3 Review of the year Introduction Welcome to our review of last year. 2010/11 was our first year of operation following the successful integration of Portsmouth Community and Mental Health Services and Southampton Community Healthcare on 1 April It was a rewarding year which also brought new challenges and required so many to stretch even harder in their daily roles. We are pleased to share just a few of our successes in this publication. In fact, there are so many things to celebrate that it has been hard to select which ones to share in this way. We are delighted with what has been achieved in our first year in operation. As well as continuing to deliver excellent services to the communities we serve, we delivered against our strategic objectives and we achieved a modest financial surplus, in line with the financial plan we signed up to. Our success has been recognised as we secured Department of Health approval to create Solent NHS Trust from 1 April Dr Ros Tolcher Chief Executive Our successes have been made possible thanks to our staff who always work hard to ensure we deliver high quality, patient-centred services and provide great outcomes. It is inspiring to see so many staff going that extra mile to achieve such high standards. We are ambitious about our future as Solent NHS Trust, an aspirant Foundation Trust. We are looking forward to building upon our foundations, providing even better services for our patients and creating an even better organisation for our staff. We hope you enjoy reading about our work. Alistair Stokes Chairman Prior to the creation of Solent NHS Trust we were hosted by Southampton City Primary Care Trust and known as Solent Healthcare. 3

4 About our organisation Solent Healthcare was created on 1 April 2010 through the integration of Southampton Community Healthcare and Portsmouth Community and Mental Services. Last year we had an income of over 180 million, provided over 100 services to patients and service users and had over 1.6 million patient contacts. Our 2010/11 finances We are extremely pleased to announce that our finances, during our first year of operation, were strong. We exceeded our surplus target of 906 thousand. During the year, we received an income of over 180 million from our Commissioners and other sources. We are pleased to be able to deliver services at around the national average cost which is measured by reference costs, an annual national exercise where Trust costs are compared across the country. We have been working hard to deliver excellent, high quality services and do things better and more efficiently for less money. We delivered our 9.5 million savings target, 5% of our annual income. These savings will help to keep our costs competitive to ensure we remain a leading provider of healthcare and help our Commissioners invest in additional services to meet growing demand. Our 2010/11 objectives At the beginning of the year we set ourselves four objectives for 2010/11. These were: l To deliver business continuity whilst improving quality. l To deliver organisational and business transformation. l To fully establish the new organisation, Solent Healthcare. l To deliver a long-term organisational form for Solent Healthcare. 4

5 Review of the year Achieving our objectives We are pleased to have successfully achieved all four of our objectives. Our work throughout the year has included: Objective 1: To deliver business continuity whilst improving quality. l We implemented and delivered action plans for improvement against Care Quality Commission (CQC) standards. l We published our Quality Accounts to demonstrate to our stakeholders the quality of care provided by Solent Healthcare. l We implemented training for safeguarding adults and children. We also reduced the number of falls, pressure ulcers and urinary tract infections in our population. l We ensured there was a Patient and Public Involvement Strategy and forums available for our patients to attend. l We put a performance management system and reporting arrangements in places. l We delivered excellent services throughout a year of major change. Objective 2: To deliver organisational and business transformation. l We undertook an annual programme of internal and whole systems clinical transformation schemes. l We delivered a rolling programme of productivity reviews across several clinical areas. l We restructured our clinical structures to bring Southampton and Portsmouth services together. l We launched a three year programme to deliver corporate efficiencies as part of our cost improvement programme. 5

