1 Poll Managers Oaths and Forms For General Elections Precinct County Date
2 Instructions For Poll Managers On Use Of This Booklet The following oaths and reporting forms are contained in this booklet: 1. Oaths of Clerks and Managers All managers and clerks must take and sign these oaths. 2. Managers Return of Ballots Must be completed in its entirety. 3. Polling Place Opening Checklist - This checklist is designed to be used by the poll clerk to help ensure each step of the polling place opening process is completed and to record the quantities of ballots, machines and PEBs at the polling place. 4. Polling Place Closing Checklist - This document is designed to be used by the poll clerk after polls close to record and verify the number of voters processed, ballots cast, machines closed, and flash cards removed. 5. Pay Vouchers This form must be completed and signed by all clerks and managers. 6. Volunteer Counters Oath If volunteer counters are used, each must take and sign this oath before assuming their duties. Return this booklet to your County Election Commission with all other election supplies.
3 Oaths of Clerks and Managers of Election State of South Carolina, County of I do solemnly swear (or affirm) that I am duly qualified, according to the Constitution of this State, to exercise the duties of the office to which I have been elected (or appointed), and that I will, to the best of my ability, discharge the duties thereof, and preserve, protect and defend the Constitution of this State and of the United States, so help me God. I do solemnly swear that I will conduct this election according to law and will allow no person to vote who is not entitled by law to vote in this election, and will not unlawfully assist any voter to prepare his ballot and will not advise any voter as to how he should vote at this election. Number of emergency/provisional ballots unused and returned to the County Election Commission The total of 3, 4 and 5 should equal 2. Failsafe Provisional Ballots 6. Number of failsafe provisional ballots furnished to precinct 7. Number of failsafe provisional ballots actually voted 8. Number of failsafe provisional ballots spoiled 9. Number of failsafe provisional ballots unused and returned to the County Election Commission The total of 7, 8 and 9 should equal 6.
5 Polling Place Opening Checklist Precinct Election Date Clerk to complete by 7 a.m. If there are any questions or issues, please contact the county election commission or a polling location technician immediately. Voting Machine Preparation Communication pack seal number matched seal envelope All machines seal numbers matched seal envelope All machines opened with green PEB Green PEB secure All voting booths plugged into power source Zero tape signed and posted at entrance; one copy in communications pack Machines connected to power source inside voting booths Machines placed at least five feet from managers table Machines spaced apart for privacy Privacy screens up Metal kickstands used to keep machines at an angle Voter Registration (VR) List/Poll List & EVRL (Laptops) Reviewed proper completion of lists with poll managers If necessary, the printed VR list was split alphabetically to aid traffic flow Laptops - reviewed user steps with poll managers Accessibility At least one entrance accessible for voters with disabilities Curbside voting signs placed at curbside voting area Sample ballot and posters placed at wheelchair eye level Ballots/Voting Machines Emergency/ Provisional ballots Failsafe ballots Machines Machines opened with green PEB Number of red PEB s Other Election office contacted to verify all machines open Quantity I confirm that all items on this list have been verified. 6 Polling Place Closing Checklist Precinct Election Date Clerk to complete after polls close. Include this checklist in the communication pack. If there are any questions or issues, please contact the county election commission or a polling location technician immediately. Audit Steps Registered voters (shown on VR lists or laptop) Signatures on poll lists (including curbside) Quantity Ballots cast on machines (from totals tape) Emergency ballots voted Provisional ballots voted Failsafe ballots voted Total paper ballots voted (Add emergency, provisional, and failsafe ballots) Total ballots cast (Add ballots cast and total paper ballots voted Machines closed with green PEB Machines closed with other PEB. WARNING: All machines should be closed with only the Green PEB. If machines were closed with another PEB, contact the CEC immediately. Flash cards removed from machines and placed in flash card pouch I confirm that all items on this list have been verified. Signature of Clerk
7 Polling Place Closing Checklist Instructions and locations for completing blanks on Polling Place Closing Checklist Registered voters Total number of registered voters in precinct. Located either on last page of voter registration lists or, if using a laptop, at top of screen. Signatures on poll lists (including curbside) Total number of signatures of voters. Combine number of signatures on poll lists at managers table and number of signatures of curbside voter poll list. Ballots cast on machines Look on Totals Tape, and add number listed as Public Count from each machine. Emergency ballots voted - Look at ballot number on stub of next Emergency/Provisional ballot. Subtract this number from starting stub number. Provisional ballots voted - Look at ballot number on stub of next Emergency/Provisional ballot. Subtract this number from starting stub number. Failsafe ballots voted - Look at ballot number on stub of next Failsafe ballot. Subtract this number from starting stub number. Total paper ballots voted - add emergency ballots voted, provisional ballots voted, and failsafe ballots voted. Total ballots cast add Ballots cast on machines and Total paper ballots voted. Machines closed with green PEB All machines must be closed with same green PEB used to open machine. Look on totals tape to verify PEB serial number used. Add number of machines closed using the green PEB. Flash cards removed from machines - After flash card door seal is broken, total number of flash cards removed from machines and placed in flash card pouch.
8 Managers pay shall not exceed two days. Clerks pay shall not exceed three days. 9 Volunteer Counter s Oath SC Code of Laws County Precinct Date Managers of election are authorized to use volunteer personnel in county ballots. Candidates and watchers for candidates for offices voted on in this election may not serve as volunteer counters. Volunteer counters must take and sign the following oath before assuming their duties: I do solemnly swear or affirm that I am not a candidate or watcher in this election, am a qualified election of this county, that I will count the ballots entrusted in my care in a fair and impartial manner, and make to the best of my ability a correct tabulation of the results.
