1 Living Well Health Center operations transition As of October 1, 2017, the management of the Living Well Health Center will be transitioned from Premise Health to Crossover Health. We selected Crossover Health to provide an excellent patient experience, extended hours of operation, and a comprehensive and coordinated model of care. We are excited to make Crossover s next-generation health care available to you on our campus. Refer to the following guidance if you have questions about the transition of the clinic s operations.

2 The Living Well Health Center is my primary health care center. What if I need medical care or attention while it s closed? The Living Well Health Center will be closed from September 30 through October 8, If you require medical attention during that time, we recommend that you visit the Getting Medical Care page on the Benefits site, refer to the list of providers in your insurance network, or visit your closest urgent care facility. And in the event of an emergency, call 911 for help. Teladoc services* are also available to you the week of the transition so you can speak with a doctor by phone, video, or mobile app 24 hours a day, 7 days a week. It s an affordable alternative for when you have non-emergency health concerns but want an answer now. You can register for Teladoc and request a consult online or by phone: (855) Standard benefit cost shares will apply. However, the consult will never cost you more than $40. *Teladoc services are only available to Premera health plan members. Kaiser Foundation Health Plan of Washington members should visit the Care Options page to find care options. Will all health care-related services on campus be closed, including Living Well Eye Care and the pharmacy? The Living Well Health Center will be closed from September 30 through October 8, The following services will remain open during the transition: Living Well Health Center Pharmacy Living Well Eye Care at The Commons Microsoft CARES Employee Assistance Program (EAP) counseling services Ergonomic consultations Why is there a transition? What does Crossover Health offer that Premise Health does not? To ensure that we re offering the best services and value to our employees, Microsoft assessed the market for innovative health partners. We selected Crossover Health to provide an excellent patient experience, extended hours of operation, and a comprehensive and coordinated model of care. We are excited to make Crossover s next-generation health care available to you on our campus. 2

3 What about my provider? Will I still be able to visit my primary care physician and other care providers at the Living Well Health Center? All of the clinic s current medical providers are transitioning with Crossover Health that includes physicians, health coaches, dietitians, physical therapists, and other specialists. You can still make appointments with your current preferred clinician or medical provider. Note that the online scheduling portal will be temporarily unavailable during the transition, but will come back online the week of October 2, All appointments made prior to the transition will transfer over and be visible in the Crossover Health Patient Portal the week of October 2, Will I be able to schedule an appointment for after the clinic reopens? Yes. However, the online scheduling portal will be temporarily unavailable during the transition but will come back online the week of October 2, My family relies on the Living Well Health Center too. Will they still be able to visit after the transition? Yes. Eligibility for the clinic will not change. Employees, interns, and dependents (age 13 or older) who are enrolled in a Microsoft health plan will have the same access to the Living Well Health Center. Will Premise Health transfer my medical history and information to Crossover Health? Will I still have access to my medical records under Crossover Health? Yes, your medical history and information will transfer to Crossover Health, and you will still have access to your medical records. After the transition, if you need a copy of your medical records, you can call the clinic at (425) or Crossover via the portal (found here: 3

4 Billings from Premise Health can take several weeks to process. What if Premise Health sends a bill to me or my insurance after the transition? You would pay any outstanding bills from Premise Health the same way as before the transition. Will the transition to Crossover Health affect the cost of my services? For most services, the cost of care will stay the same. The length of chiropractic services will increase from 15 to 30 minutes, with a new fee reflecting the increased visit length. The radiology fee will include both image and read fees. Refer to the chart below to assess the new service fees. 6 Will on-site counseling offered through Microsoft CARES EAP still be available after the transition? Yes on-site counseling will be available during and after the transition. If you have counseling needs, you can find support by visiting the Microsoft CARES EAP site or by calling (800) for immediate assistance. Will ergonomic appointments still be available? Yes they will be available during and after the transition. 6
