2 GENERAL INFORMATION Vision By sharing collective experience, the Primary Care Network (PCN) Strategic Leadership Forum is equipping primary care teams for continued development of the Patient s Medical Home for Albertans. Mission To be the preeminent setting for PCN leaders to converge, share knowledge and discuss emerging issues concerning PCNs, their partners, PCN physicians and allied health professionals. The learning and tools gained empowers participants to implement and manage changes at the provincial, zonal and local levels. Continuing Medical Education Credits This Group Learning program has been certified by the College of Family Physicians of Canada and the Alberta Chapter for up to 8.5 Mainpro+ credits. Certificates of attendance will be ed to attendees on Tuesday, September 12. If you wish to earn more credits, you may complete a Linking Learning to Practice Activity. Visit exercises/ for more details. WiFi You can connect to the wireless network by joining the PCN Forum network and using the password integration. Evaluation In-lieu of paper evaluations, an online evaluation will be sent to your upon completion of the event. Use the note pages at the back of the agenda package to note your feedback! Presentations Presentations will be posted on the pcnpmo.ca website next week. An will be sent to all attendees once they are available for viewing.

With each encounter, Dave brings the public voice as he works with health care planners to redesign services. 4 10:15 11:00 a.m. Office of the Auditor General Merwan Saher, Auditor General of Alberta In May 2017 a report by the Office of the Auditor General was released that highlighted areas for improvement and the barriers to creating an integrated health system; a system that is built around the patient, not administration needs. During this session, the Auditor General will offer insights on what is required to advance primary care as the key building block to a successfully integrated system in Alberta. 11:00 a.m. 11:45 a.m. Primary Health Care Integration Network (PHCIN) Dr. Brad Bahler, Medical Director, PCN Evolution; Senior Medical Lead, PHCIN Rob Skrypnek, Senior Program Officer, Primary Health Care; Senior Provincial Director, PHCIN The Primary Health Care Integration Network (PHCIN) is a newly formed Strategic Clinical Network that will be leading integration efforts in the province. Sessions will be repeated at 1:45 p.m. Imperial 5, 7 Hospital to Home Discharges Sheri Fielding, Clinical Director, Edmonton Southside PCN Jessica Schaub, Evaluation Manager, Edmonton Southside PCN Members of the Edmonton Southside PCN review Netcare for hospital discharges and use the LACE tool to identify patients at risk for readmission. During this session, attendees will learn about the process used at Edmonton Southside PCN, lessons learned, barriers they have overcome and the measures collected to demonstrate impact. Imperial 9 Dementia Care and Community Geriatric Service Planning Sharon Hamlin, Senior Planner, Provincial Primary Health Care, AHS Charlene Knudsen, Practice Lead, Dementia Strategy, AHS The Primary Health Care Integrated Geriatric Services Initiative, supported by the Senior s SCN and Primary Health Care Integration Network, has partnered with five PCNs, and the Alzheimer s Society of Alberta and Northwest Territories in the Central Zone to enhance capacity of pimary care clinicians and community agencies to provide care and support for those living in the community with dementia. During this session attendees will learn about this early adopter initiative, and express interest in scale and spread. Friday, September 8, 2017

Empowering the Patient s Medical Home to provide care for patients through co-created pathways, enhanced specialist support through a dedicated telephone service (Specialist LINK) reducing waitlists and improving patient access - all are intended consequences and outcomes as result of this collaboration. Alberta has taken some steps toward improving continuity. 9 9:00 9:30 a.m. CPAR: Cornerstone for Continuity Dr. Tobias Gelber, Associate Clinic, Pincher Creek; President, Section of Rural Medicine, Chinook PCN Barbra McCaffrey, TOP EMR Lead, Alberta Medical Association Coming in 2018, the Central Patient Attachment Registry (CPAR) will be a cornerstone in the campaign towards improved continuity of care in Alberta. For the first time primary care providers will receive timely reports on patients in their panel with attachments to other primary care providers in the province. Providers and team will have the opportunity to discuss with patients the importance of seeing a single provider for longitudinal care, will have a better understanding of the real make up of their panels and will be able to focus chronic disease management where it will be most effective. As a province wide database, CPAR will, once a patient s provider is validated, identify the patient s primary provider in Netcare enabling easier access to information for non-primary providers and better transitions of care between the community and the Medical Home. Reports will be developed to support PCNs with business intelligence for service planning. As a result of participating in this session participants will be able to recognize how a new technical solution will be an enabler for improved continuity of care in Alberta and evaluate the benefits of CPAR participation to patients, providers, practices and PCNs. 9:30 9:35 a.m. PCN Governance: Board Competency Training Dr. Scott Beach, Physician Member, PCN Board Training Steering Committee and Board Member, Calgary West Central PCN Bill Kreutzweiser, Consultant, Practice Management Program 9:35 10:00 a.m. PCN Governance Framework Update Dr. Jeffery Bratvold, PCN Physician Lead Executive, South Zone Shannon Berg, Executive Director, Primary Health Care, Alberta Health Rob Skrypnek, Senior Program Officer, Primary Health Care; Senior Provincial Director, PHCIN Attendees will learn more about the recent progress in implementing the PCN Governance Framework including the membership of the provincial committee, role of the zone committees, how information will be shared and how these changes will improve primary health care. 11 Imperial 9 QI Member Support Planning Workshop Mark Watt, Program Development Lead, Toward Optimized Practice In this session we will continue the discussion about linking primary care objectives and measures to facilitate planning for Patient s Medical Home (PMH) implementation. PCNs will be encouraged to share their experiences, tools and resources that have been helpful in planning and supporting practices with PMH through a guided discussion. With foresight in focus, attendees will be exposed to tools to support planning and documenting their approach to supporting PMH.