6 Objective 3: To fully establish the new organisation, Solent Healthcare. l We implemented an Organisational Development Strategy to ensure a high performing, positive and enthusiastic workforce. l We published a Communications Strategy to deliver effective communication to a number of audiences. l We ensured appropriate governance documentation, processes and systems were in place. l We developed a Board Assurance Framework. l We ensured that we had clear lines of executive level management and accountability for patient safety, clinical effectiveness, patient experience and clinical leadership. l We ensured appropriate policies, processes and systems were in place and worked to deliver joint policies for Southampton and Portsmouth. l We ensured contracts were in place and contractual processes were established. l We published a Business and Market Development Strategy. l We developed an integrated three year Information Management and Technology Framework. l We established robust procedures to comply with information governance requirements. l We implemented RiO, an electronic patient record system, in Solent East and worked on plans for the release of RiO in Solent West. l We produced an Estate Register and ensured leases and licences were in place. l We developed and implemented an Estate Environmental Improvement Plan. Objective 4: To deliver a long term organisational form for Solent Healthcare. l We established programme management arrangements for our journey to Foundation Trust status. l We implemented a Stakeholder Engagement Plan. l We developed our Business Case to become an NHS Trust which was approved by the Strategic Health Authority (SHA). l We developed a first year Business Plan which included: l programme management l business and market development l commercial and contractual l clinical and corporate governance l estates l finance l HR l information management and technology l stakeholder and communications. l We implemented the integration plan for our services. l We received Department of Health approval to become an NHS Trust from 1 April

7 Review of the year Some of our best bits Creating Solent Healthcare We joined together Southampton Community Healthcare and Portsmouth Community and Mental Health Services. This included successfully transferring approximately 2,200 former Portsmouth City Primary Care Trust staff to Solent Healthcare on 1 April 2010, creating a workforce of 4,200 people. Establishing an NHS Trust We secured Department of Health approval to create a new NHS Trust from 1 April This was a huge step in the right direction on our journey to become a Foundation Trust in Healthcare 100 The Health Service Journal (HSJ) crowned us in their top 100 health organisations, making us one of the best healthcare organisations to work for in the UK. Creating a Single Point of Access We set up a Project Team to deliver our Single Point of Access (SPA) a 24/7 point of contact for most of our services. In time, patients, carers, health professionals and partners will be able to use the SPA to: l access our services l make and reschedule appointments l monitor conditions through the use of telehealth equipment and provide a timely response l ensure patients receive the right care, in the right pace, with the best use of available resources. I really appreciate the team at Thornhill Rehab Centre for their kindness and patience in giving me my confidence back. 7

8 Best of Health Awards Two of our teams were runners up in the Best of Health Awards 2010, run by Portsmouth News in conjunction with The Coast radio station. The Continuing Care Nursing Team came second in the Community Team of the Year category whilst Dr Greenburg and Consultant Nurse, Colin Barnes and the Mental Health Team were awarded Mental Health Worker/ Team of the Year runners up. Other colleagues who made it to the shortlist included: Mental Health Worker/Team of the Year Jackie Ball, Mental Health Nurse at Cavendish House, Southsea Leigh Riste-Smith, Senior Mental Health Practitioner, Alcohol and Drugs Team, Kingsway House in Southsea Kingsway House Substance Misuse Team Martin Dunning and Shirley Mitchell who run the Day Treatment Service at St James Hospital, Milton Community Nurse/Team of the Year Jane Cameron, MS nurse, Turner Centre, St James Hospital, Milton Community Learning Disability Healthcare Team based at The Kestrel Centre, St James Hospital, Milton I want to say a huge thank you to the Wheelchair Service for sorting and organising my son s wheelchair. It is perfect for him. We won 7m of new business including: l Substance Misuse Services l Wheelchair Services l HMP Integrated Drug Treatment Service l Podiatric Surgery l Community Oxygen Assessment Service l Adult Mental Health Community Development Workers. We also won a range of services which enhance the patient experience and which contribute towards reducing hospital admissions and length of stay. Implementing RIO We successfully implemented RiO (a new electronic patient record system) across the majority of patient services in Portsmouth and South East Hampshire. The system is already delivering benefits by improving information and supporting staff to deliver clinical services more effectively. More effective communication across services and appropriate sharing of patient related information has led to improved quality of care. Using digital media for communication Through new media, we increased our presence on the World Wide Web. We launched social networking sites on Facebook and Twitter and a new website. We also successfully used this media as a marketing tool. 8

