With PresCare you can get the extra support you might need, like an extra pair of hands around the house, or help with the shopping or simply have someone there to sit and have a cup of coffee with. Aged care experts who come to you PresCare provides support that is more you. We have been providing quality aged care for 90 years. The quality care and support that you would have in an aged care residence, without having to leave the comfort of your home. Great people make it feel easy Asking for help may not be easy for some. We do our best to make you feel that nothing is too much trouble. Because nothing is too much trouble. Our highly-trained staff will spend a lot of time getting to know you and what you need, so they will know how you like things done. And we won t send someone different each time. Quality care, whatever your situation We offer access to an extensive range of support services and activities tailored to meet individual needs. We take a relationship based approach which enables us to fully understand the needs of each client. A co-ordinator will make sure the services you receive meet your changing needs, a registered nurse makes sure your health remains our number one priority and a wide range of health professionals work hard to help you stay at home for as long as possible. Our specialists help you enjoy your time at home. You have access to a large range of highly qualified health professionals such as a physiotherapist, podiatrist, massage therapist and if you need them access to dieticians, occupational therapists and exercise physiologists. 6 In-home personal care Our experienced and friendly community care workers are available to help with your personal care needs such as assisting with medications, showering and toileting, moving around the house, general household duties (e.g. cleaning), meal preparation, and shopping. In-home nursing PresCare s qualified nurses will help with your clinical care needs such as wound dressings, medications, and more. You have the option to include regular access to nurses as part of your packaged care, or just when you feel you need additional clinical care and support. See page 9 for full details about our nursing services. Rehabilitation and physical therapy PresCare s expert team is available to help you recover from a setback or simply to maintain your physical health so you can stay safe living at home and being a part of your community. Exercise physiology Individual or group sessions are run by accredited exercise physiologists who specialise in graded exercise therapy and lifestyle interventions for those at risk of developing, or currently living with, existing chronic and complex medical conditions and injuries. Occupational therapy If you need help to move around your home safely and comfortably, our occupational therapists provide an invaluable service including advice and assistance about what adaptive equipment to purchase and install as well as appropriate recommendations for home modifications. We also provide you and your extended support team with education on falls prevention, work simplification and energy conservation in the home to increase independence and safety. Podiatry The health of feet and associated bones, muscles and blood circulation are critical in allowing us to keep moving. We offer preventative care, treatment and management of a wide range of medical conditions and injuries of the foot, ankle and lower limb. Physiotherapy We provide individual treatments to assist with pain relief, increased range of movement, improved muscle strength, maintaining mobility and activities of daily living and post-hospital discharge care. Treatments may be provided in-home or at a centre. We also offer specialised group exercise sessions at our centres to improve muscle strength, range of movement in upper and lower limbs, prevention of falls and improvement in function. Speech pathology This specialisation allows us to assist with communication and swallowing difficulties, and also with the maintenance of hearing aids. Communication and swallowing difficulties often result from neurological problems (dementia, Parkinson s disease, strokes) and sensory loss. Our specialists work with you to improve these functions. Education is also provided to carers and family members. Respite care (day and overnight) At PresCare we offer a range of flexible options for you and your loved ones to take a break. Our coordinators will work with you to find a service or solution that is tailored to your specific needs. 6 Version: September

These may include minor repairs such as changing light bulbs and replacing tap washers; Home modifications can include installing: grab and shower rails, ramps, emergency alarms and other safety aids, such as hand rails. Home and garden maintenance We understand that sometimes it may be a little challenging for you to keep on top of home and garden maintenance. That s where we can help by arranging for house cleaners and gardeners to keep your home environment spick and span. Meal services What would you prefer? We can come to your home and cook for you, or you can simply order delicious prepared meals, delivered to your door by our friends at Contented Chef.

