1 Plan Document Navigation Goals Goals Co-Curricular Learning Objectives Key Performance Indicators Signature s Priorities (including assessment and outcomes) Other s Goal 1: Provide high quality primary health care service that is accessible, personalized, and fiscally responsible. Goal 2: Emphasize health education, prevention and early intervention to maintain a safe, inclusive and healthy learning environment. Goal 3: Promote healthy behaviors and lifestyle choices so students can be personally and academically successful. Goal 4: Provide an environment that teaches students to become an informed participant in their healthcare. Goal 5: Provide culturally competent services with an awareness of global issues that have an impact on our students, campus and community. Co-Curricular Learning Objectives 1. Students will access campus, community and virtual resources related to improving and maintaining health. 2. Students will maintain and improve their health by making choices which support continued well-being. 3. Students will understand their diagnosis and options for care and will be able to apply that information to make well-informed decisions about their future care. 4. Students will engage as active participants in their health care by seeking out treatment options, participating in dialogue with their providers, and understanding their role as the primary decision-maker. 5. 4 Priorities Student Health Services Plan Evaluation of direct patient care services provided by Southwest Health to ensure continuation of quality care. Begin the first full year of a five-year contract with Southwest Health whereby Southwest Health will staff medical doctors at Student Health Services. Conduct several assessments over the course of the year in order to ensure that high quality primary care is continued; health education, prevention and early intervention strategies are offered; and, that students are informed participants so they can be personally and academically successful. Make appropriate plans for follow-up based on the results of the quality assurance reviews and the annual user survey. s:, Spring 2015 o Outcome: The percentage of students reporting that they would return to SHS again if they needed health care will stay within the acceptable range of the results. To stay within an acceptable range it can drop by no more than 5%. o Outcome: The percentage of students reporting that they would recommend SHS to a friend will stay within the acceptable range of the results. o Outcome: The percentage of students reporting that they made improvements on the issues for which they sought SHS will stay within the acceptable range of the results. To stay within an acceptable range it can drop by no more than 5%. o Outcome: The percentage of students reporting that they were struggling with class attendance will stay within the acceptable range of the results. o Outcome: The percentage of students reporting that SHS helped them stay enrolled in school will stay within the acceptable range of the results. o Outcome: The percentage of students reporting that they understood their DX and TX plan will stay within the acceptable range of the results. To stay within an acceptable range it can drop by no more than 5%. o Outcome: The percentage of students reporting that they knew how to prevent reoccurrence of illness/injury will stay within the acceptable range of the results. To stay within an acceptable range it can drop by no more than 5%. Quality Assurance Plan/Reviews Staff: Lab Proficiency o Outcome: All staff performing lab tested will be expected to pass lab proficiency testing on a pass/fail assessment. Quality Assurance Plan/Reviews SWH Provider: Lab Results Follow-up o Outcome: All lab will be acknowledge in the EMR, recommendation for abnormal follow-up recorded along with student notification of results Quality Assurance Plan/Reviews Mono Testing Criteria, Diagnosis, Education i. Outcome: All students diagnosed with mono will have documentation of education to include: transmission, prevention, treatment, and activity restrictions. 5 Plan Utilize results of the College Health (NCHA) that was administered in spring Identify key findings from the NCHA in conjunction with the Director of University Counseling Services, the Prevention and Education Coordinator and the Coordinator of Co-Curricular and Program Review. Identify one to two projects for sharing the data with interested constituencies and for acting on the data. Act on those projects as needed. This will be an ongoing process for the next three years until the assessment is administered again in spring s: To be determined once we get further into the analysis and planning process Other s Quality Assurance Plan/Reviews - Scooter usage Outcome: Learn more about how students are benefiting from use of the scooters.
