3 Liaison Officers Each Day Reporting and Services Center (DRRSC) site has a Liaison Officer (RLO) RLO works with the Detention Liaison Officers, Case Management Probation Officers, primary partners, community and faithbased partners. RLO also complete referrals to DRRSC services, such as Job Readiness Training and other program services. Detention Liaison Officers Probation connects in-custody offenders to pre-release services. Four Detention Liaison Officers (DLOs) have been assigned to San Bernardino County jails. Each DLO is assigned to the jails to coordinate service referrals, contact assigned Probation Officers, and providers located at the DRRSC sites to assist probationers with their plan for their post-release reentry. Any probationer in-custody may request to speak with a DLO or contact our toll free information line at 1 (855) All messages will receive a response by mail for incustody probationers. Homeless Services Officer are available to provide homeless intervention and/or prevention services to probationers experiencing homelessness. Conduct rapid assessment of cases. Identify appropriate emergency, short-term or long-term housing. Coordinate referrals to housing resources. Assist probationers with accessing resources. Services available to in custody and out of custody probationers Department of Behavioral Health (DBH) Each DRRSC site, under the leadership of DBH, has received Medi-Cal certification to provide Alcohol and/or Drug Treatment services, and/or mental health services. On site staff is available to provide one-on-one assessments and a variety of services: Individual Counseling Group Counseling Outpatient Alcohol and Drug services Outpatient Behavioral Health services Crisis Response Outreach and Education services Health and Human Services: Transitional Assistance Department (TAD) The goals of TAD are to meet the basic needs of families and individuals, while working with them to attain selfsufficiency, and to promote work and personal responsibility. On site staff: Administers nutritional and/or medical assistance. Assists with enrolling clients with Medi- Cal or Arrow Care applications Assists with enrolling clients with Cal Fresh applications Also provide referral to complete applications for General Relief or CalWORKS Public Health Department (DPH) Public Health staff work at each DRRSC site to promote the health and well-being of clients: Medical and health education services at the DRRSC include: Health Screening, Assessments & Testing Health Education Medical Referrals Care coordination: Once enrolled into Medi-Cal or Covered California, ensure clients are accessing care Workforce Development Department (WDD) Workforce Development works with DRRSC clients to develop employable skills. WDD provides job readiness training and other assistance: Job Searching and Career Exploration Career assessments and individual employment counseling Staff-assisted placement services, personal coaching/mentoring, job search resources & provide referrals to partner agencies Internet access to apply for job leads at the DRRSC Employment Enhancement Workshops: how to write a resume, interview techniques and dressing for success Desert Industries Community Partnership County of San Bernardino Public Defender Legal Aid Society of San Bernardino Operation New Hope San Bernardino County Schools Thinking for a Change Courage to Change Parent Project Free GED program Tattoo Removal Relational Violence Moving On Gender Specific program Computer Lab access Bus passes and gas vouchers Education scholarships Housing assistance for treatment and non- treatment needs education and resources for community and faithbased organizations 3

4 The Role of Human Services (HS) and the Day Reporting Centers (DRC) TAD is an available resource to fulfill some of these challenges through a number of programs such as: o CalWORKs Financial assistance o CalFresh Food nutrition assistance program o Medi-Cal Medical coverage o ArrowCare Medical coverage for indigent adults o Affordable Care Act Expanded health care coverage effective 01/01/2014 TAD staff is available at the DRC for the AB 109 population as a resource to the various programs. Staff provides same-day assistance with the application process and conducts an eligibility determination. Horizontal integration ensures all available assistance programs are explored for potential eligibility. Benefits and wrap-around services are provided to meet identified needs. Referrals are made to meet service needs that cannot be addressed through the TAD administered programs. PROGRAM NEW AB 109 CASES AVERAGE BENEFIT AMOUNT PER CASE CalFresh 3351 $ 266 CalWORKs 600 $ 430 Medi-Cal 991 N/A ArrowCare 1442 N/A *Summary Report for AB109 Services 09/2013 Vickie Baumbach, Program Manager II Dr. Rebecca Gehlke-Baez, Program Manager I The Department of Behavioral Health (DBH) plays an integral role in providing behavioral health services to the Public Safety Realignment population Designated DBH personnel are co-located at the Day Reporting and Service Center (DRRSC) sites DBH personnel currently provide Mental Health and Alcohol and Drug screenings along with referrals to community resources DBH is conducting mental health assessments and treatment services at all 3 DRRSC sites since receiving Medi-Cal certification. 4

