2 Criteria David T. Cocks Walden Savings Bank Scholarship Fund Scholarships will be awarded based on character, potential for success, academic achievement, participation in school activities and community service. One $2,500 scholarship will be awarded in one of the following 13 school districts and private high schools as follows: 1. Chapel Field Christian School 2. Cornwall Central School District 3. Florida Union Free School District 4. Goshen Central School District 5. John S. Burke Catholic High School 6. Marlboro Central School District 7. Newburgh Enlarged City School District 8. New Paltz Central School District 9. Pine Bush Central School District 10. Valley Central School District 11. Wallkill Central School District 12. Warwick Valley Central School District 13. Washingtonville Central School District Primary consideration will be given to graduating students who have completed an Eagle Scout Project. Secondary consideration will be given to graduating students of the above schools who are who are recipients of the OCIAA (Orange County Interscholastic Athletic Association) Scholar Athlete Award, MHAL (Mid Hudson Athletic League) Scholar Athlete Award, or the equivalent named scholar athlete award in the respective school district. All students who apply must have been accepted and ultimately matriculate as full time students at any accredited college or university located in the United States. Scholarships awarded by the Fund may be used to aid in the payment of room, board, tuition, books and other fees. Selection Process David T. Cocks Walden Savings Bank Scholarship Fund The board of directors of Walden Savings Bank will recommend the recipient each year to The Community Foundation of Orange County, Inc. d/b/a Community Foundation of Orange and Sullivan.

3 AWARD PROCEDURES FORM Advisory Committee Recommendation Form David T. Cocks Walden Savings Bank Scholarship Fund Responses will provide information to indicate that the selection procedures are objective and nondiscriminatory. 1. Criteria 2. The applicant pool are graduating students of who meet the criteria. Burke Catholic High School Marlboro Central School District Newburgh Enlarged City School District New Paltz Central School District Pine Bush Central School District Valley Central School District Wallkill Central School District Warwick Valley Central School District Washingtonville Central School District 3. The Guidance Departments of each respective school district or private school are responsible for distributing the application form to all graduating seniors. 4. The Guidance Department of each respective school district or private school requires that students submit completed hard copy applications to them by April 15. Immediately after the April 15 student submission deadline, the Guidance Department reviews the applications to ensure the students meet the criteria, and forwards the completed hard copy of each application to the Community Foundation of Orange and Sullivan at 30 Scott s Corners Drive, Suite 203, Montgomery, NY Walden Savings Bank scholarship committee reviews applications and recommends scholarship recipient annually to the Community Foundation of Orange and Sullivan by May 15 annually. Walden Savings Bank may require an interview. 6. Walden Savings Bank is responsible for submitting the name, contact information, and scholarship application of the recommended scholarship recipient to the Community Foundation for scholarship payment processing by May 15 annually. A congratulatory letter will be mailed to the Guidance Department to be given to the recipient at the awards ceremony (if applicable). 7. The Community Foundation will notify the respective school district of the scholarship recipient.

Cocks: On June 19, 2011 David Cocks, President of Walden Savings Bank, passed away suddenly. He was a man of great character, a friend to many, a devoted husband, father, and son, and truly made a difference in the many lives he touched whether as President of Walden Savings Bank, or in his many volunteer roles. He exemplified the definition of character and left a legacy of service and good work. After Mr. Cocks passed, the board of Walden Savings Bank made a decision to establish a charitable fund in his name. Deserving students will be the recipients of the David T. Cocks Walden Savings Bank Scholarship in honor of a man who was loved by his family and friends, respected in his community and left a legacy of distinguished citizenship.
