2 Foreword Dear Colleague, Welcome to our first ever GP Service Guide, which we have produced to help you refer patients into our mental health services across Dudley. You may be aware that we have recently made changes to our Adult Community mental health services. This guide contains everything you need to know, including up-to-date information about our services, details on referral criteria and key contacts. Please keep this guide handy. It has been designed so that, as information about our services changes or is updated, we will send you a new page that can be easily inserted into the ring binder. 3 Who we are and what we do Our Vision Better Together - delivering flexible, high-quality, evidence based services to enable people to achieve recovery. Dudley and Walsall Mental Health Partnership NHS Trust was established in October 2008 and specialises in the treatment of both common and complex mental health conditions in children, adults and older people. It provides a comprehensive range of mental health and social care services including primary care mental health, community based services and inpatient facilities. In 2009 the Trust was selected by the National Commissioning Group to be one of four centres in England to deliver a national Deaf service for children. Serving a population of approximately 306,600 in Dudley and 255,900 in Walsall, services are provided from a number of sites across the boroughs. Most are owned or leased by the Trust; some services are provided from within GP practices, health and social care centres and other community venues. The Trust also provides services directly to people in their homes or in residential and care homes. In the past four years the Trust has established itself as a provider of high quality, safe and effective mental health services for the populations of Dudley and Walsall. This is evidenced by unconditional Care Quality Commission (CQC) registration, NHS Litigation Authority (NHSLA) compliance, Accreditation for Inpatient Mental health Services (AIMS) and successful performance in a variety of regional quality, safety and governance assessments and evaluations. The Trust s vision is one of providing recovery oriented services. This vision has been guided by a growing emphasis on the wellbeing of the population and a focus on prevention and early intervention. It guides our future plans for mental health services. 5 1. Referral Pathway to Mental Health Services Dudley Is the patient presenting with a mild to moderate mental health issue of low risk? Yes Enhanced Primary Care Referrals can be made to the Enhanced Primary Care Service Monday to Friday 9am - 5pm. Please give the patient the Enhanced Primary Care information leaflet to self-refer to the service and book an appointment within the GP surgery. 6 2. Enhanced Primary Care Service What is the Enhanced Primary Care Service? The Enhanced Primary Care service offers access to primary care based mental health screening, assessment and interventions. The team provides a flexible and innovative service that meets the various needs of adults aged 16 years and upwards presenting with mild to moderate mental health problems associated with low risks and care clustered within the range 1 to 4. Who can refer to this service? Referral to the Enhanced Primary Care service can be via GP referral or self-referral. Consent may be gained to discuss the case with their GP. How to make a referral For a self referral, please issue the service user with a patient information leaflet which provides information about the service and how to make an appointment. If the referral is via the GP please use the Enhanced Primary Care referral form. 8 3. Early Access Service What is the Early Access Service? The Early Access Service (EAS) delivers a single point of entry into secondary mental health services for all adults (17 to 65) and provides mental health screening and comprehensive mental health assessments. Who can refer to this service? The service accepts referrals for patients presenting with a moderate to severe mental illness made by: GPs Mental Health Act Mental Health Primary Care Teams Patients Transferring from CAMHs New transfers from other mental health trusts The Early Access Service is commissioned to accept referrals for patients presenting with a moderate to severe mental illness. Patients who, following assessment, do not meet the criteria for secondary mental health services will either be signposted to other services or referred back to the referrer. How to make a referral To make a referral please write to the address below detailing the presenting mental health problems and a short medical history of the patient. The service will aim to offer the patient a routine appointment within 15 working days from receipt of the referral. Urgent Referrals If the referral is urgent and a same day response is required, please ring the service number ( ) and discuss the case with the Duty Worker who will support the appropriate response process. (Please note posted referrals will not be classed as urgent).

