OPR: 436 AW/SEW Supersedes: DOVERAFBI July 2006 Certified by: 436 AW/SE (Lt Col Kit Conn) Pages: 17 This instruction establishes responsibilities for mishap prevention in handling, storing, transporting, identifying, and controlling explosives and hazardous materials in accordance with Air Force Policy Directive (AFPD) 91-2, Safety Program in conjunction with corresponding Department of Defense (DoD), Air Force, AMC, and 18th AF directives, establishing unit-level Weapons Safety Program Management for Dover AFB. This instruction applies to all Dover Air Force Base (DAFB) host and tenant units (i.e. 436 AW, 512 AW, AFMES, JPED, etc) that store, transport, handle, or utilize explosives in the performance of their assigned mission. This instruction clarifies wing and unit responsibilities, establishes local processes for unit-level weapons safety program management, and addresses various other related weapon safety program issues that are unique to DAFB operations. This DAFBI does not supersede higher authoritative guidance. Ensure that all records created as a result of processes prescribed in this publication are maintained IAW Air Force Manual (AFMAN) , Management of Records, and disposed of IAW Air Force Records Information Management System (AFRIMS) Records Disposition Schedule (RDS). Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the Air Force (AF) Form 847, Recommendation for Change of Publication; route AF Form 847s from the field through the appropriate functional s chain of command.

This change clarifies the roles and responsibilities of units at squadron-level or above with missions that include explosives and requirements of the weapons safety program; revises the current explosives primary explosives route; provides direct links to the D8 base explosives map, provides information on Hazards of Electromagnetic Radiation to Ordnance (HERO) program; includes Nuclear Certified Equipment (NCE) and the USAF DULL SWORD Reporting Program to include DULL SWORD worksheet; provides clarification for the maintenance, transportation and storage of Flotation Equipment Deployment System (FEDS) operations.

3 DOVERAFBI JUNE Chapter 1 PROGRAM MANAGEMENT 1.1. Weapon Safety Program Management. In accordance with (IAW) AFI , The US Air Force Mishap Prevention Program, units at squadron-level or above with missions that include explosives must have a weapons safety program. The 436 Airlift Wing weapons safety office (436 AW/SEW) is host and coordinates all weapons safety requirements for the base Additional Duty Weapons Safety Representative (ADWSR) Appointment Unit commanders will appoint at least one primary and one alternate ADWSR to manage the unit s weapon safety program. As a minimum, ADWSRs must hold a 5-skill level, have a minimum of one year retainability and have a working knowledge of the area of responsibility. ADWSRs will be trained within 30 days of appointment. In the event the primary and alternate ADWSRs are to be absent for more than 30 days, a new primary ADWSR shall be appointed Local Instructions. IAW AFMAN , units that utilize munitions must develop written instructions approved by the squadron commander or equivalent for all explosives operations. Dover AFB weapons local instructions will be written IAW , paragraphs (AMC) requiring the most restrictive guidance to be used either, AMC or AFRC. Local instructions must be coordinated through the appropriate 436/512 AW Weapons Safety Staff prior to squadron commander approval Local Instructions will be reviewed by 436 AW/SEW) during unit annual assessments. This review will be documented in the unit s assessment report. Unit review of local instructions will be conducted and documented IAW Dover AFB Publications/Forms Guide and/or other appropriate unit specific procedures. Notify 436 AW/SEW when unit local instructions are going into re-write or when changes are to be published Explosive Facilities License (EFL). Units requiring storage of ammunition and explosives must have a legitimate mission requirement to store limited amounts of operational explosives outside the base 436 MXS MSA. Legitimate is defined as those locations required to support continuous base defense and security missions, emergency response operations, and requirements supporting continuous aircraft mission operations Prepare AF IMT 2047, Explosive Facility License, IAW AFMAN for each ammunition and explosives storage locations that are not sited for explosives. The license will require unit commander approval and coordination through Security Forces Squadron (Resource Protection), Base Civil Engineering Squadron (Fire Department) and Munitions Accountable Systems Officer (MASO) prior to 436 AW/SEW final approval Explosive Safety Training. All personnel who operate, handle, transport, maintain, load, or dispose of explosives must receive initial weapons safety training before performing any of these tasks. Training conducted by ADWSR. Conduct recurring training prior to the end of the 15 th month thereafter.

