3 Public Works Project of the Year Award Submittal Instructions 1. Complete the two-page Nomination Entry Form. 2. The supporting documentation should address items All project photos should be submitted electronically in digital format. If the photos are not incorporated into the belowmentioned PDF file, then the photos should use JPG format and limited in size to 2 MG per photo. 4. All submittals should be made electronically and should be in PDF format. The address for the submittals is apwa.awards@gmail.com. The PDF file should include the information requested in items 1, 2 and 3 listed above. If PDF format is not possible, then WORD format is acceptable. 5. Due to limitations, all submittals must be limited in size to 10 MG. If the submittal exceeds this size, the submittal should be split into smaller s. Supporting Documentation Please address each of the following areas in your supporting documentation, adhering to the sequenced below when possible. Please limit the response to each criterion to 100 words or less. 1. Discuss the use of good planning, design and construction management techniques to complete project on schedule, budget, and regulatory requirements. 2. Discuss the degree to which the project enhances a public service or facility. 3. Highlight community relations or actions to minimize public inconvenience, safety precautions to protect public lives and property, provision of observation areas, guided tours, or means of improving relations between the agency and the public. 4. Highlight environmental concerns and actions for mitigation, enhancement, or avoidance during project development or construction. 5. Discuss accomplishments under adverse conditions including, but not limited to, weather, soil, traffic, site conditions, space, time budget, or other conditions over which there was to control. 6. Discuss any additional considerations deemed of importance to the public works agency such as quality control, safety, value engineering, construction innovations, and money-saving techniques. 7. Provide a concise summary of the project on why it is deserving of the award. 8. Provide the name of the public agency official that can speak on behalf of the project s accomplishments. Provide name, title, and telephone number. 9. Project Manager must verify that the project was completed on time and within budget. If not, please explain why.

4 Individual Awards Nomination Entry Form One nomination per form and self nominations will not be accepted. Outstanding Service in a Public Agency Public agency individual who inspires excellence and dedication in public service through outstanding career service achievements; Focus of award is on career service to the public works profession rather than on any event or project; and Nominee must be a Chapter member. Outstanding Service in the Private Sector Private sector individual who inspires excellence and dedication in public service through outstanding career service achievements; Focus of award is on career service to the public works profession rather than on any event or project; and Nominee must be a Chapter member. Contractor of the Year Award Use of good construction management techniques and timely completion of project; Demonstrated awareness safety throughout the construction of the project; Community relations as evidenced by efforts to minimize public inconvenience during construction, safety precautions to protect the public, provision of observation areas, guided tours, etc.; Protection of the environment during the project; Unusual accomplishments under adverse conditions such as age or condition of the facility, adverse weather, soil, etc,; Exceptional efforts to maintain quality control, value engineering, cost control, etc; and Young Leader Award Candidates must be active members of APWA and not exceed 35 years of age as of the award nomination deadline (February 17th, 2017) ; Contributed in some significant manner to his/her agency or company by implementing new ideas, overcoming problems, controlling costs, etc.; Helped advance the cause of the public works profession either with leadership commitments to significant community project or as an advocate for public works issues at the legislative level in local, state, or federal governments; and Demonstrated an ongoing commitment to continuing education in the public works field, i.e. related educational degrees, attendance at workshops, etc. Award of Merit/Certificate of Merit An individual who provided outstanding service in a clerical, entry level, operational or a supervisory position; One recipient per public agency or company; and Public Works Advocate Award An individual who celebrated or promoted an understanding and appreciation for public works; Individual cannot promote a manufacturer, product, or person;

5 Student Award Candidates must be a current student at a local university or community college; Contributed as a team member to a local public works project within the past year; Demonstrated an understanding of public works; and Nominated by Name: Agency/Organization: Telephone: Supporting Documentation On a separate sheet, provide a summary of why the individual is deserving of the award (200 words or less). Outstanding Operations & Maintenance Personnel Award An individual or group of operations and or maintenance staff who have developed or implemented an exemplary process or system improvement that greatly enhanced the safety of the process, the cost effectiveness of services, or public interactions. Innovation of new procedures or tools that have improved performance and safety of personnel. Protection of the public, exemplary service during emergency responses, or challenging situations can also be considered. Nominee s agency needs to have at least one Chapter member, but the nominee does not need to be a Chapter member.
