2 EAC Survey Election Assistance Commission (EAC) issues survey every two years Required by the Help America Vote Act (HAVA) Survey gathers information on federal elections
3 EAC Survey Survey requests information on: Voting registration Uniformed & Overseas Citizens Absentee Voting Act (UOCAVA) voting information Domestic civilian absentee voting Election administration Provisional ballots Election day activities
4 EAC Survey Section B (UOCAVA) has been decreased from 35 segments to 29 segments eliminating 25 data points to complete
5 EAC Survey Before completing any of the Survey, you must: Review the Supplemental Instruction Manual. Have a copy of your SOS Official Certification Reports on absentee and provisional ballots available. Create a VR report that indicates the number of VR forms received and where the VR forms originated (mail, BMV, designated agency, etc.).
6 EAC Survey Before completing any of the Survey, you must (cont.): Print of copy from the SoS Website of the Voter Turnout by County This report lists the total number of registered voters and total voters for the November 8, 2016 general election for each county. You must use these numbers to complete the Survey.

9 EAC Survey There are six sections to complete: A. Voter Registration B. UOCAVA C. Regular absentee voters D. Election Administration E. Provisional ballots F. Election Day activities
10 Section A -Voter Registration Number of registered voters for the 2016 general election. Number of forms received from close of registration for 2014 general election to close of registration for 2016 general election. Types of forms (e.g. new, change of name or address).
11 Section A -Voter Registration Section A cont. Number of new registrations, duplicates and invalid or rejected forms by source. Confirmation notices. Voters removed.
12 Section A -Voter Registration A1: Must match the number provided to our office prior to Election Day. If you are unsure of the number, it is posted under Election Results on our website. A2: Answer: A2b A3: Answer: Data not available A4 and A4b - Answers: Not applicable

Number of valid registrations d. Number of duplicates e. Number invalid or rejected f. Number of changes of name, address within your county g. Number of address changes to another county
14 Section A Voter Registration Section A6-A9 d. BMV includes in-person BMV and BMV update from online change of address. These are not to be reported under c. (forms received via the Internet). h. Other state agencies designated by the State libraries, county treasurers and high schools. If you track these separately, you may break them out in j., k. and l.
15 Section B UOCAVA Ballots Use the numbers provided on the Official Certification of Absentee Ballot report (Sections D through I) submitted to our office with one exception: Do not include in-person UOCAVA voters in the totals for the Survey. Transmitted only refers to ballots mailed, faxed or ed. Adjust your totals accordingly.

C4b: Number rejected C5a-v: Reasons for rejection The numbers must match the data you provided to the SOS office on the Official Certification of Absentee Ballots Report with the same exception as the UOCAVA section.

24 Section F Election Day Activities Other sections in F are requesting information on poll books and voting equipment When completing section(s) on voting equipment, be sure to complete the correct category for your county s equipment
25 EAC Survey After receiving the surveys, our office will review and perform the state error check on all data. We will contact boards they may need correct any data errors or provide an explanation for the discrepancy. Our office will submit the report to the EAC s vendor for review. They will review and notify our office of any discrepancies to be resolved.
