2 The Minnesota Association of Community Health Centers presents 2017 UDS TRAINING December 13, :30 AM 4:30 PM The Commons Hotel (aka The Graduate Hotel) SESSION DESCRIPTION The Uniform Data System (UDS) in-person training is a full day program covering the preparation of the 2017 UDS Report. The training addresses each of the report s tables, including a discussion of the changes that have been made and the definitions necessary to complete the Report. Deb Oleson will be our trainer again this year. WHO SHOULD ATTEND The UDS training is aimed at those who are responsible for gathering and reporting the data elements included in the UDS Report, as well as management and clinical staff who need to understand the definitions and concepts used, and those who use the data in their program management and quality improvement activities. REGISTRATION Complete the registration by November 28, See attached form or register online by clicking here. Lunch and UDS training materials are included in registration fee. 4 The Minnesota Association of Community Health Centers presents A ROADMAP FOR POPULATION HEALTH MANAGEMENT: GETTING STARTED WITH SOCIAL DETERMINANTS DATA December 12, :30 PM 4:30 PM The Commons Hotel (aka The Graduate Hotel) Health Centers have long recognized the impact the social determinants of health (SDoH) have on a patient s ability to be healthy. This workshop will help health centers start and make progress along the path of using social determinants data to address population health. We will build upon participants understanding of population health management and SDoH, and discuss their interrelationship and relevance for health centers. We will present case examples of health centers successful use of social determinants data to implement population health management and improve health. Participants will learn how to embark upon and make progress along the implementation journey using a Roadmap for Population Health Management, to guide them in the foundational steps of leveraging social determinants data to drive population health. The focus of this workshop centers on using SDoH data for HIT including (1) capturing and enriching SDoH data for PHM through screening and documentation processes and tools; (2) aggregating, extracting, analyzing and using SDoH data to drive QI and PHM; (3) using SDoH data for tracking and documentation; (4) and evaluating and managing improvement and population health through dashboards and reporting. KEY OBJECTIVES Describe PHM and SDoH and relevance for health centers. Discuss three ways that SDoH can be used by health centers to improve population health. Describe potential sources of data for SDoH and PHM. Understand methods, applications and challenges of aggregating, extracting and analyzing SDoH data to drive QI and PHM. Employ the Roadmap for Population Health Management for Health Centers to plan and create a plan to implement SDoH and PHM at their own center. Locate and access additional PHM and SDoH learning resources and tools to develop increased knowledge and competence.
