5 Health and Safety State fines UCLA in fatal lab fire May 05, 2009 Kim Christensen State regulators on Monday fined UCLA more than $31,000 for three "serious" violations of workplace safety laws in the fatal burning of a staff research assistant in a Dec. 29 chemistry lab fire. The findings by the California Division of Occupational Safety and Health concluded that Sheharbano"Sheri" Sangji, 23, had not been properly trained and was not wearing protective clothing when an experiment exploded, spreading second- and third-degree burns over 43% of her body. She died 18 days later. Substance she was working with --tertiary-butyllithium in pentane (pyrophoric material in a flammable liquid). 6/20/2014 Criminal case against the PI (Dr. Patrick Harran 4 felony violations) resolved-5yrs probation. Felony charges filed against the University dropped in
6 Health and Safety A grad student was working alone in a laboratory on a Friday morning a couple weeks ago conducting an experiment whose procedure involved HF, piranha and nitric acid. The grad student accidently put nitric acid waste in the wrong waste container which was dedicated for solely HF waste. The student immediately recognized the error and left the lab to seek assistance. In that period of time of seeking help the container exploded expelling the mixture within the hood as well as in the immediate area outside the fume. The lab was off-line for 3-days. THANKFULLY NO ONE WAS INJURED! SOME TAKEAWAYS: Never work alone in the lab! ALWAYS Be aware of what you are doing! Keep hood sashes, shields, etc. positioned to protect you. Carefully design experimental procedures and equipment such that minimal manipulation is required. This will significantly reduce the risk of potential exposures and hazards. 6

8 Examples of Institution Requirements -- Prior Approval Needed Environmental Health and Safety must be notified when certain operations or projects occur. The notification is required in order for EHS to help researchers and students safely perform the task or process. Renovations to laboratory and research spaces Use of Class IIIB or IV lasers Explosive laboratory reactions or use of high explosives Use of highly toxic chemicals, radioactive materials, carcinogenic materials, reproductive toxins or materials of unknown toxicity Purchasing of refrigerators for chemical storage Purchasing, moving or installing a chemical fume hood, laboratory exhaust ventilation or laminar flow equipment Minors (under 18 years of age) working with hazardous materials or performing hazardous operations Experiment or process that impacts building or laboratory design, i.e. a large piece of equipment or apparatus that blocks sprinkler heads Purchase or use of a respirator. Purchase of 3D Printers