4 4 DOVERAFBI JUNE Training will consist of the use of an approved lesson plan. Lesson plans must provide the student with general knowledge of explosives and the associated hazards and tailored to the specific duties and weapons systems of the unit. 436 AW/SEW will review unit lesson plans during annual safety assessments and will be documented in the annual assessment report. Notify 436 AW/SEW when unit lesson plans are to be re-written or be published Safety Inspection Program. Safety inspections help identify hazards and measure compliance with safety program requirements outlined in AFI , Air Force Consolidated Occupational Safety Instruction. Through inspections, the safety staff helps the commander determine the condition of work areas, the safety of work practices, the degree of compliance with safety standards, and the degree of compliance with safety program requirements. There are three types of inspections within the weapons safety program arena; spot, high-interest area (HIA), and annual (assessment). To provide assistance, the safety staff will use outlines as provided in AFI AW/SEW will: Inspect all explosive locations at least once a year, except as otherwise directed Inspect all high-interest areas monthly by spot inspection and annually as part of the explosive location inspection Inspect all explosive locations periodically, through spot inspections Inspections will be either "prior-notice" or "no-notice." Inspections will be combined with other inspections to reduce the number of inspections a unit receives Weapons Safety Award Program. An integral part of an effective and successful unit safety program is recognition. The Wing Safety Office provides this recognition through the wing safety awards program. Safety awards are awarded in several categories on a quarterly and annual basis per AFI , Safety Awards. Submit award nominations IAW formats and suspense s as directed through local policy and as listed in DAFB writing guide. Contact 436 AW/SEW for further guidance on nomination procedures.

5 DOVERAFBI JUNE Chapter 2 ROLES AND RESPONSIBILITIES 2.1. Mishap Notification: In the event of an explosives mishap, i.e., malfunction of an explosive item, unintentional functioning, dropping, etc., notify base agencies as directed by unit local instructions or regulations. Stop all operations and evacuate personnel to a safe withdrawal distance. Do not move equipment, explosives, etc., until a thorough investigation is completed. Resume operations only when authorized by the On-Scene Commander Program Elements: All organizations having a mission or participating in functions involving explosives or hazardous materials are responsible for developing and implementing an effective explosives mishap prevention program. These organizations will: Review and enforce mishap prevention and damage control techniques, procedures and requirements Coordinate all directives and matters involving explosives and hazardous material with the 436 AW/SEW Prepare AF Form 2047, Explosives Facility License, for each explosive location that is outside the MSA, when required. Prepare license in accordance with AFMAN Submit licenses to the 436 AW/SEW office for approval Prepare unit guidance for handling, using, storing and transporting explosives and other hazardous materials in accordance with AFMAN and AFI Coordinate through Security Forces Resource Protection (436 SFS/SFAR), Fire Protection Branch (436 CES/CEF), Explosives Ordnance Disposal (436 CES/EOD), 436 AW/SEW, and the Chief, Wing Safety (436 AW/SE). For units and squadrons having a custodial ammunition account, coordinate with Munitions Accountable Systems Officer (MASO), 436 MXS/MXMW. In all instances, 436 AW/SEW is required to coordinate on all unit guidance Ensure operating personnel periodically review unit operating instructions on the handling, storing and transporting of explosives and other hazardous materials Ensure supervisors apply accident prevention principles and safety standards. Supervisors will train operators Notify Fire Protection Branch (436 CES/CEF) Emergency Communication Center (ECC) when fire or hazard symbol changes on your facility Notify 436 AW/SEW if explosive-related information conflicts and cannot be resolved in the unit Operators of vehicles loaded with explosives and explosive handling equipment will complete Explosive Safety Training (SAFE 538) every 15-months or incorporate the training into local operating instructions The Civil Engineering Squadron 436 CES/CECC will: Coordinate with 436 AW/SEW on all Installation Comprehensive D-8 Explosive Location Map updates.

