2 welcome At IHS, we believe that a healthy nonprofit sector is vital to maintaining the quality of life in a community. We also believe that nonprofit organizations are most effective when working together, sharing and leveraging resources, and continually striving to improve how they do their work. As you will see in the following Annual Report, 2015 was an exciting year for the Institute. In addition to very successful outcomes across our services and programs, we relocated our offices to 50 Liberty Street in Bath, fully upgraded our IT system, and implemented state of the art call center technology. This now provides us a very effective infrastructure and professional office environment. Because of greatly increasing demand, we developed a new transportation call center, expanded and improved our HELPLINE contact center, and enhanced our capabilities in both with the implementation of an advanced digital based integrated telephone system. In 2016, we continue to focus on the evolving needs, trends, and best practices in the nonprofit sector, as well as align our services and resources across the organization to address the four IHS organizational goals: Be the regional leader in nonprofit information and communication. Be the leader in providing structure and services to enhance regional collaboration. Be the first point of contact for regional nonprofit capacity building. Be a model for nonprofit organization sustainability through effective governance, leadership, and management brings all of our member organizations new challenges and new opportunities. As a community of nonprofit and public sector organizations, we have to continually assess what we do, how we do it, and who we can collaborate and partner with that will improve the effectiveness, efficiency, and sustainability of our work and organization. IHS will continue to provide professional communication, collaboration, and capacity building services that strengthen our regional nonprofit organizations and build awareness of the role and impact that our members have in the community. We always appreciate the skills, professionalism, and dedication of our member organizations - as well as the impact they make in the community. This sustainable growth enables us to develop our capacity to serve our members while expanding services in critical areas. By leveraging staff skills, resources, and technology, we have developed an integrated portfolio of programs and services that provides a diverse funding stream. We continually look across the nonprofit and profit-based sectors to identify and implement best practices in organizational management and capacity building.

Online with Most Common Search Categories in 2015: Food Pantries Low-Income Housing Rent Payment Assistance Electric Bill Assistance Volunteering An entirely new website was launched in March, designed to be responsive to screen size/mobile device access, with a robust content management system that enables us to customize suggested searches, add seasonal content, and better support disaster response & recovery efforts. We also created DDinfo and Teen HELPLINE, specialized sites for those with intellectual/developmental disabilities and adolescents, respectively. Comprehensive Database In addition to the contact center, the health and human services database is one of the key components of the HELPLINE program. It currently comprises 572 agency records covering 1,840 programs, each of which is updated on a regular basis. In 2015, there were 449 agency records updated equaling 78.5 dial percent of the entire database! HELPLINE connects people with the services they need throughout Steuben, Chemung, Allegany, Schuyler, and Yates Counties, through a contact center, website, contracted services, public outreach, and other means. Comprehensive information and referral has been a service of the Institute since its founding in IHS continues to partner with Goodwill of the Finger Lakes and the Human Services Coalition of Tompkins County to provide service across a 13 county area, as a member of the Finger Lakes Collaborative. In 2015, 33,731 contacts were met with 35,299 referrals, with the top categories of need including health care, income support and assistance, and transportation. Website sessions, tracked separately, reflected searches for food pantries, low-income housing, rent payment assistance, electric bill assistance, and volunteering. Unmet Needs Of the needs identified in 2015, 1,198 were logged as unmet, for which we did not make a referral to an agency or program. Reasons for unmet needs include the service not being available, the caller not being eligible for available services, the service being too far away, or transportation not being available, among others. New Website! An entirely new website was launched in March 2015, designed to be responsive to screen size/mobile device access. The new site consists of a robust content management system, enabling the customization of suggested searches, the addition of seasonal content, and the provision of better support disaster response and recovery efforts. Resource data are uploaded daily and are searchable by service or agency, with options for further filtering, as well. The new site includes online chat as an additional way to connect individuals in need of help to find services or volunteer opportunities. Microsites In addition to the launch of the new website in 2015, microsites were created as extensions of the HELPLINE service DDinfo is an online, searchable database that provides detailed information about the services available in Steuben, Chemung, Schuyler, Allegany, and Yates Counties, specifically for people with developmental disabilities, their families, and service providers. A free, safe, and confidential source of support and information for youth, parents, and professionals, Teen HELPLINE operates 24/7 through various platforms including calling the contact center, searching the website, and chatting online for meaningful, guided connections to help. Comprehensive Database In addition to the contact center, the health and human services database is one of the key components of the HELPLINE program. It currently comprises 572 agency records covering 1,840 programs, each of which is updated on a regular basis. Caller Satisfaction in 2015 In 2015, 65 follow up calls were logged. Of these, 100% of callers were satisfied or very satisfied, with 98.1% stating they would call HELPLINE again.

