2 Our personalised care and support services create the foundations for a brighter future Who we help We offer a full range of personalised services designed to create stable, supporting environments enabling our clients and their families to build a bright and fulfilling future. For those affected by illness, injury or disability, we specialise in providing person-centred complex care at home. We enhance our clients quality of life by enabling them to remain living in their own homes. We aim to build effective positive relationships with our clients and their families, facilitate their involvement in their own care and give them the opportunity to shape our service. We are registered with the Care Inspectorate as a Care at Home provider and a Nursing Agency, allowing us to directly staff complex care packages for our clients across Scotland. We specialise in enabling rehabilitation as well as maximising independence and well-being for children and adults with complex needs, including (but not limited to): Head injuries Spinal injuries Home ventilation and respiratory support Multiple sclerosis Cerebral palsy Huntington s disease Korsakoff s disease Stroke Parkinson s disease 2 3

3 Services we provide We involve every client in the development of their personalised care plan, and in the recruitment, training and development of their team of carers. We also empower and assist each client and their family to organise or coordinate aspects of their life. This can include: leisure and recreation providing assessments and training purchasing of specialist equipment Nursing and care at home Our nursing and care-at-home services are provided by professional staff trained to work with, and meet the specific needs of, each client. We can provide a full nursing team to cover a 24 hour package as required. We support clients with specific nursing needs such as PEG feeding tubes, tracheostomies, supra pubic catheters or assisted ventilation. Personalised rehabilitation and support We create person-centred rehabilitation plans to ensure the right balance of support, development of independence and, particularly, of choice. Our staff work in partnership with other health professionals and providers to ensure a continually evolving and adaptive service. occupation/ education/ training liaising with legal personnel Occupational therapy services Our fully qualified occupational therapists can provide assessment, advice and treatment to promote daily living skills and enhance independence. This includes advice on equipment and property adaptations to assist those affected by illness or disability. As part of CareTech Community Services, we benefit from belonging to a network of services which support children, young people and adults across the UK. 4 5

4 Quality assured A high standard of direct care to service users with complex medical needs Care Inspectorate report We aim to provide our clients with a comprehensive service of the highest quality. We strive to offer a flexible, efficient and professional service which is tailored to meet each person s individual needs. We respect the right of each client to lead as independent and fulfilling a life as possible. We have set out a Client s Charter of Rights which we believe should be the minimum entitlement for each client. All staff follow this charter. We integrate quality in our way of life and in everything we do. For each client, we aim to provide specialist care to suit their needs and quickly act on their feedback if adjustments are required. For complete transparency, we fully document every aspect of running and managing our business according to a comprehensive set of policy documents. These policies are assurance that we run our care-athome and nursing services to the highest standards, in compliance with national guidelines, and cover all aspects of staffing, managing, caring for our clients, and the preservation of health and safety standards. We audit all of our policies and work practices at least annually to ensure that we maintain the standards we have set ourselves. In addition to our own continual self-assessments, we also receive regular inspections from the Care Inspectorate. We follow stringent recruitment procedures to ensure we only employ staff of the highest calibre who are compliant with the Care Inspectorate Standards of Care. Such care support is essential to our quality of life as a family Phoenix client 6 7

5 Tony s story Tony was 21 when he was involved in an accident at his work on a building site. As a result of this he sustained a complete C6-7 fracture of his spinal cord. Tony spent a year in the Spinal Injuries Unit in Glasgow. During this time he underwent intensive rehabilitation with the goal being to return to as independent a life as possible. A comprehensive support plan was developed to be used as a guide in the journey towards Tony s increased independence. Phoenix then put into place a care package to support Tony within his home. He was actively involved in the recruitment of staff and the care they would provide. Staff were trained by our Nurse Consultant to provide the complex daily care Tony required. This plan is adapted frequently to meet Tony s changing needs. Six years on, Tony continues to receive support from Phoenix. He learned to drive following the accident and can now transfer independently into his car. Areas of Tony s care will always remain complex. However, he now has less day-to-day support from staff. Tony enjoys playing wheelchair rugby and travels throughout the UK to attend tournaments. He also supports others who have newly acquired spinal injuries, through Spinal Injuries Scotland, as they begin the journey he undertook many years before. 8 9

6 Working together Our clients are at the centre of everything we do at Phoenix Therapy and Care. This is how we ensure that every client s individual needs and requirements are looked after: Initial meeting Getting to know a client, their family and the professionals around them helps us build a detailed, individualised care plan to include the client s care requirements, preferences and ambitions. Recruitment The client personally selects the people in their care team to ensure that they get the right level of support to develop their independence. Training A bespoke training programme is developed, incorporating the client s input. The Care Manager identifies any specialist training requirements relating to clinical procedures identified in the client s care plan. Care begins The care team gradually begin working to allow a smooth transition, with the client having time to build a relationship with each team member. Care plan An holistic assessment is completed to identify training, clinical needs and social needs, which are documented in the care plan. The client meets their dedicated Care Manager regularly. Ongoing reviews The Care Manager provides ongoing supervision, training and support to the staff, gathering and providing feedback. Care plans are regularly evaluated and goals maintained through continual reinforcement. Funding Our care and support packages can be funded by a variety of means including selfdirected support, personal injury litigation or compensation payments, insurance and private healthcare funds, and via public funding under contract from local authorities and primary healthcare trusts
