2 Decentralized System Current System The Catalan healthcare model is a multi-provider one integrated in a unique public network. It enhances the autonomous management of each provider. Providers are free to select their information systems; however 85% of the primary care centres have the same system (ecap) Interoperability among systems must be guaranteed. 2
3 The Health Plan of Catalonia The Health Plan provides ICTs as a model of change to achieve the new model of care. The Health Plan proposes a new model of care focused on the citizen, who is involved and co-responsible of its own health care 3
4 ICT Challenges Information sharing among citizens, professionals and organizations is the main challenge in the consolidation of ICT in Health (ehealth). 1. Advanced and Mature ICT Mismatch of functionalities at different entities. Diversity of ICT solutions 2. Transformation of the public HC System organizational model citizen Fostering Information Integration 3. More active citizen role in the HealthCare system professionals organizations Common Services 4
5 ICT tools to develop the Health Plan There are two key elements to develop ICT according to the objectives of the Health plan: Electronic Health Record of Catalonia (EHR) As an information and services network Allows organized access to relevant information of different centers health records and to some central databases of the health system. The EHR is not the sum of the electronic records of the healthcare centers; it doesn t incorporate all the information from medical records. Personal Health Channel Deployment of a multichannel network to communicate and interact with the citizen The citizen is the holder of the data contained in its medical record He will have access to its health information available in its electronic Health Record 5