It may also be the perfect time to make new friends and learn new things and your community care worker can help you with this too. Mobility equipment Need help getting around? From power chairs to bathroom aids and kitchen equipment, we have what you need to keep moving. Transport to and from appointments Get the VIP treatment on those trips to the doctor, dentist or accountant. We offer a friendly, door-todoor service to make life a lot easier. Chaplaincy, pastoral and spiritual care We all have spiritual needs, which emerge when we ask ourselves the big questions in life. That s where PresCare chaplains come in. We are a proud Christian organisation offering a range of spiritual services to our community. Dementia care services Supporting people living with dementia requires a special level of care and sensitivity. At PresCare, we understand these different stages of dementia require specialist knowledge and support. Other services Everything we do is about you. Making sure that you are able to maintain your independence and lifestyle while staying connected to your family, friends and local community. So if there are other services that you would like to know about for example, travel companionship, pet care support, cultural activities and more please discuss with your PresCare Coordinator or community care worker. 8 Version: September
9 IN-HOME NURSING SERVICES Choice is at the heart of everything we do at PresCare and that includes our nursing packages that let you choose what best suits your needs. ON DEMAND SERVICES We offer at home nursing services outlined below as and when you need them. NURSING ACCESS PACKAGE Proactive health management and preventative care with nursing services available as clinical needs arise. NURSING COMPREHENSIVE PACKAGE Proactive and regular in-home clinical care and treatment to support physical and mental health needs and improved quality of life. Includes regular care consultations with family and carers. Proactive health management Proactive health management through monthly checks to monitor health status, early identification and referral. In addition, consultation with GPs and other health professionals will be held, as needed, to ensure you get timely access to appropriate care. Proactive health management through weekly health reviews to closely monitor health status and signs of clinical deterioration, including dementia care. Provide consistent ongoing treatments with regular consultation with GPs and other health professionals. In addition, monthly family case conferencing is available either in-person or through video conferencing to support family members in understanding care needs. Clinical care Advanced care planning Medical management Medical and care supplies Palliative care Provide clinical care as needed including treatment of wounds, pressure injuries, and skin tears. Also provide education to you, carers and family, on pressure injury and falls prevention. Support in the advanced care planning process, including arranging enduring power of attorney, advance care plan/ advance health directive. Monthly review of medication and impact on your health. Assessment and supply of clinical care needs, including continence aids and oxygen therapy. Delivering regular treatments, including complex wound care, preventing pressure injuries, palliative care, stomach care, and catheter management. Also support and educate families with management of clinical care. Support for you and your family in the advanced care planning process, including arranging enduring power of attorney, advance care plan/advance health directive. Close monitoring of medication and the impact on your health, including pain and diabetes management, and oxygen therapy. Assessment and monitoring of changing clinical care needs, and arranging supply of consumables as needed. Providing ongoing support and care during the palliative and end-of-life phase, including organising equipment needs, linking with specialist palliative care services, and identifying grief and loss needs. 11 Home Care Packages A Home Care Package is a type of government funding to help you remain living at home for longer. It is provided by the Australian Government to help people aged 65 years and over to obtain the in-home care and supports they need to stay living safely and independently in their own home for as long as possible. Home Care Packages are delivered with a consumer directed care approach. This means you are given a budget (funding) to help cover the cost of your coordinated care plan and you decide how it is allocated. This approach gives you more choice and flexibility about the types of services and supports you need and want. There are four levels of home care packages to help meet the different levels of care needs. During your aged care assessment, your assessor will discuss your current care needs and determine the best level to meet them. Level 1 Support packages for older people with basic care needs Level 2 Support packages for older people with low-level care needs Level 3 Support packages for older people with intermediate care needs Level 4 Support packages for older people with high-level care needs Refer to My Aged Care for current subsidies
12 How to access PresCare Home Care Packages PresCare is an approved Home Care Package provider and we re here to help you and your loved ones maintain their independence at home through a range of aged care services including, but not limited to: nursing, cleaning services, homecare, personal care and household assistance services. How to access a home care package When doing things around the home might becoming a bit of a struggle then there are some important steps you will need to undertake to gain access to a Home Care package. STEP 1 STEP 2 Seek assistance Discuss your situation with a Home Care Specialist from PresCare, your Doctor or contact My Aged Care on or Meet with ACAT The Aged Care Assessment Team (ACAT) will arrange to meet with you to determine if you are eligible for a home care package and which package level best meets your care needs. STEP 3 Choose a home Care provider Once you have been approved and assigned a home care package for support, you can choose a home care provider like PresCare. STEP 4 Contact PresCare Phone us on to make an appointment to meet with one of our Co-ordinators. STEP 5 STEP 6 Meet your Co-ordinator Your Co-ordinator will meet with you to discuss your requirements. Your support plan is developed Once your requirements have been identified your Co-ordinator will work with you and your support person to develop a plan and complete the necessary paperwork which includes the home care agreement. STEP 7 You receive your support The support services you receive will help you live independently in your own home and will be reviewed to ensure your needs are met as things change. 12 Version: September
