If someone deserves an award, they deserve a thoughtful nomination with specific details about why they are being nominated. 2. Browse through the categories and read the criteria carefully to ensure the best fit. 3. Gather all the information BEFORE you apply. Incomplete nominations are invalid. 4. Nominate. 5 Adult Volunteer of the Year Giving of oneself & helping others This award recognizes an individual volunteer or group of adult volunteers who have made contributions to the quality of life for residents/tenants/clients, family members, or to the working environment for staff. Eligible Nominees: All adults who volunteer at any member organization. 1. The volunteer s length of service, when and how they began volunteering at your facility 2. The volunteer s activities and contributions 3. The volunteer's impact and how it has improved the quality of life for residents/tenants/clients, family members, or the working environment for staff 4. 6 Aide Caregiver of the Year Shows fabulous caring, commitment, and dedication! The Aide-Caregiver of the Year Award honors a caregiver (NAR/HHA/TMA) who displays extraordinary skill in caring for individuals residing in a care facility or receiving care at home. The nominee communicates effectively and sensitively with residents/tenants/clients and family members; listens carefully; exhibits empathy and respect; and promotes dignity. Eligible Nominees: Any qualified employee (NAR/HHA/TMA) nominated by a nursing facility, assisted living/housing, corporate, or associate member. 1. 7 Champion Award Such an amazing long-term care supporter! The Champion Award honors an individual, family, or group who advocates for residents/tenants/clients or the care profession. Their words or actions have enhanced the public perception of the profession and have made a positive impact on the lives of those who live and work in these communities. Eligible Nominees: Any champion nominated by a nursing facility, assisted living/housing, corporate, or associate member.. 1. 8 Community Partnership Award Creating community bonds The Community Partnership Award recognizes the efforts of a Care Providers of Minnesota member to create a partnership with a community group or organization that is typically outside of the care industry. The partnership improves the quality of life for residents/tenants/clients, staff, and/or volunteers. The creation of community bonds may take the form of educational activities, cooperative ventures, creative collaborations with a vendor, or other initiatives. Eligible Nominees: All corporate, nursing facility, assisted living/housing, and associate members. 1. The partnership created by the nominee and how it started 2. 9 Dare to be Great Award Trying something different To do something different, with no guarantee it will work or pay off that s daring! The Dare to Be Great Award recognizes an individual or group that has had the courage to try something different in their delivery or provision of a program or service. It recognizes those who have tried something a little out-of-the-ordinary and as a result, enhanced the lives of residents/tenants/clients, staff, or volunteers at a member organization. Eligible Nominees: Any nursing facility, assisted living/housing, corporate, or associate member. 1. The daring adventure and what prompted the development of the new program or service 2. Steps taken to make the program or service a success 3. What were some of the challenges and how these were met or overcome 4. How it enhances the lives of residents/tenants/clients, staff, or volunteers 5. 10 Dedicated Service Award A career dedicated to caring The Dedicated Service Award recognizes any employee whose service over the course of her/his career, has positively enhanced the quality of life for individuals residing in a care setting. The individual s efforts demonstrate a commitment to their organization and the care-providing profession. Eligible Nominees: Any individual nominated by a nursing facility, assisted living/housing, corporate, or associate member. Award Criteria please describe the following 1. 13 Excellence in Assisted Living Award An opportunity to focus on all the things you do right! This esteemed award recognizes a housing with services/assisted living establishment for exemplary dedication and commitment to quality. Nominations are judged on criteria which is based on the National Center for Assisted Living s (NCAL) guiding principles for quality in assisted living: person-centered care, ethical practices, mission and vision statements, quality improvements, and workforce issues. There are two phases to the award: 1) a written application which consists of no more than 25 single-sided pages, and 2) a tour of the assisted living establishment IF applicant achieves a minimum score of 85% percent on their written application. Eligible Nominees: All housing members are eligible. Written Presentation: Submit written materials addressing the award criteria. Use Arial font [10 point] or Times Roman [12 point] on application; maximum amount of pages: 25 single-sided, double-spaced pages. Entry Deadlines: Presentations must be received by the Association or postmarked by 11:59 p.m. on May 3, Procedures Finalists are selected based upon a numerical point distribution assigned to each award criterion. If applicant achieves a score of 85% or better, they will proceed to the next step: a tour. The scores of the written portion and the tour will be combined to determine the award recipient. 