2 Introduction Woking Hospice was set up 20 years ago. From that early beginning, it has developed to become a local centre of excellence, as is the case with all Hospices in the provision of palliative care. It has always enjoyed a close working relationship with the local NHS. Sam Beare Hospice was previously a NHS run palliative care unit. It is based at the Weybridge Community Hospital. From July 2006, Woking Hospice took over the management of the Sam Beare Unit and the two hospices are now run as one. The Hospices provide specialist palliative care to both cancer and non-cancer patients (motor neurone disease, Parkinson s, end stage cardiac failure, dementia etc.). In addition, three beds at each hospice are available to the NHS Continuing Care Team for patients who may have prognosis of months, but their specialist palliative care needs are such that it would be difficult to care for them in other settings. The two Hospices are 7 miles apart, based in the towns of Woking and Weybridge. Woking has 10 beds; Sam Beare Hospice has 10 beds. Both Hospices have a community team supporting individuals in their own homes. Woking Hospice acts as the centre for the fundraising, retail operations, volunteer management, finance and administration teams.

3 The two Hospices serve a patient population of around 360,000 covering all or part of the boroughs of Woking, Surrey Heath, Guildford, Elmbridge, Spelthorne, Runnymede, Walton and Weybridge. Hospice care is based on the belief that each person is more than their illness, and that each of us has unique physical, emotional, social and spiritual needs. Woking & Sam Beare Hospices respond to the needs of each individual, helping people achieve the best possible quality of life. Key facts for delivering care in the local community ( ): In the UK, one person in three has been touched by Hospice care since the first modern hospice was founded in million is required every year to keep our Hospices open. We receive 27% funding from the government Through our doctors, in-patient care and our community team we provide care and support 24 hours each day, every day of the year The number of patients being looked after in their home rose to over 1,260 Our Community Nurse Specialists had over 4,000 patient contacts, including telephone support and home visits. Patients admitted into our in-patient units for symptom relief, respite and end of life care and support was 380, providing almost 5,700 days of patient care Our day care units welcomed 2,200 patient attendances during and arranged 344 different themed sessions Patients attending day care can also access treatments including physiotherapy and occupational therapies, complementary therapies, physical care and clinical Our counselling services provided almost 2,000 hours of one-to-one sessions to our adult clients and over 200 sessions for children

4 A new future for Hospice care in the heart of Surrey In 2017, we will be opening a state-of-the-art Hospice in Woking and have launched a 3.4 million Capital Appeal to raise the required funds. We will be calling on individuals, companies, groups, legacies, trusts and grants to ensure a new future for Hospice care will be available to all who need it. Build

5 Our Care - Medical Our team of doctors, along with our Clinical Services team, plays a leading and integral role not only in individual patient care, but also in the on-going development of patient centred medical practices and innovation, and in the strategic direction of the Hospices. Our doctors see patients wherever they are in their homes, our in-patient wards, as they visit our day care units or visit the Hospices for out-patient support. They are also closely involved during patient referrals, assessing and advising on patient treatments and endeavouring to ensure patients are in the best location to receive their care. Working closely with our neighbour hospice, the doctors ensure that specialist out-of-hours advice can be sought 24-hours a day, every day of the year. Complementing their Hospice work, our doctors work closely with others in the healthcare sector including the medical teams of local Acute Trusts. The doctors also provide training for General Practitioners and Registrars who wish to specialise in palliative medicine and, related to this, have extended their reach internationally including having developed a close relationship with Maastricht University. During the year the Hospices welcomed several GPs in training from the University on secondment, receiving thanks for the invaluable contribution the Hospices have made to developing their expertise in palliative care. To assure patients, the public, employers and other healthcare professionals that the skills of licensed doctors are up to date, complying with the relevant professional standards and fit to practise, the General Medical Council (GMC) have introduced a process by which they will confirm the continuation of a doctor s licence to practice in the UK. The process is called revalidation and once validated each licensed doctor will have to revalidate every five years. Under this process Hospices doctors are undergoing regular appraisals and are being revalidated in line with the GMC schedule.