5 Other services offered include a continuum of care for alcohol and other drug treatment services, such as residential and outpatient treatment, community based recovery center services and other specialized services such as Narcotic Treatment Programs (NTP), Drug Court and Perinatal Services Coming to RC-DRRSC-Outpatient AOD Program Services to be provided are as follows: Up to six months of Evidenced-based Cognitive Therapy Trauma Informed, Evidence Based Curriculum Individualized Treatment Planning Individual and Group Counseling Random Urinalysis Case Management Linkage to Vocational and Literacy Training Relapse Prevention Integrated model - Mental Health (MH) and Alcohol and Drug Services (AOD) on site at each of the DRRSCs One SWII and one AOD Counselor co-located at each DRRSC Provide an initial screening for MH/AOD services Utilize a brokerage and linkage process to community resources Partner with Public Health for medical/physical health needs Provide an array of gap services (i.e. Life Skills, AOD education groups) Identify barriers to accessing services (i.e. transportation) Monitor cases (i.e. appointments attended) SWII assigned to EOP cases Mental Health (MH) and Alcohol and Drug (AOD) Treatment Services Outpatient mental health clinic located at each of the 3 DRRSC sites Intensive day treatment mental health program located at the Adult Forensic Services Program in Colton Outpatient AOD program located at the Rancho Cucamonga site Psychiatric services (i.e. evaluation, medication) provided at Colton site The Public Safety Realignment population may also access DBH services through additional points of entry such as outpatient clinics, crisis walk-in centers, the mobile crisis response teams, and inpatient psychiatric hospital settings Conduct and process educational groups, presentations pertaining to facts about alcohol and drugs, rules and regulations of treatment programs and explain treatment process General Services Worker II provides transportation services to and from mental health treatment program sites and other appointments, as needed, that support mental health treatment Identify community social, mental health, and substance abuse service needs Develop, coordinate and evaluate social services through liaison, educational, and consultative activities with other agencies Participate in the Rapid Response Team (RRT) Daniel Perez, MSW Division Chief of Disease Control and Prevention 5

6 Goal: This project will serve 90 high-risk, post-incarcerated individuals placed on California s AB109 Post-Release Community Supervision, targeting those within San Bernardino City s boundaries. Population: Using the Correctional Offender Management Profiling Alternative Sanctions (COMPAS) Risk Score, 72 male/18 female, high-risk offenders, will be selected for services. CSUSB SBCRC Partners/Stakeholders PRE-RELEASE ACTIVITIES BRIDGES will Provide Intensive Case Management Services provided by Peer Advocates (PRAs). The PRAs will: Contact participants at least 3 months prior to release to build trust and to begin assessing post-release needs, Coordinate with Social Workers, Educators, and Release Planning Specialists to ensure that BRIDGES services enhance and fill gaps, not duplicate County Jail services, Identify post-release resources with assistance from the BRIDGES Housing Specialist, and Pre-enroll participant in services pre-release; and develop the reentry plan in conjunction with County Jail staff and reflecting data from COMPAS to ensure services in the plan are tailored to the participant. POST-RELEASE ACTIVITIES Upon release, participants will receive services at SB County Probation Day Reporting Centers (DRC) BRIDGES post-release services will include: Immediate and ongoing contact with the participant, Ensure that release planning is implemented and participants are linked to services, with a special emphasis on housing, and Coordinate with DRC Probation Officers to ensure participants are linked and remain in services particularly in those services criminogenic risks. Collecting/Reporting Key Data Elements: BRIDGES will engage in a robust project evaluation by having California State University San Bernardino (CSUSB) conduct a process/outcomes-based evaluation. COMPAS will capture information pre-release to identify the needs/risks of each client and the dosage/intensity of services needed. CSUSB will develop/conduct the Process/Outcome evaluation to measure program impact. BRIDGES will collect/analyze/report data using an electronic Client Contact/Information Case Management Database system. The County will be the keeper of all data and will be able to report on outcomes for the required five year period and beyond.