14 Excellence in Assisted Living Award Nomination Criteria The written presentation criteria: 120 points possible. Assisted living establishments must achieve a score of 85% (a score of at least 102 points) to proceed to community tour. 1. GUIDING PRINCIPLE #1: PERSON-DIRECTED CARE (25 points) Person-directed care focuses on meeting an individual s needs and is based on the concept that employees and management know each tenant their history, needs, preferences, and expectations. With that in mind, please describe the following: How person-directed care occurs in your community. How management and employees have assisted the tenant and tenant s family members transition into life at the community. Include two recent (within the last 12 months) testimonials from a tenant and/or family member. 2. GUIDING PRINCIPLE #2: ETHICAL PRACTICES AND FINANCIAL STEWARDSHIP (25 points) NCAL and Care Providers of Minnesota believe that all providers should operate their assisted living communities based on a foundation of trust. Providers will operate their communities and provide service with integrity. Based on the statement above, please describe how services are provided in your community. 3. GUIDING PRINCIPLE #3: MISSION AND VISION STATEMENTS (10 points) A mission statement incorporates a community s purpose and values. A vision statement focuses on tenant, family, and employee satisfaction and strives for progress and continuous improvement while maintaining core values. Describe your mission and vision statements and how they are woven or integrated into practices and procedures at your community. 4. GUIDING PRINCIPLE #4: QUALITY IMPROVEMENT (10 points) Care Providers of Minnesota believe all assisted living communities should develop and implement a quality program that best serves the needs of the tenants and their families. Please describe the following: Give an example of one area of quality improvement including baseline data, measurement tools, the process used, and the demonstrated improvement Describe any participation in AHCA/NCAL s Quality Award program 5. GUIDING PRINCIPLE #5: EMPLOYEES (25 points) A higher level of employee satisfaction directly correlates with higher levels of satisfaction among tenants and their family members. Describe how the following are demonstrated in your organization: Ongoing training and education for all employees Recognition and rewards program for employees Employee satisfaction surveys Timely and concise communication from leadership Include 2 letters from (non-leadership) employees as to why your community is a great place to work 6. GENERAL (25 points) Describe the following: How your assisted living community integrates with the larger (surrounding) community Participation in Care Providers of Minnesota activities, initiatives, or governance (e.g. committees or boards, CARE-PAC, education programs, etc.) Involvement of leadership and employees in external professional groups Why your community deserves this award 14

As you tour delegates from the Recognition Committee around your community, please keep in mind the following: 1. The tour accounts for 50% of the total score 2. Staff and residents are to be included in the tour. 3. Please make sure you address each area outlined below. 4. If you have questions about the process and/or expectations, please contact Care Providers of Minnesota PRIOR to the tour. 16 Leadership Award Ability to motivate and inspire others This award recognizes the achievements of any employee, at any level of service, for her or his leadership contributions and skills. Eligible Nominees: Any qualified individual nominated by a nursing facility, assisted living/housing, corporate, or associate member. 1. Why this individual is valued as a leader in your organization 2. How the nominee teaches or demonstrates skills that are practical and relevant to the profession 3. How the nominee motivates and inspires others 4. How the nominee has influenced you and/or others to excellence 5. 17 Life Enrichment Award Person-centered care happens This award acknowledges the people, programs, and teams whose efforts enhance or enrich the lives of residents/tenants/clients and strive to make life more meaningful. The nominee (individual or team) empowers and supports individuals at member facilities/establishments through personal engagement. Eligible Nominees: All corporate, nursing facility, and assisted living/housing members. 1. How your nominee strives to make life more meaningful for residents/tenants 2. The specific program or service that enriches the lives of residents/tenants/clients and who it involves 3. The steps taken to enhance or encourage person-centered care 4. Impact on the residents/tenants/clients and employee(s) 5. 18 Nurse Caregiver of the Year Shows exceptional caring, commitment, and dedication This award honors a nurse who displays extraordinary skills in caring for individuals residing in a nursing facility, assisted living establishment, or receiving care at home. The nominee communicates sensitively with individuals who are in their care and their family members; the nominee listens carefully, exhibits empathy and respect, and promotes dignity. Eligible Nominees: Any qualified nurse (LPN/RN) nominated by a corporate, nursing facility or assisted living/ housing member whose direct care activities comprise 50% or more of practice. 1. How your nominee is an exceptional caregiver, exceeding what is expected of him/her 2. Specific examples your nominee s exceptional skill in caring for others 3. Examples of your nominee exhibiting empathy and respect 4. Instances of how your nominee promotes dignity 5. 