9 Review of the year Reducing Chlamydia We achieved two challenging national targets. Our Chlamydia Screening Teams in both Southampton and Portsmouth screened the required number of young people aged 15 to 24 years for Chlamydia, the most common sexually transmitted infection in young people. Our Portsmouth Chlamydia Programme formed part of the 5th most successful screening area in the country. Eliminating mixed sex accommodation We eliminated mixed sex accommodation across all sites. Nurturing our talent 102 staff achieved National Vocational Qualifications. Supporting local young people We teamed up with other local organisations to provide workplace training for young people out of work. The Southampton Skills Development Zone (SSDZ) is a city-wide project which offers workplace training and education. The SSDZ apprenticeships are aimed at helping people get the right training to get into full time, permanent work. Welcoming new staff We introduced a revised corporate induction for new staff. The first day of the new look programme features a welcome from Dr Ros Tolcher, Chief Executive or nominated Director, and an introduction to our values and business objectives. Our approach to providing an excellent customer experience is also included in day one as well as equality, diversity and human rights, staff engagement and culture and values. Day two is dedicated to the promotion of health, wellbeing and safety. 394 members of staff attended the revamped course in 2010/11. E-rostering We introduced e-rostering into the organisation. This allows staff to log hours on an electronic system, saving time and increasing productivity. Healthy School Meals The Healthy Schools Team, part of the Health Promotion Team, joined with Southampton City Council to offer Year Five pupils from schools in Southampton the chance to design their own school meals. The winning dishes included a delicious chicken and vegetable casserole and a heavenly bread and butter pudding. The winning meals now feature on the school menu. 9

10 Encouraging volunteering We encouraged people from the local community to volunteer with us. During Volunteers Week, staff from the Patient and Public Involvement Team manned a stand at the Royal South Hants Hospital. They spoke with members of the public and patients about opportunities to volunteer with us. Working with local businesses to help people quit smoking Southampton employees of banking giant, HSBC, took advantage of free NHS professional help to stop smoking. 19 HSBC employees based at Norwich House, Nelson Gate, took part in an eight week workplace smoking course run by Southampton Quitters Team. This was just one of many initiatives which contributed to the Team achieving their target to support over 740 people to quit smoking. Inter-professional learning comes to Solent Healthcare We hosted over 100 final year students from the Health and Social Care Schools at the Universities of Southampton and Portsmouth. Based in multi-professional groups, the students undertook nine different service development projects in the following services: Child and Adolescent Mental Health Services (CAMHS), Community Nursing, Children and Families, Health Promotion, Learning Disabilities, Adult Mental Health and Learning and Development. The students produced nine business cases which have been used to improve our services. I would like to say how fantastic the Children s Sleep Disorder Service is. My son was referred to them earlier in the year. I attended a workshop and found it inspirational and gave me the confidence to make changes to help my son. Minor Injuries Unit We opened our first Minor Injuries Unit (MIU) at the Royal South Hants Hospital to help improve unscheduled care services and to relieve pressure on the Emergency Department. During their first year in operation, staff at the MIU saw 32, 582 patients. Health event for local people To raise awareness about the services available to them, the Thornhill Healthy Living Centre Team in Southampton held a health event for local residents. Many services from across Solent Healthcare attended the event to speak with members of the public. 10

11 Review of the year Praise for The Limes The Limes, a 36-bed unit for older people experiencing conditions such as Dementia and Depression, was visited by renowned dementia expert, Professor Dawn Brooker. Prof. Brooker described it as a great facility and the wards one of which is named after her as a testament to the passion for improving the lives of older people. Involving patients The Speech and Language Service in Southampton asked patients for their feedback on the service. All patients discharged from the service were asked to complete a questionnaire. In addition, two successful patient and carer focus groups were held. Students from Reading University helped patients with communication difficulties. As well as constructive comments for service improvement, compliments were also received. Improving facilities for Homeless Healthcare Service The Two Saints Day Centre, which houses the Homeless Healthcare Team in Southampton, received a facelift. Thanks to the renovation, the team is now able to offer many complementary health services including Acupuncture and Osteopathy. The new layout has also helped to provide a welcoming space to improve the experience for patients who are marginalised by society. Integrated Drug Treatment Services We launched a new drug treatment system for offenders at Winchester Prison. The service increases the range of options to drug and alcohol services available to prisoners, in turn reducing levels of reoffending when released. Increasing access to Physiotherapy Services The Physiotherapy Service at the Queen Alexander Hospital in Portsmouth now offer a seven day service. I would like to say thank you to the Out of Hours Service for an excellent service. The doctor that visited me was fantastic and very professional. They explained everything to me and I feel so much better since seeing the doctor. 11

