1 Job Description Job title: Deputy Team Leader Community Nursing Band: Band 6 Responsible to: Accountable to: Responsible for: District Nurse Team Leader Service Manager n/a JOB PURPOSE To provide skilled nursing care working as part of the Primary Health Care Team to patients in a variety of community settings between the hours of 08:00 and 21:00, including weekends. To actively monitor, maintain and develop the service provided. To take responsibility for managing the caseload and the team, with the support of the Team Leader. To deliver a proactive case managed approach for patients with long term care needs. To actively participate in service development activities. MAIN DUTIES AND RESPONSIBILITIES 1.0 Clinical 1.1 To provide skilled, effective and evidence based nursing care to patients in a variety of community settings. 1.2 To act as an autonomous Named Nurse taking continued responsibility for first assessments and on-going assessment, planning, implementing and evaluation of care given to individuals, groups of patients and their families. 1.3 To undertake comprehensive assessment of the physical, psychological and social Care needs of patients using the Single Assessment Process and to include the needs of carers. 1.4 To utilise a case managed approach in the co-ordination of care that aims to prevent And reduce hospital admissions. 1.5 Actively work with the Community Matrons, team leads, GPs and others to identify patients with Long Term Conditions requiring proactive case management.

2 1.6 To act as the patient s advocate and to facilitate the patient s own choices with regard to nursing care, promoting independence and self-care, as appropriate. 1.7 To identify and record new problems and other relevant information reporting back to the District Nurse Team Lead as appropriate. 1.8 To liaise with, and where appropriate initiate referrals, to ensure adequate support to patients and carers. 1.9 To provide information and support to patients, relatives and other carers that promotes and optimises positive health To undertake health promotion and disease prevention activities such as flu immunisation,advice on stopping smoking, dietary advice and foot health To have knowledge of, and be able to effectively use local services and resources To promote patient care To ensure the safe handling of body fluids and contaminated sharps To be alert to the needs of vulnerable adults including routinely undertaking pressure ulcer and falls risk assessments, considering safeguarding and mental capacity and taking appropriate action when required To provide specialist nursing skills such as leg ulcer assessment and management, syringe driver set up, intravenous drug administration, continence assessment and palliative care To support patients and their carers with managing their medicines and undertaking medicines management reviews as directed To provide care to people with long term conditions undertaking reviews in accordance with the individual patient care plan and with the support of more specialist colleagues as required To undertake nutritional assessments and advice and support patients with their nutritional needs. For example PEG feeds To have an innovative approach to practice in response to changing service needs and priorities To manage the caseload with the support of the District Nurse Team Lead. 2.0 Managerial 2.1 To establish and maintain a good working relationship with colleagues, the primary health care team and other health and social care service providers, so as to provide a comprehensive service to patients and carers. 2.2 To be the named link nurse for designated GP practice(s) 2.3 To supervise and co-ordinate the team on a day to day basis and to manage the team, with the support of the District Nurse Team Leader. On-going management responsibilities include the appraisal of staff, reviewing and supporting junior staff with their personal development plan and assist in the recruitment and selection of staff and assisting in the performance management of junior staff. 2.4 To contribute to the implementation, monitoring and reporting of performance

3 outcome measures, alerting team leads and managers of any shortfalls. 2.5 To complete statistical returns as required by the Trust including entering data onto the computerised system within the required time frames. 2.6 Ensure the timely and correct entry of clinical and statistical data onto the RIO system of own data and that of other team members. 2.7 To be responsible for the care and safe keeping of equipment issued for personal and team use and to report any defect or loss. 2.8 To attend and participate in Team, Locality and other relevant meetings. 2.9 To be a member of working parties and groups considering aspects of the service and professional practice as required To take an active role in service development activities, identifying areas for improvement and options for change Assist in the implementation and monitoring of clinical and service standards Apply equal opportunities in practice taking into account own behaviour and the needs of patients, carers and colleagues Respond to complaints appropriately and in accordance with Trust policy seeking Guidance as required. 3.0 Educational 3.1 To actively participate in the teaching and support of junior staff and act as a preceptor for new staff in the locality. 3.2 To act as a mentor to student nurses and support staff, for example health care assistants and assistant practitioners. 3.3 To provide training and support to nursing colleagues and others, for example allied health professionals. 3.4 To actively participate in clinical supervision on a regular basis in line with Trust guidelines. 3.5 To participate in annual appraisal and maintain a personal development plan. 3.6 To participate in research and audit to ensure the development of effective and innovative practice and maintenance of standards. 3.7 To develop and ensure an active learning environment for all staff grades. 3.8 To provide clinical supervision to others in line with Trust policy. 4.0 Personal and Professional Responsibilities 4.1 To support the development and implementation of the Trust s clinical and Operational guidelines and policies. 4.2 To be aware of, and act in accordance with, the Trust s clinical and organisational policies and guidelines.

Staff should be aware that patients records throughout CLCH NHS Trust will be the subject of regular audit. In addition, all health professionals are advised to compile records on the assumption that they are accessible to patients in line with the Data Protection Act All staff that have access to patients records have a responsibility to ensure that these are maintained and that confidentiality is protected in line with CLCH NHS Trust Policy. Health and Safety All staff are required to comply with the requirements of the Health and Safety at Work Act and other relevant health and safety legislation and CLCH NHS Trust Policies and Procedures. All staff are required to make positive efforts to promote their own personal safety and that of others by taking reasonable care at work, by carrying out requirements of the law or following recognised codes of practice and co-operating with safety measures provided or advised by CLCH NHS Trust to ensure safe working. Managers are responsible for implementing and monitoring any identified risk management control measures within their designated area/s and scope of responsibility. In situations where significant risks have been identified and where local control measures

Fire Safety Fire Safety in all the premises from which we operate is the concern of all those who work within these premises. It is of the utmost importance that all members of Staff whether part-time, temporary, bank or permanent are fully aware of and familiar with the Fire Safety Policy and Procedures in the department they are working in. All employees will receive Fire Safety training as part of the Trust s induction and mandatory refresher training programmes. In addition, all non-clinical employees will receive Fire Warden training as part of the Trusts induction and mandatory refresher training programmes. Infection Control All staff are required to follow CLCH NHS Trust infection control policies and comply with all measures known to be effective in reducing infection risk. All staff must complete infection control training at induction, and all staff whose duties involve patient contact must also complete infection control refresher training annually. No Smoking Policy There is a no smoking policy in operation in CLCH NHS Trust. In accordance with this policy smoking is positively discouraged and is not permitted in any areas. Hours: [37.5] Salary: Pensions: Annual Leave: 30,940-40,809 pro rata per annum, inclusive of higher cost area supplement.
