CPM awards presented at Care Providers of Minnesota s Annual Convention & Exposition at the NEW Mystic Lake Convention Center, November 12 14, 2018 NEW FOR 2018 Any award nomination to Care Providers of Minnesota s Award Program that qualifies for NCAL s Award Program will be automatically submitted to NCAL (the National Center for Assisted Living). NCAL awards presented at AHCA/NCAL Convention & Exposition in San Diego, CA The basics 1. Browse through the categories and read the criteria carefully to ensure the best fit for your nominee. 2. The employee s efforts demonstrate a commitment to their organization and the care-providing profession. The Innovative Program of the Year Award recognizes an individual or team that has tried something different in their delivery or provision of a program or service. It recognizes out-of-the-ordinary efforts to enhance the lives of residents/tenants/clients, staff, or volunteers at a member organization. HINT: think PIPP (performance improvement projects) Eligible nominees: Any nursing facility, assisted living/housing/home care, corporate, or business partner member. 1. The Nurse of the Year Award honors an individual who displays extraordinary nursing skills in caring for individuals residing in a care facility/senior care. Eligible Nominees: Any qualified nurse (LPN/RN) nominated by any nursing facility, assisted living/ housing/home care member whose direct care activities comprise 50% or more of his/her practice. 1. Why your nominee deserves to be the NURSE OF THE YEAR AWARD recipient 2. Specific examples of your nominee s exceptional skill in caring for others 3. Specific examples of your nominee demonstrating outstanding compassion, supervisory success, innovation, achievement, and capabilities in his/her provision of high-quality, person-centered care 4. WE LIKE WHERE YOU RE GOING The Rising Star Award recognizes the contributions of an employee from any discipline, who is new to his/her profession (within the last five years) and has made an impact in his/her years of service. Eligible Nominees: Any nursing facility, assisted living/housing/home care, corporate, or business partner member. 1. Why you believe your nominee deserves the RISING STAR AWARD 2. The Volunteer of the Year Award recognizes a youth, adult, or group of volunteers who have made contributions to the quality of life for residents/tenants/ clients or family members, or to the working environment for staff. Eligible Nominees: Individual(s) of any age who volunteer at any member organization. 1. Explain why your nominee deserves to be this year s VOLUNTEER OF THE YEAR AWARD recipient 2. The volunteer s length of service; how, when, and why he/she/they began volunteering 3. The volunteer s activities and contributions to improve the quality of life for residents/ tenants/clients or the working environment for staff 4. That s right new this year, any housing member who submits a nomination for these Care Providers of Minnesota awards: Administrator/Executive Director/Senior Living Manager of the Year Award Caregiver of the Year Award Nurse of the Year Award (and possibly Employee of the Year, if applicable) will be automatically submitted to your national association the National Center for Assisted Living for their awards program (Note, you are also welcome to submit a nomination directly to NCAL!). WHY IS A NATIONAL AWARD IMPORTANT? It gives you bragging rights for BEING THE BEST IN THE NATION! When you win, it lets others know what an amazing staff and/or place you have. It boosts morale and can impress future employees. 14 NATIONAL AWARDS NATIONAL RECOGNITION SUNNY & WARM SAN DIEGO IN OCTOBER...WANT TO BE THERE? The National Center for Assisted Living (NCAL) Awards Program recognizes assisted living employees from around the country who demonstrate exemplary work within the profession. YOU ARE WELCOME TO APPLY DIRECTLY FOR ANY OF THESE AWARDS at For Housing Directors/Executive Directors/Administrators NCAL ADMINISTRATOR OF THE YEAR This award recognizes an assisted living Administrator or Executive Director who demonstrates outstanding innovation, achievement, and capabilities in their provision of high-quality, person-centered care in a service-oriented culture in an assisted living community. For LPNs and RNs NCAL ASSISTED LIVING NURSE OF THE YEAR This award recognizes an assisted living nurse (must be a licensed practical or registered nurse) who demonstrates outstanding compassion, supervisory success, innovation, achievement, and capabilities in his/her provision of high-quality, person-centered care in a service-oriented culture in an assisted living community. For direct care staff from nursing, plus support/office staff including dietary, housekeeping, laundry, and maintenance departments NOBLE CAREGIVER IN ASSISTED LIVING This award recognizes frontline caregivers, support, and office staff from any department for their contributions that lead to improved quality of life for the residents and create a better work environment for the assisted living community s staff. This includes, but is not limited to, direct care staff from nursing (does not include licensed nurses), support/office staff, housekeeping, dietary, activities/recreation, laundry, and maintenance departments. For those who participated in 2017 National Assisted Living Week Programming NATIONAL ASSISTED LIVING WEEK PROGRAMMING National Assisted Living Week, established in 1995 by NCAL, provides a unique opportunity to bring together residents, families, staff, volunteers, and the community to celebrate residents and the services provided within assisted living communities. Nominees are evaluated on their successful incorporation of the previous year's National Assisted Living Week s theme into their activity plans and implementation resulting in outstanding special activities and community events for their residents, families, staff, and volunteers to enjoy.
