6 REDESIGN OVERVIEW HOSPITALS Will complete Ascend s Level I screens online, Ascend determines the outcome of the Level I screen Will complete the interrai LOC short form online for all Medicaid beneficiaries and all Level II referrals, Ascend determines the outcome of the LOC Who needs to attend trainings on how to use Ascend s system? 8 REDESIGN OVERVIEW NURSING FACILITIES Will complete Ascend s Level I screens online only for persons with emergency, respite and out-of-state admissions Will use Ascend s Level I system to trigger PASRR status changes or resident reviews as needed Will complete full online interrai LOC screens when persons convert to Medicaid or a Level II is needed for a resident Who needs to attend trainings on how to use Ascend s system? 15 WHAT YOU CAN DO NOW TO PREPARE Review Level I screen items and determine how your facility can best collect this information. Learn the version of the interrai assessment your facility will use 1. Ascend will webinar attendees a copy of Ascend s Level I items. 2. The Division of Aging will train on the short and long forms of the interrai 3. The Division of Aging will distribute sample interrai forms ASCEND MANAGEMENT INNOVATIONS LLC. ALL RIGHTS RESERVED.
16 WHAT YOU CAN DO LATER TO PREPARE 1. Your facility s staff logons will be managed by Facility Access Administrators (1 to 3) at your facility. 2. Your Facility Access Administrator will apply for access through AssessmentPro by downloading a form to complete and upload. 3. Facility Access Administrators should be selected by facility management and be 1) tech savvy, 2) have ability to act as system resource for your facility, and 3) know the staff that should be authorized to use the system. 4. Instructions and timelines will be distributed via ed Ascend Updates. 5. Users will request access through AssessmentPro. Facility Access Administrators will grant access (or not) and assign each user s role (clinical or not) 2015 ASCEND MANAGEMENT INNOVATIONS LLC. ALL RIGHTS RESERVED. 16
17 WHAT YOU CAN DO LATER TO PREPARE Hospitals and Nursing Facilities: Review the new LOC short form interrai screen and determine how your facility can best collect this information. Ensure your staff members register for a Division of Aging training on how and when to administer the LOC short form. Nursing Facilities and AAAs: Ensure your staff sign up for a Division of Aging training on how and when to administer the interrai LOC assessment ASCEND MANAGEMENT INNOVATIONS LLC. 18 SMALL CHANGES All persons who are triggered for a PASRR Level II evaluation must have the interrai LOC completed before admission to the NF. All persons who are Medicaid recipients must have the interrai LOC completed before admission to the NF. All will be reviewed by Ascend clinicians, so hospitals will want to start the screening process early Once you have uploaded all required information, Ascend will issue a decision within a few business hours 2015 ASCEND MANAGEMENT INNOVATIONS LLC.