6 6 DOVERAFBI JUNE Provide 436 AW/SEW and owning organizations with the inspection results of the electrical grounding systems of all explosive facilities in accordance with AFI , Grounding Systems Ensure 436 CES/CECC maintains a base map showing all explosive locations and fire hazards Coordinate all demolition, modification, and repair to facilities and proposed construction within quantity-distance clear zones with 436 AW/SEW prior to planning phase of proposed project Chief of Resource Plans (436 AW/XP) will: review and coordinate with 436 AW/SE on all host-tenant agreements Dover Command Post (436 AW/CP) will: Consult with 436AW/SEW on all questions concerning explosives or hazardous materials/items Dispatch applicable hazardous cargo information in AMC air movement messages When required, request 436 CES/EOD assistance IAW AMCI , and 436 AW/CP procedures When required, consult with 436 AW/SEW to request an operational waiver. Refer to AFMAN , paragraph 1.5 for further guidance Base Operations (436 OSS/OSFA) will: Coordinate with 436 AW/SEW on airfield matters involving explosives Ensure flight plan remarks contain complete information when transporting hazardous cargo Ensure Flight Publications Information En-route Supplement contains adequate explosives capability information about Dover AFB Aerial Port (436 APS/TRKS) will: Coordinate operating requirements with the Command Post and Base Operations when aircraft loading, unloading or in-transit activity involves explosives or hazardous items as required by DAFBI , Support of Aircraft Transporting Hazardous Cargo Disaster Preparedness (436 CES/CEX) will: coordinate with 436 AW/SEW on all disaster control activities that involve ammunition, explosives and other hazardous materials Life Support (436 OSS/OSTL) will: make sure aircraft carrying explosive items and pyrotechnics for survival purposes and actuating devices are serviceable and properly recorded on AFTO Form 337, Life Raft/Container Inspection Record, and AFTO Form 338, Survival Kit Record Medical Group (436 MDG/SG) will: Provide medical and bio-environmental engineering services and support when explosives and chemical agents are involved.

7 DOVERAFBI JUNE Security Forces Squadron (436 SFS/SF) will: Coordinate with 436 AW/SEW on routes vehicles will use in transporting explosives on base Provide 436 AW/SEW with the following information when encountering explosives or ammunition during customs inspection: type, amount, when discovered and person(s) involved with the investigation Coordinate with the 436 AW/SEW on AF Form 2047, Explosives Facilities License Review and recommend approval or disapproval of all SFS Local Instructions involving explosives Inform 436 AW/SEW when contraband explosives or hazardous materials are discovered on base The Weapons Safety Manager (WSM) 436 AW/SEW will: Implement and manage the 436 AW Weapons Safety Program The 512 AW/SEW will assist 436 AW/SEW in maintaining this program by implementing this instruction with reserve forces as applicable and in accordance with the Host base Support agreement Ensure all host and tenant organizations involved in explosive operations participate in the 436 AW/SEW program Annually inspect all ammunition and explosive operations/sites on Dover AFB. Maintain documentation of areas inspected, discrepancies noted and corrective action taken Review and recommend approval/disapproval of Explosives Local Instruction. An index and copy will be filed in 436 AW/SEW Investigate and report on all explosive mishaps and incidents occurring on base Provide technical assistance to officials responsible for transporting, storing, training and securing the various types of explosives, ammunition and other hazardous cargo on Dover AFB. Maintain and prepare required reports for the 436 AW/SE, Chief of Safety's coordination and review Assist units in the preparation of explosives waivers, exemptions and deviations Prepare site plans for new construction or modification of explosives facilities or facilities that may indirectly experience explosive hazard exposure Maintain site plans in a permanent file When an emergency condition dictates, coordinate and recommend approval and or disapproval of all waivers from quantity-distance criteria. Identify organizations requiring waivers, dates and hours of waiver required, quantity-distance class/division, net explosive weight involved, number of non-essential personnel involved, reason for waiver, exposed location/facility number(s) and specific location of potential explosive site, identifiable on the Base Master Plan, Tab D-8 map Provide initial weapons safety training for all unit Additional Duty Weapons Safety Representatives.

