year from original submission date. 8. Upon completion of project, fill out and submit an Amendment and Completion Notification, documenting actual date of completion. 9. Asbestos notifications require a 14-day prior notice before work begins. ADDITIONAL REQUIREMENTS Owner Occupied Residential Dwelling means any single family housing unit which is permanently or seasonally occupied by the owner of the unit after the proposed

22 project. This term includes houses, mobile homes, trailers, houseboats, and houses with mother-in-law apartment or a guest room. This term does not include structures that are demolished or renovated as part of a commercial or public project; nor does this term include any mixed-use building, structure, or installation that contains a residential unit, or any building that is leased or used as a rental, or for commercial purposes. Emergency Project: An operation that was not planned but results from a sudden, unexpected event that, if not immediately attended to, presents a safety or public health hazard, is necessary to protect equipment from damage, or is necessary to avoid imposing an unreasonable financial burden. This term includes operations necessitated by non-routine failures of equipment. The property owner must provide written notification explaining the reason for the emergency. There is a $52 emergency fee in addition to the notification fee. These fees are due upon submission of your notification, and are non-refundable.
