1 Creating A Patient Portal Link From More Patient Button Go to More Patient and click on the Export PHI tab. From this tab, click on Create Patient Portal Link. Note: Allow Internet Based Delivery Of Reminders Must Be Set To Yes. address optional (recommended in order for patients to retrieve username/reset password)

2 Creating A Patient Portal Link From Appointment Calendar Marking a Patient Here on the Appointment Calendar will prompt user to create a Patient Portal Link if patient does not yet have one.

3 Creating A Patient Portal Link Click Create to establish the patient portal link for patient. Portal Access instructions can be ed, printed or both printed and ed. The PIN is the patient s 4 digit birth year. User must check I confirm that the above information is accurate and click Create after choosing option from dropdown. By clicking Make Changes, user can update the patient s demographic information.

4 Patient Portal Pending Link After Portal Instructions have been generated to patient, the Authorization Code generated shows along with the expiration date (valid for 3 days). Will be Pending until patient completes the verification process with the authorization code and creates a Portal Login. Clicking Provide Instructions Again allows office to resend instructions to patient. *Note: Until patient has activated their link, print and mail clinical summaries to the patient or send them through .

5 Pending Patient Portal Link Message While Patient Portal Link is Pending, Patient Chart will have pending link message at bottom of chart.

6 Creating A Patient Portal Account Patient will receive these instructions via or printed copy. Patient Instructions

7 Click More Patient and choose Export PHI tab. Message shows Patient has an Active Portal Link. Information can now be uploaded to the Patient Portal. Options to Disable Patient Portal Link or to View Patient Portal Documents. Active Patient Portal Link

8 Pending Patient Portal Link Message After Patient Portal Link is Activated, Patient Chart will have active link message at bottom of chart.

9 Disable Patient Portal Link From Export PHI tab, Click Disable Patient Portal Link. Then click yes to the popup to disable a Patient Portal for a patient.

10 In-Active Patient Portal Link For an In-Active Portal Link, Click Enable Patient Portal Link to reactivate the link to the patient s portal account.

11 Re-Activating Patient Portal Link Click Yes to popup confirming reactivation of Portal Link.

12 To have patient instructions from Sammy display your practice website, go to System Setup, Patient Portal Settings. Then choose Custom Settings. Enter your website address and any instructions to find the link on your website. Custom Website Link

13 Sending Clinical Summary to Portal To send clinical summary to the patient s portal account, click Send To Portal. Sending the summary through the patient portal within 3 business days of the visit will satisfy the requirement for Measure 13: Clinical Summaries. *Note: Continue to print and mail clinical summaries to the patient or send them through until the patient has activated their portal account and the link shows as an active link.

14 Clinical Summary Sent to Patient Portal You will then receive a notification that the summary was uploaded to the portal.

15 Sending File Cabinet Documents to Portal To upload a file cabinet document to the patient portal in order to allow the patient to view the document online, highlight the document in the patient s file cabinet and click Upload to Patient Portal.

16 Accessing The Patient Portal To login to the patient portal, patient goes to and enters their username and password. To set up an account for the first time, the patient clicks create new account.

17 Creating a New Patient Portal Account To create an account after provider has issued instructions, patient clicks I have an Authorization Code from my provider.

18 Creating a New Patient Portal Account Patient then enters the authorization code (from the printed instructions/ generated to patient) and the PIN (Birth Year for the patient).

19 Creating Patient Portal Account Patient must then verify information is correct and create a password following instructions on the screen. The patient must also select 3 security questions. Click Create to finish creating account. Note: After creation, the portal link will not show as active until office runs the morning index. Continue to print clinical summaries until Portal Link changes to Active.

20 Creating Patient Portal Account Without Instructions from Provider Patient can also Create a portal account prior to receiving instructions from provider. Patient must complete all fields with red asterisks.

21 Navigating The Patient Portal To access clinical summaries or other health documents, patient clicks link for Health Documents. Other options include Manage Account and Manage Providers.

22 Viewing Clinical Summaries on Patient Portal By clicking the Health Documents link and then choosing the clinical summary link, the patient sees the clinical summary as shown above.

23 Viewing Health Record on Patient Portal By clicking the Health Documents link and then choosing the Health Record link, the patient sees the their health record as shown above. This includes the patients active problem list, Medication Allergies, Current Medications and any Diagnostic test results.

24 Managing Providers Shows listing of providers linked to portal account and status (active, disabled). Patient can also click Add My Health Care Provider to link a provider to their portal account. Patient will need to enter authorization code from provider in order to add the new provider. Patient can also Disable a provider from communicating via the portal and Delete providers in this area.

25 Adding Health Care Providers To add a provider, patient must enter authorization code supplied by the doctor s office along with the PIN supplied to office by the patient and then click Add My Provider.

26 Manage Account Patients can update their information from this screen or change their password.

27 Viewing Account Activity on Patient Portal Clicking View Activity menu option displays all activity on account such as modifications, document uploads, removals and linking status for providers.

28 Resetting Password for Patient Portal In patient forgot their password, they can click Reset it.

29 Resetting Password for Patient Portal Patient enters their User Name and an is automatically generated to the address associated with the User Name.

30 Password Reset Form Patient clicks on link and is directed to the password reset form to create a new password for their account.

31 Retrieving Username For Patient Portal Patient must enter address on file and an will be generated with the username for the account.