8 8 DOVERAFBI JUNE 2017 Chapter 3 EXPLOSIVE SAFETY 3.1. Transportation of explosives, both on and off-base, requires training and certification, proper equipment and vehicles, use of explosive transport routes, and proper notification procedures. ADWSRs should ensure unit personnel who transport explosives are properly qualified and are complying with the regulatory requirements to do so Units with a mission that requires off-base transportation of explosives must adhere to CFR 49 and local directives for items such as Bill of Lading and vehicle inspection. Contact the 436 LRS/TRT for further guidance on training and certification requirements Equipment and Vehicles Units will ensure proper utilization of equipment and specified vehicles for the transportation of explosives. Refer to AFMAN , Chapter 8 and 436/512 Airlift Wing ADWSR Guide, Section 2 for requirements of transporting explosives Explosive Routes On-base explosive routes will always be posted on the base EIM On base routes are established for delivery of explosives between the MSA, flightline aircraft parking spots, and between the Main (East)/West gates and the munitions storage area. Delivery routes are annotated on the Base Comprehensive Map, Tab D-8. Explosive routes are designated as primary and secondary routes; primary routes will be utilized unless conditions dictate use of the secondary route as the safer means but is limited to no higher than 1.3/1.4 HD. Use of secondary routes will be at the approval of the 436 MXS Munitions Flight Chief and 436 AW/SEW, and only when the primary explosive route is not accessible On-base explosive routes for licensed locations. Units must travel in authorized explosive transport vehicles via the most direct route between the Explosive Licensed Facility (EFL) and the MSA. Hazard Division 1.1 and 1.2 explosives will not be transported on Atlantic and Paris Street until coordinated through 436 AW/SEW Munitions Restricted Area. Non-mission essential personnel (or dependents) shall not be permitted along Perimeter Road (e.g. no jogging, walking, biking, hunting, etc.) within the explosive clear zones located between jogging turnaround points on the northwest and northeast side of the installation. Contact 436 AW/SEW for further guidance Off-base explosive routes Units with a requirement to perform explosive movements to off-base locations for operational or training requirements must adhere to both DoD and DoT regulations and the local state and jurisdictional law. ADWSRs must ensure

9 DOVERAFBI JUNE personnel who transport explosives to off base locations are trained and certified IAW 49 CFR, the Defense Transportation Regulation, AFI Flare and Flotation Equipment Deployment System (FEDS) Component Transportation. Refer to MXGI 21-03, Explosives, Handling, Transportation, and Temporary Procedures for FEDS operations and procedures Pre-Task Safety Briefings. Pre-task Safety Briefings will be given to all personnel who will be involved in explosive handling or transport operations as shown on the D8 map. EXCEPTION: Not required for Security Forces personnel during shift change (guard mount) involved in issue/turn-in of small arms loads for normal base security details and emergency response/security detail duties. Also not required for flight line maintenance personnel who utilize technical order listed pre-task safety briefings. All other units will utilize Attachment 2 for the pre-task safety briefings Exercises/Training Events utilizing Explosives. For the purpose of this section, the term exercises and training events utilizing explosives will be defined as events that require planning, involve the functioning of explosives, and are not considered individual mobility qualification or recurring competency training as outlined by AF instruction (i.e., combat arms training, ground burst simulator training, survival, evasion, resistance, and escape training, EOD proficiency, etc. ). Events included in this section encompass local or joint exercises, exercise/training events involving real estate not considered as a range or normally open to the base populous (SFS training events, etc. ), EOD or other authorized demonstrations, and all other training events not covered by the above definition Include 436 AW/SEW in all planning for exercise and training events that will utilize explosives. Refer to DAFBI , General Safety for Use of Blank Ammunition, Ground Burst Simulators and Pyrotechnics During Combat Training Exercises, and AFMAN , Explosives Safety Standards, Chapter 7 for further instructions.

