2 241/23/09 Minutes/Miontuairiscí The minutes of the previous meeting held on the 14 th July were proposed by Councillor Austin Francis O Malley, seconded by Councillor Tony Ward and adopted. 242/23/09 Chairman s Business/Gnothai an Chathaoirligh The Chairman stated that discussions have taken place at both Committee Meetings regarding the reconfiguration of such meetings. It was agreed that he will arrange to meet with the Chairs of the and PCCC Committees with a view to discussing the matter further. He will also discuss the matter with Dr. Conroy, Director, so as to establish the regulations, etc., in relation to such meetings and report back to the next Regional Forum Meeting. These discussions were in the context of members feelings that Committee Meetings could be more focused in relation to local services. 243/23/09 Presentation on the Pandemic H1N Swine Flu Dr Diarmuid O Donovan, Director of Public, Mr Alfie Bradley, A/General Manager, Emergency Management, HSE West and Mr. Tony Canavan, Manager, Mayo General made a comprehensive Presentation on the Pandemic H1N Swine Flu. The Presentation covered the following: Description of the flu Ongoing upgrading and more scientific knowledge becoming available Strategies for containment, mitigation Situation worldwide, Europe and in Ireland in terms of the number of cases, number deaths etc Who is affected? Preventive measures for members of the public Measures to take if you get the flu Seasonal flu Historical details regarding previous pandemics service issues Defined risk groups Anti-viral vaccinations Occupational for health care workers Pandemic vaccinations Goals in the pandemic vaccinations Why immunise? 3 The Chairman and members of the Forum warmly welcomed the comprehensive and clear Presentation on the influenza pandemic. Members raised the following issues: Period during which person is infectious Licensing of vaccination Side effects of vaccination Measures to be taken for vaccination of large institutions such as universities and schools Vaccination of health staff Uptake of vaccination by health staff Why vaccination was not administered during school holidays Measures for involvement of GPs in the vaccination programme Time frames for mass vaccination in each county The normal flu vaccine and the swine flu vaccination Procedures and information on public vaccination Risk Groups Vaccination of residents in private nursing homes Procedures to be taken to protect individuals from flu Procedures to be taken if one has the flu Criteria for closure of schools and colleges in the event of a swine flu outbreak Dr O Donovan, Mr Bradley and Mr Canavan dealt with all issues raised. It was agreed that a copy of the presentation would be circulated to all members. It was also agreed that final details of locations, times, etc., for Public Vaccinations will be communicated to all members of the Forum when they are agreed. 244/23/09 Committees/ Councillor Tom McNamara reported on the PCCC Committee Meeting as follows: The first meeting of the new Primary, Community & Continuing Committee took place on Tuesday 1 st September, 2009 in the HSE Offices, Merlin Park,. A total of thirteen members attended. Officers Elected: The following Officers were elected: Chairperson: Councillor Tom McNamara representing Clare County Council Vice-Chairperson: Councillor Michael Hourigan representing Limerick City Council Presentation on Services provided by the Primary Community and Continuing (PCCC) Services Mr Bernard Gloster, Local Manager, North Tipperary/ Limerick PCCC Services gave a presentation, which gave a broad over view of health and personal services provided by the PCCC Services and also an update on the budgetary and other challenges facing the services. Presentation on Services provided in the Population Directorate Dr Jacky Jones, Functional Manager Promotion, HSE West made a Presentation on the services provided in the Population Directorate including details of health promotion initiatives in progress within the HSE West and an update on the preparations, plans, etc., by the HSE in dealing with the Swine Flu. 4 Presentation on Diabetic Retinopathy Members recommended that a presentation made by Dr Ann Shannon, Public Specialist, Department of Public, HSE West on a Diabetic Retinopathy Project, to previous meeting of the PCCC Committee project should be made to a future meeting of the Regional Forum. Local Meetings: Members recommended that at least two local meetings should take place annually between members of the Regional Forum and Local, and Ambulance managers. Such meetings to be scheduled and Forum members informed of dates locations, etc. Next Meeting: The next meeting of the Committee will take place on Tuesday 1 st December 2009 in the HSE Offices, Catherine Street, Limerick. Councillor Austin Francis O Malley reported on the Committee Meeting as follows: The Committee Meeting was held on Tuesday 8 th September, 2009 in the Education, Merlin Park, HSE West,. 