1 This FAQ will help Version 3 and Version 4 clinical users who wish to: Change the interventions for an Omaha problem; or Change the Omaha problems that are on the care plan. This FAQ assumes that you have already set up a care plan and are in the process of documenting interventions when you realize the need for a change. Adding An Intervention to the Current Visit 1. In the Visit tab, tab or scroll down to the Plan of Care Interventions list. 2. Double click on the desired Omaha problem. 3. INSIDE the Omaha problem, place the cursor in the Interventions list and right click new. Right click new in Interventions list INSIDE the problem to add an intervention for the current visit. 4. Now you have a screen that allows you to create a new intervention, chart the results, and decide whether to add it just to the current visit or for future visits too. 2 Adding An Intervention to the Current Visit (continued) Click Done if you acted on this intervention during the current visit; then chart results in Care Status Note. Select Category, Target, and Care Description to create new intervention. Check This Visit Only if you do not wish the intervention to appear in future visits. Check Add to Care Plan for the intervention to appear in future visits. Modifying Interventions for Future Visits 1. If you wish to add, revise, or discontinue interventions for future visits, click the Modify Care Plan button inside the Omaha problem. Clicking Modify Care Plan affects ONLY FUTURE visits. 3 Modifying Interventions for Future Visits (continued) 2. The list of Interventions will appear. 3. To add a new intervention, place the cursor in the list and right click new. You will see a screen that allows you to create a new intervention. 4. To revise an existing intervention, double click on it. a. Then type changes in Note field as needed. Type changes in the Note field as needed. b. You will be prompted Do you wish to add changes to current visit? Clicking yes will change the intervention for the current visit. It is the only aspect of Modify Care Plan that affects the current visit. 5. To discontinue an intervention, place the cursor on it and right click DC. 4 Discharging an Omaha Problem 1. In the Visit tab, tab or scroll down to the Plan of Care Interventions list. 2. Double click on the desired Omaha problem. 3. INSIDE the Omaha problem, click the Discharge Problem button. Click the Discharge Problem button to rate the patient outcomes and remove the Omaha Problem from the Care Plan. 4. You will be prompted as shown below. Click Yes if you wish to proceed. 5. Enter the Discharge Ratings and the Discharge Note. 5 Adding an Omaha Problem to the Care Plan 1. In the appropriate visit, tab or scroll to the Omaha Assessment list towards the top of the Visit tab. 2. Place the cursor in the list and right click new. The Assess Problem list will appear. 3. Double click on the desired Problem. 6 Reassessing an Omaha Problem 1. In the appropriate visit, tab or scroll to the Omaha Assessment list towards the TOP of the screen. 2. Place the cursor in the list; right click new. The Assess Problem list appears. 3. NOTE THAT PROBLEMS WHICH HAVE ALREADY BEEN ASSESSED APPEAR IN BOLD. IN ORDER TO CHANGE THE ASSESSMENT INFORMATION, YOU MUST RIGHT CLICK REASSESS. BOLD problems have been assessed before. Right click reassess to change assessment information. 4. Place the cursor on the desired problem and right click reassess. 5.