5 2-1-1 HELPLINE Seasonal Initiatives HELPLINE s seasonal initiatives involve compiling specialized resources for the following efforts, providing opportunities for donations and accessing the services: Income Tax & Financial Literacy Summer Food Service Programs School Supplies Holiday Caring 211counts 211counts (211helpline.211counts.org) is a new dashboard for viewing call volume in (almost) real time. Data are uploaded daily from our call logs and may be refined by date, county, or zip code, with options for further exploring each category of need in greater detail. Partners include New York, University of Washington St. Louis Health Communication Research Laboratory, and the Kenan Charitable Trust ST/FL VOAD The Southern Tier/Finger Lakes Voluntary Organizations Active in Disaster (VOAD) is a collaboration among IHS, County Emergency Managers, American Red Cross, Salvation Army, and other public agencies and nonprofit organizations. In addition to strengthening our relationship with NY Department of Homeland Security and Emergency Services in 2015, the ST/FL VOAD tested the regional disaster volunteer management plan in cooperation with Lutheran Disaster Response regional representative and our intern. Contract Services and Special Projects HELPLINE was invited to participate in a disaster drill involving county and state agencies across all of Western New York, and represent IHS at the Steuben County Emergency Operations Center; this followed a statewide review of s disaster plans. This exercise allowed us to test our information management processes, as well. Volunteer Income Tax programs: VITA/CA$H/AARP In partnership with Arbor Housing and Development, Allegany County United Way, and Pro Action of Steuben & Yates, Inc DDinfo: Launched website & DDinfo at IHS conference; Webinar offered in November In partnership with New York and the New York State Developmental Disabilities Planning Council ACA Appointments In partnership with AIM Independent Living Center and S2AY Rural Health Network Teen HELPLINE In partnership with the Steuben Rural Health Network and Chemung County Youth Bureau 511NY Southern Tier Rideshare: Customer Support In partnership with the Elmira Chemung Transportation Council and IHS Coordinated Transportation Services Volunteer Management Support In partnership with the United Way of the Southern Tier (reorganized effective Fall 2015) Additional Updates We welcomed Michelle Hall as a new full-time staff member, and Michelle Dartt and Nicole Archibald, interns from the Alfred State College s Human Services Management program, provided invaluable support for the HELPLINE program. Thanks also to Louise Salerno, our loyal volunteer! Michelle Dartt joined the staff effective January 2016.

6 Steuben Rural Health Network The Steuben Rural Health Network embarked upon a challenging and progressive work plan in 2015, with initiatives supported by the Charles D. Cook Office for Rural Health, NYSDOH. With IHS serving as the lead agency with 40+ collaborating Network Partners representing Southern Tier healthcare and human service organizations, SRHN successfully met and/or surpassed all goals outlined in the annual work plan. Chronic Disease Self-Management With a focus on prevention and health education, the Steuben Rural Health Network continued to offer the Stanford Chronic Disease Self-Management Program and the Diabetes Self-Management Program, locally known as Living Healthy Southern Tier, at sites throughout Steuben and Chemung Counties. The National Diabetes Prevention Program, a 16 week weight-loss and activity program through the CDC, was completed by 17 participants in 2015,with participants losing 5-7% of their original weight when starting program. Girls on the Run of the Southern Tier SRHN expanded Girls on the Run, an experience-based empowerment program, to 16 sites throughout Steuben, Chemung, Schuyler, Yates, and Allegany Counties, serving 290 girls in The 5k Celebration, hosted by the Campbell-Savona School District, brought in over 600 runners and 1,000+ spectators cheering on the girls and their adult running buddies. Over 100 5k volunteers and 45 coaches supported the program, along with numerous community sponsors to include Guthrie, the Wineglass Marathon, and Excellus BlueCross BlueShield. INSPIRE Adolescent Health & Wellness 290 girls in 3rd - 5th grade served throughout the Southern Tier In acknowledging, supporting, and implementing a holistic approach to adolescent health and wellness, the former Teen Parent Panel was transformed into the newly created Youth Leadership Council in The Youth Leadership Council is an innovative program that supports young people in advocating for their own health and the health of their peers, engaging youth and their communities through peer education. In coordinating, facilitating, and leading INSPIRE s peer education sessions in schools, after-school programs, community centers, and more, the Youth Leadership Council provided innovative, informative, and inspirational presentations to over 400 youth in The newest installment to the Staying Inspired documentary series was made available in November A resource for youth and adults alike, the series features real teens sharing their stories of overcoming life changing obstacles. The most recent addition to the series, titled Staying Inspired: Leading a Healthy Lifestyle, highlights Noah, an 18-year-old athlete from Bath, New York and recipient of the 2014 INSPIRE Award, who, after