19 Rising Star Award Shows exceptional promise and commitment This award recognizes the contributions of an employee from any discipline, who is new to her or his profession or to the industry within the last five years, and has made an impact in their years of service. The nominee should have a can do attitude, be a team player, and have made a positive impact on his or her organization. Eligible Nominees: Any qualified employee nominated by a corporate, nursing facility or assisted living/housing member. 1. About your nominee (i.e., who they are, job title) 2. 20 Service of Excellence: James B. Swanson Award James B. Swanson was a facility owner, corporate administrator, consultant, state and national industry leader, and activist. An innovative and inspiring professional, he encouraged his peers and colleagues to do their very best. The Service of Excellence: James B. Swanson Award is named in his honor, in recognition of his passionate commitment to the elderly and our profession. Competition Rules Nursing facilities applying for the 2013 Service of Excellence: James B. Swanson Award may only submit materials within the format and criteria established for the competition. Any nursing facility that has regulatory agency-imposed sanctions, substandard care, or immediate jeopardy, or receives such restrictions during the course of the competition, will be disqualified from competing. Past award recipients may not enter the competition for three years immediately following receipt of the award. Competition Requirements: Written Presentation: Submit written materials addressing the award criteria. (There is a limit of 25 single-sided pages.) Objectives The Service of Excellence: James B. Swanson Award is designed to recognize Minnesota s best nursing facilities. The award honors the contributions and commitment of all nursing facility employees and the important services they provide as they strive for excellence in the performance of their duties. Participating members are judged on criteria such as leadership, client satisfaction, person-centered care, and employee engagement. The competition provides the opportunity to educate the public about our profession and the high quality of the services we provide. It is intended to promote the long-term care industry s efforts toward excellence to public officials, to the general public, and within the profession itself. Procedures Finalists are selected based upon a numerical point distribution assigned to each award criterion. Finalists who achieve a score of at least 85% will proceed to the next step, a facility tour. The scores of the written portion of the submission and the tour will be combined to determine the award recipient. The finalist with the highest total point score will be named the 2013 Service of Excellence: James B. Swanson Award recipient. The name of the award recipient will be kept in strict confidence until it is announced during the award presentation event. (In the event there is only one finalist, the facility still needs to reach a certain score on the tour in order to receive the award.) KEY DATES FOR THE SERVICE OF EXCELLENCE: JAMES B. Please address the following criteria: A. 5 points Facility description and mission. a. Describe your physical environment, individuals served, services, philosophy, and goals. b. Include your mission statement. c. In 100 words (or less) explain why your facility is deserving of this award. B. 15 points Quality improvement process. a. Give an example of one area of an improved outcome including baseline data, measurement tools, the process used, and the demonstrated improvement. b. Describe any participation in AHCA/NCAL s Quality Award program. C. 10 points Leadership. a. (internal) Describe how your management team creates a positive work environment for employees, communicates organizational expectations and standards, and promotes excellence and quality improvement. b. (external) Describe how staff are engaged in professional leadership opportunities. D. 15 points Client satisfaction. Submit five current (within 12 months) testimonial letters that detail your customer service efforts and responsiveness to client s needs. Include the most recent Customer Satisfaction Survey results and plans for improvement. E. 5 points Community involvement. Describe efforts to create links with your community, but concentrate on one outstanding example. F. 15 points Employee engagement. a. Describe a creative/innovative program that came about as a result of an employee survey b. Describe one program that addressed teamwork and morale. G. 10 points Family engagement. Describe what is done to engage resident s family members into the facility. H. 10 points Volunteer involvement. Describe how volunteers enrich the lives of clients. I. 15 points Person-centered/directed. Describe the ways in which your facility is person-centered/directed. J. 5 points Association participation. Describe how your facility is involved with Care Providers of Minnesota activities, initiatives, or governance (e.g., serving on committees, boards, Care PAC). K. 10 points Letters of support. Include five letters from (non-leadership) employees stating why your facility is a great place to work. L. 5 points Past awards and honors. 23 Superstar Award Not just a star...a SUPER star The Superstar award recognizes an employee who not only does her or his job, but also does a whole lot of extras that enhance the quality of life for residents/tenants/clients and others.