19 members attended the Committee, including new and returning members. Election of Chair and Vice Chairperson Councillor Austin Francis O Malley was nominated for the post of Chairperson and as there were no further nominations was deemed elected. Councillor Rose Brennan was nominated for the post of Vice-Chairperson and as there were no further nominations was also deemed elected. Presentation on the Regional Forum West The Regional Office Director, Dr Seán Conroy, gave a brief presentation on the history of the Forums to date, the legislative background, the structure of the Forums and the HSE West Region, and the workings of the current Committees. Presentations on the HSE Budget/Service Plans 2009 for Services in the West Mr Alan Moran, Network Manager, HSE West and Mr John Doyle, Local Manager, Clare, who attended on behalf of Mr. John Hennessy, Network Manager, Mid-Western s, each gave a comprehensive presentation and update on the current budgetary situation in their respective service areas. Many issues were raised and discussed including: current overspends and associated risks to services; the transfer of cancer services to ; stat results, the Swine Flu Pandemic; the transfer of hospital services to Limerick; bed closures in limerick hospital; staff embargo implications; low morale; derelict accommodation in Limerick and Young Adult Cystic Fibrosis Services in the Mid West. Ambulance Service The Ambulance Service provided documentation setting out each area s current budgetary situation and activity. Population Dr Jacky Jones, Functional Manager Promotion, HSE West, gave a presentation on Population Services. Issues discussed included: Cancer Deaths in the West in 2008 and the Prevention of Suicide. Dr Jones also highlighted and circulated the Standard Drink in Ireland What Strength report to members. 5 Matters raised by members, to be further discussed and considered by the Forum Chair, Committee Chairs and the Director included the possible reconfiguration of the Forum Committees and an Update on Cancer Services in the West. The next meeting of the Regional Forum Committee will take place on Tuesday 8 th December, 2009 in Limerick. The approved Minutes of the PCCC Committee Meeting held on the 7 th April, 2009 and the Committee Meeting held on the 28 th April, 2009 were circulated with the Minutes of the meeting. 245/23/09 Questions/Ceisteanna W23Q457 Councillor Gordon Hughes To ask the Executive of the HSE North and West, what is the up to date position regarding the provision of a new community hospital/day care centre in Ballinamore. Stage 3 of the process was due to be completed by the end of June 2009 and was due to proceed to the planning stage thereafter. Consultation was also due to take place with local residents regarding the design and layout, when it is proposed that this will happen? Can I also ask has it been approved in the 2010 National Development Plan and if so, what is the envisaged budget and timeframe for the completion of this project? The design process has now been completed in respect of the new Community Nursing Unit/Day and Primary in Ballinamore. It had been anticipated that a planning application would be completed by the end of June 2009, however, further consultation was required with Leitrim County Council Planning Department in respect of minor layout issues. These matters have been agreed and the planning application will be submitted before the end of September As you are aware, the planning process allows for the public to view the plans, however the HSE is more than amenable to having a public information evening, if so required. In 2009, the HSE has approved funding to complete design work and planning application. Tendering and award of a construction contract is subject to approval at national level in quarter It is anticipated that a 12 month construction period would deliver the project, if approved to proceed. W23Q458 Councillor Gordon Hughes To ask the Executive of the HSE North and West what is the up to date position regarding the provision of a new 60 bed community nursing unit in Carrick-on-Shannon? Can I also ask has it been approved in the 2010 National Development Plan and if so, what is the envisaged timeframe for the completion of this project? There has been no change in the position in respect of the provision of a new 60 bed Community Nursing Unit in Carrick-on-Shannon, Co Leitrim. The project is not included in the 2009 National Development Plan, Design and Build Programme under Older Persons. The project will receive further consideration for inclusion in the 2010 National Development Plan, the outcome of which will not be know until 1 st quarter, FinalMins_RHF#23_

6 W23Q459 Councillor Brian Meaney How many individuals are currently involuntarily detained under the Mental Act, in facilities in the HSE West region? What are the figures for 2007 and 2008? Response provided at the meeting. In 2007 there were 524 individuals detained under the MHA 2001 in facilities in the HSE West Region. In 2008 there were 484 individuals detained under the MHA 2001 in facilities in the HSE West Region. Figures in relation to the current year will not be available from the Mental Commission until W23Q460 Councillor Brian Meaney I request a full report on repayments made under the Nursing Home Repayment Scheme for each County in the HSE West. How many claims are outstanding? How many eligible claimants decided not to pursue repayment? The Service Executive (HSE) has responsibility for administrating the Repayment Scheme in conjunction with the appointed scheme administrator KPMG Accountants and McCann Fitzgerald Solicitors. The attached table indicates the number of claims received by the Scheme Administrator, the number of rejection decisions issued, offers completed and claims paid to claimants for each county in the HSE West. On the attached table under the heading Processing this relates to the number of claims remaining to be processed for each of the counties named in the West area. These claims have