Any patient 40 years and above can be admitted for medical care and rehabilitation. 2. Any race and religion 3. Medically stable for physical rehabilitation or continuing care 4. Suffering from stroke, hip fracture, amputations and general de-conditioning post surgical / medical conditions Patients unsuitable for admission include those who: 1. Are medically unstable e.g. febrile patients, or those who require urgent investigations and treatment for unresolved problems like endoscopy, surgery, blood transfusion; 2. Have unresolved social problems / discharge plan; 3. Have drains, traction, or require heavy spinal nursing care; 4. Require assisted ventilation; or 5. Require peritoneal dialysis. 2 Inpatient Services Our multi-disciplinary team is staffed by experienced registered medical practitioners, specialising in geriatrics and rehabilitation. In addition, there are visiting geriatric, neurology, orthopaedic, respiratory, rehabilitative medicine and psychiatry consultants from acute hospitals, who monitor patients progress, and lead clinical reviews and multi-disciplinary meetings. Patients with complex wound or stoma problems are referred to us for specialist inpatient or outpatient care in wound management and stoma care. More information at Outpatient Services St Luke s Hospital s (SLH) Outpatient Services includes outpatient clinical care, and day rehabilitation, home care services. Outpatient Clinic CHAS and Medisave schemes for chronic disease management may apply. SLH provides outpatient follow-up services for the following: 1. Chronic medical conditions - Continuing care and management of chronic diseases 2. Geriatric syndromes 3. Wound and leg ulcers - Management of advanced wounds and leg ulcers 4. Mood disorders and dementia-related problems 5. Musculoskeletal Physiotherapy - Acute pain management and manual therapy 6. Speech Therapy - Swallowing, speech and language therapy By appointment only, please call Day Rehabilitation Centre (DRC) Day Rehabilitation Care is a customised therapy programme, with physiotherapy, occupational and speech therapy, group activities and caregiver training. The programme aims to help patients achieve functional recovery. They may be re-integrated back into the community after learning to cope with physical impairments or functional deficits resulting from medical conditions such as stroke or orthopaedic conditions. 3 Supporting Services (Inpatient / Outpatient) These include: Visiting geriatricians, for clinical support in sub-acute care Visiting psycho-geriatrician (psychiatrist), for consultation on difficult behavioural problems or severe depression Visiting orthopaedic surgeon, for review of post-fracture patients to help reduce the need to return to acute hospital outpatient orthopaedic clinic for review Visiting neurologist and rehabilitation physician for challenging neurological and rehabilitation cases Visiting respiratory physician, for cardiopulmonary rehabilitation of chronic heart and lung conditions Dietary counselling, for nutrition counselling to inpatients, outpatients and their caregivers Podiatry services, for consultation on in-grown nails, corns, calluses and other peripheral vascular diseases Community eye services; in collaboration with NUH, visiting ophthalmologist provides consultation for simple eye conditions such as conjunctivitis, macula edema and diabetic retinopathy. Dental services including basic polishing, scaling, extraction and denture services specially for wheelchair or bed-bound clients. X-Ray and laboratory services Chaplaincy service, for counselling and comfort to patients and their relatives. The actual charges will depend on your care needs and the status of your application for Government subsidy. 3. The estimated average monthly bill size is based on a 30-day stay at the hospital. Charges include ward charges, treatment fees, therapy treatments and ancillary charges such as laboratory, specialized investigations, non-standard medicine and diapers but do not include charges for specialized treatments such as cell mattress, wound care, oxygen therapy and expenses that cannot be claimed from Medisave. 4. The amount indicated above is an estimate based on past data. The final bill amount would depend on your condition and care needs and may differ significantly from the above estimates. 5. Medisave for community hospital charges is capped at $5,000 per calendar year or up to $250 a day. 6. The above rates are in Singapore Dollars and do not include GST. Please note that GST at the prevailing rate will be charged on all bills.
