Providers and staff embrace HealthSource s mission to provide access to quality, comprehensive primary care regardless of our patients income level or insurance status. Simply put, we put our patients first and it shows. In a 2014 survey, 98.6% of patients said they would recommend HealthSource to their family and friends. In a time of unprecedented innovation and reform in health care, patients have more choices than ever. We know we must offer excellent customer service and deliver quality health outcomes to be our patients provider of choice. That is why we have instituted a comprehensive customer service program and achieved Level 3 Patient Centered Medical Home accredited by NCQA. HealthSource is truly committed to improving the health of our community through comprehensive care delivered with compassion. We strive to be the model of excellent health care. 4 2014 HIGHLIGHTS HealthSource launched the NextGen Patient Portal. Patients can access their medical records, test results, request appointments, and ask their provider questions - 24 hours a day, 7 days a week from the privacy of their own home on any computer or mobile device. HealthSource launched our re-designed website, providing users with easy access to the information they need. Outreach and Enrollment HealthSource has Outreach and Enrollment staff dedicated to helping individuals and families throughout the service area evaluate and apply for health care coverage under the Health Care Marketplace and The Washington Court House, Wilmington, and Medicaid. In 2014, we assisted over 10,000 community Loveland centers all moved into their new locations. members. Our Outreach and Enrollment staff are able to make a direct impact in the communities we serve. Being part of the outreach and enrollment staff at HealthSource has been a rewarding experience. One of my favorite stories involves a mom who scheduled an appointment to complete an application for Medicaid. While completing the application she indicated that she hoped her son would be covered too. He s 12 and she said she couldn t remember the last time he was seen by a doctor. When I asked if he had a family physician she said no. Her application was completed and an application for Presumptive Eligibility for her son was submitted. When she found he was approved she cried. She said that other kids at his school had been sick that year and he kept telling her I won t get sick Mom, I ll be okay. She was thankful and relieved that her son would be receiving the health care he needs. - Beverly Gifford, Director of Outreach and Enrollment Centerprise is a wholly owned, for-profit subsidiary of HealthSource that was founded to provide comprehensive revenue cycle management services to health centers and other health care providers. In 2014, Centerprise established executive level professional and consulting services for clients nationwide. It has quickly become an industry leader in billing, quality improvement and population health management.

5 Foundation In its second year of operation, the HealthSource Foundation received $80,866 in contributions from foundations, corporate partners, and individual donors. Gifts are used for Capital Growth and Enhancement, Patient Assistance through the employee-sponsored Health and Hope Program, Endowment Funds and the Reach Out and Read program. Specifically, the Foundation provided funds to purchase new waiting room furniture for its Washington Court House center, early childhood development books to parents and children in its pediatric centers and numerous patient assistance initiatives in each HealthSource center. The Foundation s signature fundraising event is its annual Race a Chair for Health Care held in the Fall.