12 Supporting patients with breathing problems We set up a new Breathe Easy Support Group for the residents of Portsmouth and Eastleigh. The Breathe Easy Support Group Network provides support and information for people living with a lung condition, and for those who look after them. This provides the patients with a service which was not available before and enables them to talk to other people suffering with lung problems. Securing a grant to support staff Our Clinical Psychology Team secured a grant from the Department of Health s Children s Palliative Care Fund, to work with Southampton University Hospitals NHS Trust to provide clinical supervision training to all nursing staff across the Children s Oncology and Haematology Service and the Paediatric Intensive Care Unit. The main aim of the project is for all staff, whether newly qualified or very experienced, to learn and practice skills to be able to reflect on their clinical work, and to train a group of supervisors within each service to facilitate group supervision sessions. Research from across the UK and Europe has demonstrated a number of benefits of clinical supervision in improving patient care, increasing job satisfaction and staff morale, and in reducing sickness absence. Bringing health services to people in supported housing We won acclaim from leading medical research charity, The Bupa Foundation for improving access to healthcare for people living in supported housing. People in supported housing often have health and social care needs and seeing a nurse regularly can be an important part of maintaining their health and wellbeing. Our Community Matrons have converted a communal lounge into a health club for local residents. Thanks to the new lounge, symptoms are recognised more quickly and at risk patients can be more closely monitored. I would like to thank everyone at Podiatry for the help and support they gave me. 13 Review of the year Reducing length of stay and admissions to hospital We achieved our admissions avoidance target and achieved a 30% reduction in length of stay in community hospitals. We also reduced delayed transfers of care from an average of 13 to 5. This is the equivalent of 12 extra beds or 2 million cash savings. Most importantly, people were back home much more quickly. To achieve these targets we: l Extended our Emergency Department Older Persons Support Team based at Southampton University Hospitals NHS Trust to avoid more hospital admissions than ever before. This success has been replicated at Queen Alexandra Hospital with the support of both Health and Social Care Locality Teams. l Introduced Ambulance Anticipatory Care Plans for all vulnerable and frail elderly case managed patients linked to our Single Point of Access. The Ambulance Service are able to access the plans and are therefore better able to avoid admissions to hospital by engaging the right service. l Introduced personal care plans for vulnerable and frail elderly case managed patients. These ensure people are provided with the right care and by the right health professional, leading to avoided admissions and early discharge from hospital. Digital pens Staff across Paediatric Asthma, Paediatric Diabetes and Community Children s Nursing Services in Southampton trialled digital pens. The pens allow staff to complete on the spot notes without having to return to base. This enables the service to operate much more in real-time and makes the data recorded available to all staff. It also reduces the time taken for the service to compile this information for reporting requirements. The pens work by capturing the user s handwriting. It does this by recording an image of the form the user completes. The data is then transmitted to the user s Blackberry where it is forwarded to the CaRS database. 13

14 Launching the C-card We launched the C-card scheme, one of the many initiatives aimed at improving young people s sexual health and preventing unwanted pregnancy. C-card is a card for young people aged to help them to get free condoms at many locations across Southampton. NHS Innovations South East Award We were presented with an award from NHS Innovations South East for the New Forest Parenting Programme project for their research and innovative approaches to helping children and young people with Attention Deficit Hyperactivity Disorder. Ultra light Notebooks for mobile clinical working Staff members in Health Visiting and the Substance Misuse Service have been provided with ultra light, secure and robust Notebooks with 3G connectivity. This enables staff to use electronic patient records for real time data capturing. Virtual wards We set up 11 virtual wards across our two cities. Virtual wards help reduce hospital admissions by managing patients more effectively in the community. A virtual ward provides the strengths of a hospital ward in the community by using a multidisciplinary team approach. There are now five wards in Portsmouth and six wards in Southampton. Overall 1,300 vulnerable adults are being closely monitored in these virtual wards. Merging Out of Hours Services in Portsmouth and West Hampshire We merged our Out of Hours Services in Portsmouth and West Hampshire to create Solent Out of Hours Services. The integration bought together best practice to ensure the service meets the needs of patients. 14