10 10 DOVERAFBI JUNE 2017 Chapter 4 HAZARDS OF ELECTROMAGNETIC RADIATION TO ORDNANCE (HERO) 4.1. HERO Reports: AW/SEW will prepare and maintain the HERO package for Dover AFB The HERO package will be provided to base users, Explosives Ordnance Disposal (EOD), Munitions Supervision/Control, Fire Department, and any organization deemed necessary Electromagnetic Radiation (EMR) Survey. It is critical for all SEW offices to maintain a current listing of emitters on the installation to ensure a safe operating environment when explosives are involved AW/SEW will annually review installation HERO/EMR information per AFMAN and AFI procedures CS/SCOT will: Route work orders from the 436 CS/SCOT Plans and Programs section for installation of new emitter antenna systems to 436 AW/SEW for approval prior to antennas being installed or relocated AW Installation Spectrum Manager (ISM) will provide all new emitter parameter information to 436 AW/SEW for approval prior to emitter system being installed or relocated and will assist 436 AW/SEW during the EMR/HERO annual review.

11 DOVERAFBI JUNE Chapter 5 NUCLEAR CERTIFIED EQUIPMENT (NCE) PROGRAM 5.1. Units that operate, maintain, acquire, sustain, or modify NCE will appoint monitors from within the unit to manage the program. NCE monitors will: Be appointed by their Squadron CC and must have a minimum of one year (projected) remaining on station Unit NCE Monitors will complete the NCE Users Course CBT within 30 days of appointment and annually thereafter Inform 436 AW/SEW of any NCE related deficiencies within their unit Verify and validate unit NCE listing against the MNCL quarterly and send to 436AW.SEW.WeaponsSafety@us.af.mil Forward equipment/vehicle modification requests through the 436 AW/SEW DULL SWORD Reporting and Investigation. 436 AW/SEW, along with the units, manage the wing DULL SWORD Reporting Program IAW AFMAN , Weapons Safety Investigations and Reports and AFI , Nuclear Certification Program. Units will initiate and complete reports for any suspected DULL SWORDS using the DULL SWORD Worksheet (Attachment 3). completed worksheet to 436AW.SEW.WeaponsSafety@us.af.mil within two duty days of mishap. 13 DOVERAFBI JUNE Attachment 2 PRE-TASK SAFETY BRIEFING (MINIMAL INFORMATION REQUIRED) PRE-TASK SAFETY BRIEFING 1. Prior to conducting explosives handling or transport operations, operation crew chief must brief explosive and personnel limits, maximum drop distances, specific safety hazards, technical aspects of the munitions item(s) involved, and each crewmember's role during the operation. Each individual must also know what specific task(s) he/she will be performing in case of an emergency. Assign each individual a specific task such as notifying the fire department and other required agencies, the evacuation of all nonessential personnel, fire-fighting responsibilities and directing the emergency response forces to the mishap scene. (Recommend units copy and laminate this standard briefing, fill out with grease pencil/marker, and utilize to brief crewmembers prior to each operation involving explosives) Direct operating personnel to remove all rings, watches, and jewelry Visually inspect grounded surfaces and grounding points for serviceability as applicable Inspect fire extinguishers for serviceability Ensure equipment has been inspected and is serviceable Brief EMR hazards and personnel grounding measures (LMR, cell phones, etc., should not be utilized during explosive operations. 17 DOVERAFBI JUNE Use the DULL SWORD Worksheet to gather information for the actual report. Do not attempt to prepare an actual Dull Sword Report without the assistance of the Weapons Safety Manager. DULL SWORD reports will be reported IAW AFI and AFMAN WHO WILL INITIATE WORKSHEET: The individual or work center in physical possession of the equipment or present at the time of the deficiency will initiate the DULL SWORD report worksheet. Note: If the equipment is owned by an agency outside of your unit, the Weapons Safety Manager will contact the owning WHEN agency. TO TAKE ACTION: Upon discovery of a safety or security deficiency involving a nuclear weapon or nuclear certified equipment. Specific areas of concern are listed in AFMAN , Para 1.5.
