ADDENDUM NO. 2 Dated: 8/29/17 IFB No. B17031 For Purchase & Installation of Manufactured Home All Contractors bidding the project are hereby advised to incorporate the following changes and/or clarifications and related dollar amounts in their bids for the above-referenced project. All conditions described in the project SOW and Specifications shall apply in full force. 2.1 Clarification: The following items noted as items a) and b) of this Amendment are not required in this project: a) Ref. IFB section 2.0 Scope of Work and Technical Specification, pg. 22 item No : Contractor shall maintain daily and weekly records as to the employees/workers performing work on this project. Additionally, Contractor shall provide to SNRHA after execution of contact, upon request: a. List of Employee working on site b. Job Title c. Description of Work Duties d. Hourly Rates Note: At any time during the term of this contract employees are removed or replaced; SNRHA shall be notified promptly of the change and provide the required information for the new employee. Contractor shall provide to SNRHA upon request documented proof of pay to each laborer working on this project. (This also applies to all sub-contractors, consultants, etc). b) Ref. Addendum No. 1 dated 8/21/17, Item 1.5: Question: Is Certified Payroll required? Answer: No, Davis Bacon is not applied to this project. However SNRHA will be requiring the company awarded the contract to provide a list of the employees, trades and hourly rates; and the employees have to able to legally work in the US. 2.2 Attachment A Form of Proposal has been revised and issued as an attachment to this Addendum. Bidders are to use the revised Attach A Form of Proposal in their bid submission. Attachment E contains a form for Proposed Product Specifications for the Mobile Homes. This form shall be filled out completely and submitted under Tab No. 9 along with any other related information bidder(s) want to include. Questions received and with Responses: 2.4 Q: What size homes would like for these lots? We took a look at the lots, and here are the largest sizes each lot will hold: Lot #12-27'x60' Lot #51-14'x68' Lot #52-14'x68' Lot #63-14'x68' Lot #84-16'x60' A: The homes should be designed to maximize the living area of the unit, when placed on the lot, while remaining in compliance with all municipal setbacks. 6 IFB No. B17031 Attachment E Dorothy Kidd Mobile Home Park PROPOSED PRODUCT SPECIFICATIONS FOR MOBILE HOMES (This Form must be fully completed and placed under Tab 9 of the Bid Submittal.) Instructions: Any and all brand names used within the following specifications provided by the SNRHA are only provided so that prospective bidders are aware of the quality and functionality of the product(s) that the SNRHA desires. The SNRHA is, pursuant to HUD Regulations, required to consider any product that is equal to or the same as the brand name product that the SNRHA has specified ( equal to or same as means that the Proposed Specification is substantially equivalent to the specification detailed within the IFB Specification column). Each bidder shall enter within the Proposed Specification column for each listed item the Brand Name and Model Number of the product that he/she is proposing to provide as an equal or same as product to that specified by the SNRHA within the IFB Specification column. Further, each bidder shall include a Manufacturer s Specification Sheet for each such item identified as well as any warranty or guarantee of the Bidder. Please note that all specifications are subject to normal manufacturing tolerances. Feel free to duplicate this page or use Supplemental pages of your own.
