1 A NN UAL REPORT MESSAGE FROM The CEO Each year presents us with new challenges and opportunities was no exception! The incredibly talented and devoted team I have the privilege to lead finalized years of work with three large-scale programmatic accomplishments. In January of 2016 we began our participation as one of only 70 sites nationwide to participate in a Medicare pilot project focused on caring for those patients who are seriously ill but not yet ready to elect hospice care. Data gathered from the Medicare Care Choices Model (MCCM) will inform Medicare about the benefit pre-hospice services may provide. July of 2016 saw the start of a Fellowship training program in cooperation with the Penn State Milton S. Hershey Medical Center and the Lebanon VA Medical Center. HCP provides fellows with training in home-based hospice care and the interdisciplinary team function. In December, capping years of preparation, we were awarded a full three-year accreditation and The Joint Commission s Gold Seal of Approval. Throughout this report you ll see some of the numbers associated with delivering care to over 1,700 patients and families in I want to thank our team of caregivers, support staff, volunteers, board of directors and donors for helping us to provide much needed quality end-of-life services to our community. It is my privilege to serve with all members of the HCP team to provide the highest quality of emotional, physical and spiritual support with compassion and respect. One of the most impressive aspects of this organization is that it is always changing and improving. During my years of service, I have been impressed with our ability to respond to the shifting climate in healthcare and our region. It is critical for hospice providers to demonstrate the quality of their care and their efforts to identify and improve when needed. As a result, I couldn t be more proud to direct your attention to our quality story on page three of this publication. 3 Our Quality Story Our mission is clear; to provide the highest quality of emotional, physical and spiritual support with compassion and respect. Quality is always on our minds. In keeping with regulations and best practices, HCP measures the quality of our programs through third-party administered caregiver surveys. The agency we have engaged for this purpose is called Deyta Analytics. Two symptoms, pain and dyspnea (shortness of breath) are the most common for patients at the end of life. Given that, HCP closely monitors our effectiveness at screening and treating these symptoms in our patient population. Here are our measures for 2017: Deyta National Average HCP Pain Screening on Initial Assessment 95.0% 97.2% Dyspnea Screening on Initial Assessment 98.1% 100% Pain Assessment within 1 Day of Screening Positive for Pain 82.6% 97.0% Dyspnea Treatment Received (or Declined) Within 1 Day of Screening Positive for Dyspnea 82.6% 97.0% Deyta Analytics also administers the Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey which asks family members to report on and evaluate their experiences with HCP. 4 Ways to Give Memorial and Tribute Gifts Special Events A fun way to get more involved with HCP and fundraising is through special events. Every year, HCP offers many opportunities to support the highest quality of end-of-life care and bereavement care. Please contact Tiffany DePaoli in the development department to help with any of the following fundraisers: Men s Golf & Tennis Classic Butterfly Release Ladies Tennis Classic Highmark Walk for a Healthy Community Ladies Golf Classic Tree of Light Memorial and tribute gifts offer a way to remember or honor a loved one, while also supporting Hospice of Central PA. Families are notified that you have made a gift (not the gift amount) when you provide a name and address for this purpose. Restricted Gifts HCP is able to honor donor-restricted gifts for the following programs: Carolyn s House Third Party Events A 3rd party event is when a business, organization or family plans a special event to benefit HCP. Hospice of Central PA is the grateful recipient of the net proceeds of many 3rd party events throughout the year including the Bob Baish Memorial HCP welcomes gifts restricted to Carolyn s House which provides 24-hour hospice care in a comfortable home-like setting in Harrisburg. HCP offers this residential hospice service when personal choices or medical needs make other options difficult. The approach to care at Carolyn s House includes an interdisciplinary team of trained professionals who work together with the patient and family to develop a plan of care. Carolyn s House is also offered as a solution for short-term respite care to provide relief for caregivers. Golf Tournament, Aroostine s Lemonade Stand and the Shellsville Horseshoe Tournament in memory of Casey Koons. These events happen independently of HCP. It provides a safe and fun environment in the great outdoors to work on coping skills and healing activities with peers.

5 Thanks to the deliberate acts of generosity from you and others, HCP patients and families receive the highest quality of care. Your gifts make this possible and every donation regardless of size, brings comfort. Volunteers HCP volunteers started the organization in 1979 and continues to be a powerful way to give. In 2016, HCP recorded 189 active volunteers who donated 12,137 hours to our mission. Their time and talents resulted in $192,501 in cost savings to the organization. The Joint Commission is a nationally recognized accreditation program that helps organizations deliver safe, high quality care and services. HOSPICE HONORS RECIPIENT In June, HCP was named a 2017 Hospice Honors recipient by Deyta Analytics, an industry leader in data-driven management. The Hospice Honors program recognizes hospices that provide the highest level of quality from the caregiver s perspective. WE HONOR VETERANS PARTICIPANT HCP has achieved the highest level in this national partnership program Level Four Participant. HCP honors veterans through respectful inquiry, compassionate listening and grateful acknowledgment. MCCM PILOT PROGRAM PARTICIPANT Selected by the Centers for Medicare & Medicaid Services (CMS) to pilot a pre-hospice care transition program (Medicare Care Choices Model). MCCM provides a new option for Medicare beneficiaries to receive hospice-like support services while receiving services provided by their curative care providers. Through the efforts of hospices like HCP, CMS is evaluating whether providing these supportive services can improve the quality of life and care received by Medicare beneficiaries, increase patient satisfaction, and reduce Medicare expenditures ANNUAL REPORT HOSPICE OF CENTRAL PA 7
