4 Harms to Go Harms to Go Update! Data Auction Kentucky s Commitment: 1. Falls: /month (6.84% worse) 2. MRSA: 36 9/month (11% improvement) 3. Readmissions: 6,419 1,605/month (2.45% improvement) 4. SSI Total Hip/Knee (combined): 38 10/month (Abd. Hyst and Colon are at 61% and 49% improvement!) Can you commit to harm reduction in certain areas? 4

5 SPRINT Opportunity HRET HIIN Sprint Areas in which the HRET HIIN hospitals are closest to reducing all-cause harm by 20%. The Infographic in the Media Library highlights interventions and lessons learned from across the HRET HIIN network of hospitals to improve ADE-Hypoglycemia, CDI, CAUTI, CLABSI, MDRO-MRSA, and Sepsis (Post-Op) Hospitals will be invited to participate in an 8-week collaborative but if you are interested and don t receive an invitation, contact us anyway, we may be able to include you! 11 FALLS Fast and furious!! Nutrition- add this to your falls risk screening as well as risk for injury! A short term nutritional intervention of oral supplements, Ca and Vit D and dietetic counseling in malnourished older adults decreased the number of falls. Quality Conference Table Time Results! 11

Sorted by County offers location, address, phone, and hours of operation Local Police Depts., Sheriff Offices, Coroner s Offices, and State Police Posts Add to your discharge counseling to encourage disposing of these unused, 25 unwanted, and outdated medications!