overcoming obstacles, decided to lead a healthy lifestyle inspiring his peers both on and off the field. The 7th Annual Adolescent Health & Wellness Conference was held in October 2015, bringing together over 100 youthserving professionals from throughout the Southern Tier - and beyond - for a full day of networking, information sharing, and a collective passion for working with adolescents. Highlights of the event included a State of the Union on Adolescent Health in New York State, as well as a keynote address made by nationally-renowned human sexuality educator, speaker, and author, Al Vernacchio. Planning & Resource Development IHS Planning & Resource Development works with county-based nonprofit and public providers to assess, plan, and fund projects to benefit the community, and also helps individual agencies plan, fund, and build organizational capacity. In 2015, major projects included several housing grants through Arbor Housing and Development, a multi-county federal grant for collaborative domestic violence and sexual assault victim services, and funds for Steuben DSS to help low-income parents boost employment to meet child support obligations. In 2015, the Planning & Resource Development department developed and submitted 22 applications. While 6 large-scale projects are still under review (and 11 were not selected for funding), 5 funded projects brought a total of $1,277,780 to the Southern Tier. 5 funded projects brought a total of $1.3M to the Southern Tier

7 NY-501 Regional Continuum of Care The NY-501 Regional Continuum of Care (CoC) represents a collaborative of 59 member organizations, operating as regional jurisdiction under the auspices of the U.S. Department of Housing and Urban Development and local leadership. The purpose of the CoC is to provide public policy, funding, supportive services, and housing to those at risk of or experiencing homelessness in Steuben, Allegany, Livingston, Schuyler, and Chemung Counties, as well as the City of Elmira. Secured & monitored $1.7M in HUD funding In 2015, the Institute for Human Services as Collaborative Applicant for the CoC submitted on behalf of the Continuum an application for $1.7 million dollars in FY2015 HUD CoC Program funding which directly operate support services, over 400 units of housing, leverages funding for two domestic violence programs, and enables regional strategic planning and administrative efforts. Additionally, with the assistance and coordination of the Institute, the NY-501 Regional CoC conducted an annual Point In Time Count - a community based survey of homeless individuals, veterans, and families. In 2015, the PIT Count identified a total of 414 unsheltered or unstably housed people, including 68 families. This survey data is used by local, state, and federal policy makers to assist with planning and funding to end homelessness in our service region. Coordinated Transportation Services Volunteer Driver Program In 2015, the IHS Coordinated Transportation Volunteer Driver Program utilization increased over 300% to a total of 723,550 miles! 75 volunteer drivers helped individuals by providing transportation to attend medical, nutrition, and other necessary appointments. Because of our excellent service in meeting unmet transportation needs in rural areas, NYS requested IHS to expand beyond Steuben and Allegany Counties to begin providing Non-Emergency Medicaid Transportation volunteer driver services in both Cattaraugus and Livingston Counties. Volunteer drivers provided 59,394 miles for Steuben County Office of the Aging clients (+25%) and working with the Steuben County Department of Social Services, scheduled 4,164 client transports (+20%) by Steuben County drivers. In addition, IHS distributed 2,306 bus tokens (+16%) to the 3 offices of Catholic Charities Turning Point, providing over 65,000 miles of additional transportation for those in need. To meet the ever increasing needs, in 2016, we added 2 additional Transportation Specialists and implemented all new call center technology, in our new call center! Mobility Management 75 volunteer drivers dedicated their time in 2015, providing 723,550 miles of transport to attend medical, nutrition, and other necessary appointments In 2015, the Mobility Manager successfully updated the Steuben County Coordinated Public Transit-Human Services Transportation Plan, allowing several area partners to apply for and receive funding for capital and operational expenses. The Tri-County Map was printed and distributed throughout Steuben, Chemung, and Schuyler Counties. Bus buddy and travel training marketing efforts continued by direct community outreach and including the programs on the Need a Ride website to reach the community. The Mobility Manager worked with the Steuben County Planning Department in preparing the Consolidated Application for 5311 funding, which was approved and submitted to NYS DOT. A new Mobility Manager began at IHS in December 2015, which will allow Mobility Management activities to continue into In 2016, the Mobility Manager will work on two large projects involving the Steuben County Planning Department and NYSDOT. The Mobility Manager will work collaboratively with Steuben County Planning on a comprehensive transportation needs assessment to assist in coordination of transportation services within the county. Additionally, the Mobility Manager will work with NYSDOT to complete the GTFS input to better serve the county residents with trip planning activities.