not yet been finalized (less than 3% of the total population) because there is a difficulty either with the institution records, with determining eligibility or because there is a complexity in relation to probate. The Scheme Administrator and the HSE continue to work closely together to resolve these issues and claims are been concluded on a weekly basis as issues are resolved. However it should be noted that the repayments offered cannot be paid until the statutory 28 day period for appeal has elapsed and a completed acceptance form has been returned to the Scheme Administrator. The Scheme Administrator and the HSE cannot provide an accurate figure to the question of how many eligible claimants decided not to pursue repayment, however an estimate of 70,000 claims nationally was expected prior to commencement of scheme and a total of 35,000 approximate claim forms has actually been submitted for the scheme. The HSE Longstay Charges Repayment Scheme Unit previously received feedback at Regional Forum meetings, which indicated that a high proportion of patients resident at public health care facilities were satisfied with the level of care provided and these residents and or their representatives did not intend making a claim for refund of any charges due. 8 In relation to the number of claimants entitled to claim under the scheme who did not claim, this information is not available on a county by county basis. However, the position nationally is as follows: Potential Claimants When the scheme was originally introduced, the best estimate available at that time indicated that there were 70,000 potential claims. It should be noted that this was only an estimate as the figures were based on records of varying quality over a 30 year period. Actual Claimants 35,100 applications have been received under the scheme. Latest estimates received from the scheme administrator indicate that when adjusted for rejections etc., the number of valid claims that will be received under the scheme, will be approximately 21,200. The Reasons the number of claims is less than anticipated: The legal and technical standards for payments under the scheme have meant the process is more complex than originally envisaged High level of deficiencies on claim forms (40% - Signatures; Statutory Declaration; Medical Certs; Income Details) Tax implications for potential claimants. W23Q461 Councillor Brian Meaney What was the incidence of resistant infections (MRSA etc) in acute hospitals in the HSE West in 2007 and 2008? What figures are available for 2009? Is there an identifiable budget for dealing with hospital acquired infections in the HSE West, and if so, what is it and is there a breakdown by County, and how many staff are specifically deployed in the region and where in the control of HAI? WEST/NORTHWEST MRSA bloodstream infections have been reported to Protection Surveillance [HPSC] since 2006 by all the acute hospitals in the network and are reported annually. See attached statistics from the latest report (April 2009). Infection control costs form a part of the general hygiene, cleaning, laboratory costs in the budget of each hospital. Expenditure on infection control is allocated depending on the requirements, e.g., to respond to cases as they arise. W23Q462 Councillor Brian Meaney At what stage is the proposed upgrade of Ennis General and given the deflation in the construction sector, has the overall cost been re-evaluated? In order to comply with the HSE s Executive plans to reconfigure acute hospital services in the mid west region, the facilities at the Mid West Regional Ennis have been examined and recommended for upgrade, refurbishment and extension, where appropriate, in order to provide an environment suitable to meet the changing service for current and future practice. A project briefing document has now been finalised which will form part of the overall regional strategy to provide an infrastructural project to allow this new delivery of care. Included in the first phase of development are the following infrastructural developments; - Local Emergency, Assessment Beds, Theatres, and an Integrated Out Patient Service are now considered a priority. Funding will be sought to advance these projects on a priority basis from The estimate has been calculated based on current construction cost indices. In the shorter term as an interim measure a new endoscopy facility is currently being designed with an associated day ward which will increase capacity for day case activities. It is hoped that this unit will be completed by mid Recent capital works have provided enhanced facilities for pathology with new infrastructure and equipment also being provided for CT services which were completed in July In addition to these departments, an inpatient ward accommodation project delivering 50 inpatients beds is being procured under a national procurement initiative and funding is being sought which if approved will form an integral part of the infrastructure required for the reconfigured delivery of service. W23Q463 Councillor Austin Francis O Malley How many beds are allocated for cancer patients transferred from Castlebar General for treatment in the University? Can you tell me if there is a separate allocation of beds for Sligo patients? The NCCP prioritised the transfer of symptomatic breast services to the designated specialist cancer centres. Symptomatic breast services transferred from Mayo General to University in November last year and from Sligo General in