8 Recently some facilities may have received a letter this letter. We will not be discussing this letter today, due to not have a DHS Representative on the call, instead we will use the next slides to define Specialized Services, Rehabilitative Services, Community Placement Supports and what constitutes a treatment record. 8

10 Federal law requires that individuals evaluated through the PASRR process receive any services identified in the PASRR summary of findings report. If a NF accepts an individual the NF is responsible to deliver all the identified specialized services, as well as rehabilitative services, and when appropriate, community placement supports. 10

11 This is the new format of the Summary Of Findings. This section of the Summary of Findings begins to list the PASRR identified Services. We want to remind you that you will have reviewed ALL PASRR identified services prior to accepting the individual for placement, in order to make certain that you could meet their needs and deliver 100% of the PASRR identified services prior to admission. This section becomes the basis for development of a PASRR compliant Care Plan and may continue onto more than one page. The specialized (disability specific) services are always identified first and then the rehabilitative services, and the community placement supports are included in a separate section, following the rehab services. 11

12 All mental health services less intensive than specialized services are considered rehabilitative services and must be arranged for by the nursing facility in which the person lives. PASRR evaluation reports can describe a wide range of everyday services to help nursing facility staffs working with the individual. A detailed evaluation will help nursing facility staffs assure the success of an individual s NF stay. It is important that these services detail the strategies that will help the nursing facility maximize the individual s quality of life. Nursing facility staffs find Level II evaluation summaries most useful when clinical information and identified services are written in plain, layman s language. Nursing facilities are required to deliver PASRR identified services to the individual, if the individual is to remain in the NF. Nursing facilities must incorporate PASRR treatment and services within their care plans. Nursing facilities can be censured by auditors if they do not keep PASRR information in the individual s active chart and address PASRR recommendations in the care plan. 12