6 Our Care - Community Team The community team cared for over 1,200 patients during the year, an increase of 12% on previous numbers, with the average time spent with each patient increasing as treatments become more complex. In addition, the team became more proactive in providing palliative care training to others in the healthcare sector. The Community Nurse Specialists offer a Rapid Response service where patients are seen within 24 hours of referral and visited within two hours if their acute symptoms suddenly worsen. February 2014 saw the launch of a night sitting service with our Health Care Assistants providing care through the night for patients, respite for carers, and giving confidence and security to both. Well managed Hospice at Home care reduces the need for traumatic and unscheduled emergency hospital visits and enables more patients to die at the place of their choice. Our Care - In-patient units The Hospices have 20 in-patient beds split equally over two sites in Woking and Weybridge. Both units provide specialist care 24 hours a day, seven days a week. Bedrooms at both Hospices open onto gardens tended by our volunteers and we have facilities for family members, friends and carers to stay overnight. In the past, Hospice care focussed primarily on patients with cancer. Today our Hospice care has expanded to treat other life-limiting illnesses including chronic obstructive pulmonary disease, motor neurone disease, Parkinson s disease and multiple sclerosis. We admit our patients for a number of different palliative care reasons including symptom control, end of life care, respite, psychosocial and spiritual distress and occasional palliative rehabilitation. Over 50% of our patients are discharged after a short stay. We gave personalised, specialist care to over 430 people during the year. Representatives from the Care Quality Commission (CQC) visited the Woking Hospice for a routine inspection on 31 December Reporting on the visit in January 2014 the CQC, evaluating the performance of the Hospice against its five key criteria, found that the Hospice was meeting its standards for each. The report said that people were cared for in a safe environment and their welfare was promoted through the maintenance of clear and detailed plans of care.

7 Day Care Units Our day care units play a pivotal role in providing holistic, integrated and coordinated palliative care. They are the bridge between our community and the full Hospice portfolio of services, and a vital part of the end of life journey for people who are able to live at home, but need additional help and support. Our two units had over 2,600 patient visits during the year where, in a caring, safe and welcoming environment, people benefited from social interaction and support. We provided a wide range of therapeutic activities, attention from physiotherapists, occupational and complementary therapists, clinical monitoring and routine medical treatments, health and spiritual care, education and advice. During the year we sought feedback from our patients. Across the two units 97% of patients surveyed rated the overall care they received as excellent or very good. During the year the Hospices also introduced a rolling six-week educational programme for patients and carers. Counselling Meeting a growing need for patient and bereavement counselling, we continued to develop our team during the year. Our 30 counsellors, 26 of whom volunteer their services, saw an increased demand for all their activities including increases in child and adult one-to-one sessions of 17% and 14% respectively. In total the team delivered over 2,000 hours of counselling, providing support to people when they need it most.

8 Spiritual Care Our Spiritual Care team are fully integrated within the clinical and medical leadership of the Hospices. The team have continued to provide spiritual and religious care and support to those patients or staff requesting it. Valuing diversity, our team provides an invaluable service meeting the diverse spiritual care needs of today s modern society. Therapies The Hospices believe all patients with life limiting illnesses should be able to benefit from a common, integrated and holistic approach to palliative care. Physiotherapy, Occupational and Complementary therapies, as part of a portfolio of care are considered beneficial to the treatment regime and support improving quality of life. Benefits of these therapy treatments in palliative care include the psychological benefits of exercise; preserving independence; work simplification; fatigue management; relieving stress from symptoms; enhancing quality of life; educating external audiences, patients and carers; symptom control; and discharge planning. Working in the day care units, with the community teams and with in-patients, our Therapies team carried over 470 new assessments and more than 2400 follow up contacts.