13 What costs to expect The cost of aged care services depends on your situation, including the type of services you need and your financial situation. The cost you pay will depend on your income and is determined by the Department of Human Services. The funding you receive will vary depending on the level of your Home Care Package and is paid direct to your service provider. Where your personal circumstances allow, you will be expected to contribute to the cost of your care. EVERYDAY SERVICES PROVIDED BY PRESCARE Home care and support services discounts may be applied for overnight or extended services. SPECIAL PACKAGES AND SERVICES Home nursing packages Day Respite at PresCare centres (Inala, Sherwood, Fulton) Pastoral care and visits ADDITIONAL CHARGES Transport services where a vehicle is provided Travel costs applies to all services except those delivered under the Nursing Packages YOUR CHOICE SERVICES Allied health and alternative therapies physiotherapy, podiatry, occupational therapy, remedial massages, hydrotherapy and more. Home and garden services Spring cleaning, gardening, repairs, modifications and other services. Other services including requests to broker specific carers and nurses. Meal packages will require a contribution towards the cost of the food. Equipment and other items assistive aids and equipment, personal alarms, continence aids, and other items approved to relate to care needs. (capped at $100 for items over $1,000 in value) OTHER FEES AND CHARGES Care co-ordination Additional care co-ordination procurement of specially requested services, additional assessments undertaken during the year, special requests, etc. Administration Exit fees Late cancellation fees cancellations must be advised 48 before service is scheduled, otherwise PresCare must still pay the carer and thus the full cost will be charged to the client, capped at 2 hours. 14 QUESTIONS AND ANSWERS How do I arrange this funding? To be allocated a Home Care Package, you must first be assessed by the Aged Care Assessment Team (ACAT). This assessment will determine your package level. To arrange an assessment, contact My Aged Care by calling If you are currently completing an ACAT assessment or you re about to be, it s time to talk with us. I need help getting assessed, can you help me? We can support you through the process of getting assessed for funding. Please give us a call on Am I able to have the same workers each week to provide me with stability? At PresCare we understand the importance of building relationships between clients and staff and you will have a regular team of dedicated people who you will become familiar with. If your usual team are away on leave or unavailable, this will always be communicated to you prior to your visit. How do I switch from my current provider to Prescare? Home care package holders are able to change their provider if they are unhappy with their services or move. In most cases, your current provider will charge you an exit fee which will be taken from the funds available in your package, if you have funds available. Talk with your provider for the cost that applies to you as these are different for each provider. We can assist you with the switching process. You will need to: Obtain a referral code from My Aged Care by calling them on Notify your existing provider that you will be moving your package and what date. Call PresCare an and supply them with the referral code and the date you want the care to commence from. What can PresCare help with? We offer an extensive range of support services whether it is someone to call in a few times a week to help cook and clean? Maybe you need some help to shower, dress and take care of the personal things? Or maybe you just need some help to get out and about to the social stuff you love. Or someone to help you take your dog for a walk. What should I expect when an Aged Care Assessment Team visits? In most cases they will visit you at home but the assessment can take place in hospital or a rehabilitation centre if that s where you are. When they visit their goal is to work out how much help is needed in your particular situation. They look at things like your health and medical needs your physical needs your psychological needs your social needs special needs that are important to you and your lifestyle. The assessment is free and usually takes around an hour. You may feel anxious at the idea of getting assessed but the team are simply there to find out what types of care and services will help you best. You can even have a friend or relative with you and don t worry, you don t need to make any decisions at the time. 14 Version: September
15 I am currently on a waitlist to receive a Home Care Package; can I start to receive my services from you before my package comes through? Yes, talk to us today about how to commence your home care. Do my care hours need to be consecutive? No, your care hours can be spread over days or weeks. We will work with you to make sure you are getting the care and support you need. Do you provide nursing care as well? Yes we do, PresCare has home nursing packages which start from $300 a month. This means you have access to our registered nursing staff whenever you need during the month. This might include a daily visit to assist with any nursing requirements you might have or an additional once off visit if something crops up where nursing support is required. Alternatively, you can access nursing services as you need it, and be charged an hourly rate.