August of this year. 2 beds are allocated for cancer patients transferred from Mayo General and 3 beds for patients transferred from Sligo. The is working towards the re-opening of the Theatre and St. Johns Ward and the opening of the Medical assessment Unit A Working Group has been established to progress with the development of increased capacity in the Day Ward from 8 day beds to 18 day beds and the development of a 5 Day Ward. In addition, the has relocated the Special Baby Unit to a new unit with an increased capacity from 7 10 cots which include 2 Intensive Cots, 4 High Dependency Cots and 4 Special Baby Unit Cots. W23Q465 Councillor Michael Mullins Since our last meeting, what progress has been made on efforts to resolve the ongoing dispute involving Nurse Managers at Portiuncula? Mediation is ongoing under the auspices if the Labour Court. W23Q466 Councillor Michael Mullins What plans has the HSE to increase the resources of the Community Welfare Officers, so that Medical Card applications can be processed in a timely manner, and large backlogs cleared? Due to the recent economic downturn, there has been a significant increase in workloads for all Community Welfare staff throughout the Country during the period 2008/2009. The Community Welfare Service is a very unique service serving a very vulnerable population and one of its defining characteristics that it has to provide an immediate and urgent response to customer s financial and other needs. A National Business Case for Additional Resources to support the Community Welfare Service was made in May The Business Case recommended that 37 additional Community Welfare Officers be made available to administer the Back to School Clothing & Footwear Scheme for A further 25 Community Welfare Officers were recommended across 8 of the former Regional Boards (4 for HSE West) to provide support to the additional work associated with the downturn in the Economy. These posts were approved by the Dept of and Children, Dept of Social and Family Affairs and the Dept of Finance. Currently the 37 posts identified to administer the Back to School Scheme are in place and the scheme is at an advanced stage. Of the 25 posts identified and approved in recognition of the impact of the downturn in the economy, 15 are in place. A process for fast tracking the filling of the remaining posts was presented to the relevant Unions on the 17th of September and they are considering same. The Unions have also been invited by HSE Management to become involved in a process to examine the current caseloads and to examine what has happened since the original business case was finalised. Finally the clearly measurable increases in Community Welfare Services workload and potential for further increase, has strengthened a well recognised requirement to address the workload and operational issues of the service. The current service model, in place for many decades, relies largely on lone individual staff members managing community welfare clinics in a fragmented geographical framework. In addition, analysis of the workload division across the country shows discrepancies in workload distribution within areas which require equalisation across the available FinalMins_RHF#23_

14 staff resources. Such an examination of how best to reconfigure service delivery is also a pivotal part of the solution to the CWOs workload issues. All of the above will hopefully contribute to an easing of the pressures on CWOs and thereby enable them to conduct all of their duties in a timely manner and clear whatever backlogs exist. Please find the schedule for team development listed below: Primary Teams Team areas Teams in Place Tuam, Athenry, South Connemara, Oughterard, Shantalla and Knocknacarra Teams commencing 2009 Turloughmore, Clifden, Ballinasloe Teams commencing 2010 City (2), Mountbellew, Glenamaddy, Gort, Abbeyknockmoy, Loughrea Capital and infrastructure projects for Primary Centers are being progressed in Inis Boffin, Glenamaddy, Oranmore, Athenry, Portumna, City, Turloughmore and Clifden. Existing s are being used as the base for the existing and new Primary Teams. W23Q468 Councillor Catherine Connolly Please furnish the minutes of the Western Board dealing with the purchase of the house at No. 1 The Mass Path, Newcastle and the purpose for which it was bought and please give specific details of how many other properties owned and/or leased by the HSE West in City and County remain unused/unoccupied, for how long and why, together with the most up to date details of all properties rented and/or leased by the HSE West in City and County. Human Resources The house in question was purchased by UHG in December 2000 at a cost of 350,000 punts. HR has provided funding towards this project as the intended use of this house is for Occupational Services. HR does not have any unused/unoccupied properties. The Employee Support Service currently leases premises at Unit 4C, Oranmore Business Park, Oranmore,. The Terms of the Agreement is from 1 st May 09 1 st May 2011 and the total cost per year, 37,665 pa, is payable every three months at a cost of 9,416. PCCC provided at the meeting. I wish to confirm the following 4 properties are currently vacant in City and County: 14/15 Bar Aille, Tuam Road, - HSE owned. Premises currently vacant as childcare services have now been amalgamated with and to, Aras Gael. It has now been discussed and established to utilise the property as a base for a Primary Team in the near future. on Inis Oirr HSE owned.