15 Review of the year I would like to say an immense thank you to all of the Cardiac Rehabilitation Team for putting me through your course over the last six weeks. It has been hugely beneficial to me, I feel so much fitter, far more confident and aware of my capabilities. Celebrating PEAT results We received fantastic results in the annual Patient Environment Action Team (PEAT) assessments. Published by the National Patient Safety Agency, the assessment looks at the patient environment, standards of food and privacy and dignity. Both the Royal South Hants and Western Community Hospitals in Southampton achieved excellent in all three categories. Whilst St James Hospital, Jubilee House and St Mary s Hospital scored highly in all three categories with food rated as excellent at all three sites. Official opening of The Limes Celebrated actor and Chancellor of the University of Portsmouth, Sheila Hancock CBE, officially opened The Limes. The Limes is an 8 million unit for older people with mental health difficulties and is situated at St James Hospital, Portsmouth. The unit has 36 beds for older people experiencing conditions such as dementia and depression. Department of Health commends us We were commended by the Department of Health for our well-developed victim focussed service following their visit to our Sexual Assault Referral Centre (SARC), Treetops. Child and Adolescent Mental Health Services are excellent Our Child and Adolescent Mental Health Service (CAHMS) were rated as excellent under the Royal College of Psychiatrists Quality Network for Community CAHMS. The Portsmouth service for children and adolescents with mental health difficulties are the only service in the country to have received the accreditation. 15

16 I would like to say a huge thank you to the Rapid Response Team for the input and care they provided to my mother. Everyone at the service was brilliant and we are very impressed with the level of care provided. Whenever I contacted the service they were very polite and helpful. They are a very professional service. Helping to reduce falls in older people The Falls Prevention Team received a fantastic 500 referrals during its first 18 months. This service has helped improve older people s lives by helping to keep them on their feet. Queen s Nurse Improving access to Sexual Health Services We launched Southampton s revamped Sexual Health Services, offering a more accessible and convenient integrated service. Contraception and Sexual Health (C&SH) services and Genitourinary Medicine (GUM) services are now available together at four convenient, community based locations. Health Visiting Nurse, Frances Allen, joined an elite band of healthcare professionals carrying the title Queen s Nurse, which she received in recognition of her sterling work for the NHS. Getting out and about screening for Chlamydia The Sexual Health Team in Portsmouth and South East Hampshire hit the road in a portable bus for the summer months in a bid to tackle rates of Chlamydia, the sexually transmitted infection (STI) affecting 1 in 14 young people in the area. 16

17 Review of the year Emotional First Aid goes national Emotional First Aid, a training kit to help professionals support young people through emotional issues, was rolled out nationally. The training course is for adults who want to help young people dealing with emotional distress. The programme started in 2008 but has been developed with the help of NHS Innovations South East. Speaking with the public at the King s Speech Speech and Language Therapists attended the launch of the award winning film, The King s Speech. The team spoke with cinemagoers in Portsmouth about communication difficulties and how the service can help. Providing easily accessible information I want to say a huge thank you to the Wheelchair Service for sorting and organising my son s wheelchair, it is perfect for him. We developed and launched (with the support of the Learning Disability Partnership Board) a new accessible information website. The website includes information and resources to help professionals, carers and service users produce information in formats that are more accessible to people with a learning disability. Helping people with Learning Disabilities We successfully facilitated the transfer of NHS campus services in Portsmouth. As a consequence, we no longer have any people with a learning disability living within the NHS. As patients, people with a learning disability lived within NHS Services and had no tenancy rights but now they live as tenants, with increased rights, in their own homes. Their homes have been adapted to improve their living conditions and tailored to meet their individual needs. 17
