2 Who we are Silver Chain Group is a not-for-profit organisation and the largest provider of community-based palliative care services in Australia. Each day we support more than 1,000 patients, visiting in the home when you need it most. We have been providing palliative care services throughout Australia for more than 30 years. We are comprised of Silver Chain in Western Australia, New South Wales, Victoria and Queensland and the Royal District Nursing Service (RDNS) in South Australia. Our palliative care service is focused on increasing comfort and ease in your home; achieving the best quality of life possible, surrounded by the people and things you love most. Attention is given to every part of your wellbeing, including physical symptoms, psychosocial and spiritual concerns. In New South Wales, we already support the Last Days of Life program in eight Local Health Districts spread all across the state. Our new service in Western Sydney is based on our very successful Perth model of care, which is recognised nationally as a best practice model of community-based palliative care. 2 3

3 What services are available Registered Nurses with specialised palliative care skills are available 24 hours a day, seven days a week, for both phone support and home visits. Doctors both specialist palliative care doctors and GPs with a special interest in palliative care, who will work closely with your current doctors, also providing specialist symptom management and home visits as required. Assistants in Nursing who will provide personal care and if required, respite (up to 4 hours) and support for your primary carer. Social workers who will help address the emotional distress that can be caused by a terminal diagnosis or disease, as well as practical guidance around finances, accommodation and social support. A Spiritual Care Worker to listen to your thoughts and help manage emotions. Supporting you to make sense of what is happening and reduce some fears and anxieties. A volunteer coordinator and a team of volunteers to provide additional support such as transport, in-home visits for respite and to keep you company when needed. Access to other allied health staff to help meet your specific needs, such as occupational therapists, physiotherapists, and nutritionists. We also provide basic equipment for use in the home. 4 5

4 Support for carers and/or family members Our palliative care service provides support to both you and those surrounding you. We have a bereavement support service for families and carers. We understand grief and loss experienced through the death of a loved one is different for each person, and is one of the most traumatic events that can occur in our lives. It is only through our own personal resources, and resilience the support of others, that we are able to incorporate the experience into our lives. days a week.