In such circumstances EAS will contact the GP and seek authorisation to proceed. The service will aim to acknowledge receipt of all referrals and advise the referrer of the appointment date offered to the patient. The service will write to the service user s GP following assessment detailing outcomes and any initial plan of care. Older Adults Service Community Mental Health Team for Older Adults The Community Mental Health Team for Older Adults offer assessment and a range of interventions for all adults over the age of 65 presenting with mental health symptoms, and those diagnosed with dementia (including working age dementia). They help support people with a wide range of mental health symptoms and associated risks. The aim of the service is to make assessments, offer treatment and support service users in the community towards recovery and enablement. Who can refer to this service? The service provides a gateway function for all secondary Older Adults Mental Health services and can be accessed via referrals from GPs and social care professionals. How to make a referral To make a referral please write or fax a referral letter to the contact details below detailing the presenting mental health problems and a short medical history of the patient. 11 Once the team has received the referral form they will contact the patient or carer to arrange an assessment appointment. If you are unsure if a referral to this service is appropriate please call and ask to speak to the Duty Officer for Older Adults Services who will be able to advise accordingly. Urgent referrals Monday - Friday 9am to 5pm should be discussed with the Duty Officer (team contact details below), where this will be screened and appropriate response undertaken by the team. 12 5. Child and Adolescent Mental Health Service What is the Child and Adolescent Mental Health Service (CAMHs)? The service offers a multi-agency and multi-disciplinary service specialising in assessment, therapeutic interventions and consultation of moderate to severe mental health difficulties to children and young people up to and including Year 11 in school. Our team of child and adolescent mental health specialists cover a wide range of skills and include Child and Adolescent Psychiatrists, Child & Adolescent Psychotherapists, Clinical Psychologists, Nurses, Specialist Social Workers, Family Therapists and Speech & Language Therapists. If you are unsure whether Child and Adolescent Mental Health Services are appropriate please do not hesitate to ring the service number ( ) for further guidance. Who can refer to this service? The service accepts referrals for patients presenting with a moderate to severe mental illness made by: GPs Paediatricians Social Workers School Health and Youth Offending services It is important that front line services, such as counselling services, mediation services, family centres and bereavement services are tried first before a referral is made to the CAMH Service. How to make a referral Please make a written referral to the address below detailing the presenting mental health problems and details on any previous interventions including the outcome.

Substance Misuse Services Rose Cottage Community Detox Team (CDT) Rose Cottage CDT is a community drug and alcohol treatment service (Adults 18 +) in Dudley assisting individuals and families with complex need in their recovery from chemical addictions. The teams undertake a number of dedicated functions: Drug & Alcohol Liaison Team (DALT) - Provides advice, consultancy and professional training to staff in the Emergency Department and the wards at Russells Hall Hospital. Community Alcohol Team - Provides medically supervised community detoxification as well as ongoing support in more complex alcohol dependent cases. Drug Treatment Team - Provides structured drug treatment interventions, ranging from psycho-social and medical prescribing to more complex cases. Who can refer to this service? Drug and Alcohol Liaison Team - Referrals are accepted via the respective medical teams only at Russells Hall Hospital. Community Alcohol Team - Historically GPs could make direct referrals to the Community Alcohol Team at Rose Cottage. However new commissioner led arrangements require GPs to refer to the Single Point of Contact (SPOC) at Aquarius in Dudley. Following screening SPOC will refer appropriate cases to the team. Drug Treatment Team - Referrals are accepted from Atlantic House, Stourbridge only. The service will write to the service user s GP following assessment detailing outcomes and any initial plan of care.

16 7. Out of Hours Crisis Service This service operates out of hours during the evenings, weekends and bank holidays in both the Dudley and Walsall localities. If a person experiences a mental health crisis out of hours and the GP believes a mental health assessment is required then initially the Crisis Team should be contacted by calling and the crisis worker will advise on the way forward. The same process is to be followed if a Mental Health Act Assessment is required, again the crisis worker will advise on appropriate assessments, treatment and access to services if appropriate.

Compliments, Complaints and Enquiries about Our Services The Service Experience Desk (SED) is the central point of contact for all concerns and enquiries from service users, families, carers and all healthcare professionals, whether these are formal or informal, compliments, suggestions or complaints. Compliments, concerns and complaints can be made directly to staff or to the Service Experience Desk. The Service Experience Desk will liaise with staff managers and health professionals to handle concerns and sort problems as quickly as possible. The team also provides information on NHS services and advice and support to service users, their families and carers.