8 AmeriCorps Southern Tier Opportunity Corps AmeriCorps Southern Tier Opportunity Corps (STOC) is a collaborative effort of local human service agencies in Steuben, Chemung, and Tompkins Counties that offers needed services to economically disadvantaged individuals and families in the area. Awarded through the NYS Regional Economic Development Councils competition in 2014, this new AmeriCorps grant is now in its 2nd year of operation. Service sites for AmeriCorps members include participating Workforce Development agencies, Department of Social Services, and human service agencies - all providing critically-needed employment and housing support to low-income clients. In 2015, 17 AmeriCorps members committed to a national term of service with the program. Members served at ten local sites and provided a total of 13,742 hours of service in AmeriCorps members work in the employment focus area encompassed GED and other needed educational development. Working with clients to complete job assessment and readiness testing, providing individual and group training resources, and mentoring them as they carried out needed job search activities to secure employment, the work of AmeriCorps members contributed to outcomes of improved readiness for employment & employability. In 2015, AmeriCorps members served 710 individuals, who completed a variety of targeted employment search/preparation activities designed to make them prepared for employment opportunities. AmeriCorps members also worked with local human service agencies in the housing focus area to help individuals, including the homeless, find resources to support use of appropriate housing, as well as assisted individuals in accessing housing. To assist clients in finding safe and affordable housing, AmeriCorps members identified their key housing issues, formulated service plans to resolve these issues, and utilized appropriate community resources to successfully implement such plans. All of these activities focused on achieving the outcome of transitioning clients to safe, affordable housing, in counties where such housing is difficult to find. This more intensive, individualized area of service resulted in members providing 53 individuals and/or families with counseling that enabled them to find safe and affordable housing. 17 AmeriCorps members committed to a national term of service in 2015 Members served at 10 local sites, providing a total of 13,742 hours Healthy Families Steuben 234 families served 3,089 home visits completed Healthy Families Steuben, a collaborative of the Institute for Human Services, Inc. and the Kinship Division of Catholic Charities of Steuben County, provides support and education to families with young children. This preventive program uses nationally accredited best practices and research based approaches to support positive parenting, encourage healthy habits, and increase community connections for new and expectant parents in Steuben County. In 2015, the program served 234 families and completed 3,089 home visits. Of these families, 70% of families entered the program prenatally, 62% had fathers engaged in services, 100% of children, as well as primary parents, were connected to a medical provider, and 74% of families were employed, or enrolled in an educational or job training program, by the time their child was 1 year old. Additionally, 97% of children were up-to-date with recommended immunizations at 1 year of age and 100% were assessed for the risk of lead exposure. Healthy Families Steuben consistently meets the performance targets set forth by New York State Office of Children and Family Services and had 21 families graduate from the program in Healthy Families Steuben celebrated their 20th Anniversary milestone in Among other activities, the program delivered cupcakes to community partners and held a picnic for past and present participants to mark the occasion.

9 Communications The Institute for Human Services, Inc. serves as a source of local agency news and resources for nonprofit organizations throughout the Southern Tier region. With a focus on increasing outreach via multiple outlets in 2015, IHS continued the process of streamlining communications across all channels, distributing content through websites, e-newsletters, social media, press releases, and more. 70,000+ visits to the 6 websites managed by IHS Communications Member Agency news, events, job postings, and more were shared across 11 social network accounts In 2015, the 6 websites managed by IHS had over 70,000 total visits, representing a 22.9% increase over the previous year. The Provider weekly e-newsletter served as a central source of regional nonprofit news, reaching more than 2,000 professionals in Social media played an imperative role in IHS communications, strengthening the promotion and outreach of various programs. Across the 11 social media accounts managed by IHS, campaigns were created and implemented throughout the year to develop engagement and brand recognition. Such campaigns included #MemberMonday, Nonprofit Awareness Month, National Rural Health Day, and #211Tuesday features. With a specific emphasis on utilizing Facebook, page likes in 2015 increased by 521, providing a total of 1,914 followers at year end. Education & Professional Development IHS offers high quality, cost effective, and relevant professional development opportunities throughout the year to meet the educational demands of the local nonprofit sector. Educational opportunities vary from trainings for frontline staff and middle managers to opportunities for executive directors, board members, and volunteer managers. IHS professional development offerings are designed to share knowledge, build leadership skills, and provide unique tools and techniques for local nonprofits.
