1 MANDATORY PRE-BID MEETING MINUTES JUN 16, 2014 ITB SEBRING PARKAY PHASE II. IMPROVEMENTS AND INSTALLATION OF DRAINAGE PIPE PER JACK AND BORE METHOD, HIGHLANDS COUNTY PROJECT NO Meeting called to order at 2:04 P.M. In attendance: Announcement Sheet: All individuals in attendance are noted on the attached sign-in sheet. The Announcement Sheet was read by the County s Purchasing Analyst. 1. Introduction of Attendees. 2. Agenda items 3-8 were announced 3. Project overview by Project Manager a. Goal is to complete work while summer break from schools. b. The area involved has railroad on it. c. Easements are shown on the construction plan d. Fiber Optic is shown on plans e. Utilities- coordinate with utilities 4. Sebring Gas a. Line can be lowered or raised b. Within 5-10 there is a water line 5. Questions: a. Does the insurance have to include insurance for railroad? Yes. b. Lane closure will be provided c. Budget is $200, d. If easement is disturbed, restoration is not required. (very sandy ground) Anything outside the fence is the Contractor s responsibility e. Accumulated dirt to be piled up ban be left behind. (to be used on berm) f. Shell can be hauled in to stabilize the easement g. Casing cannot be sized down. h. PVC pipe cannot be used in place of HDPD. i. Comcast doesn t have anything in this area j. There possibly are some abandoned phone lines k. There are no traffic signals close to the project site The meeting was adjourned at 2:27 P.M. 1

Bid Opening date is Thursday; June 26th 2014 at 2:00 P.M. in the Highlands County Purchasing Department located at 4320 George Boulevard, Sebring, FL Proposals recieved later that this date and time will be rejected. This meeting is open to the Public, attendance is not required from the Bidders. 5. Request for Information (RFI) Cut-Off Date: June 19th, All RFI s must be in writing to contact listed in the bid document. 6. Local Preference will apply to the award of this Proposal; 7. Each Bid must contain Proof of enrollment in E-Verify. 8. Minority or Women owned business; please submit a copy of the certificate. 9. Comments from the Project Manager 10. Questions

Brief Minutes of this meeting are being taken and copies of such minutes may be viewed or obtained through the Public Records Office at 600 S. Commerce Avenue, Sebring, Florida The public will be allowed to ask questions and have input: during the meeting before any vote, recommendation or formal action is taken at the end of the meeting, before the meeting is closed. Because minutes are being taken, I will ask that anyone from the Public who wishes to speak please obtain permission first by raising your hand to be recognized. 5.
