1 Self- Assessment Survey Survey Terms: In this survey the following terms are used: Collaborating Physician a physician who works closely with a nurse practitioner in a collaborative practice. Collaborative Practice a relationship among a nurse practitioner, a physician or group of physicians, an employing organization and other health professionals and staff. Patient/Resident the individual who is the recipient of nurse practitioner services. Practice of a Nurse Practitioner the application of advanced nursing knowledge, skills and judgment which includes: Making a diagnosis, identifying a disease, disorder or condition; Communicating the diagnosis to the patient and health care professionals, as appropriate; Performing procedures; Initiating consultations and referrals; Ordering and interpreting screening and diagnostic tests; and Prescribing or reordering drugs, blood, blood products, and related paraphernalia. Scope of Practice the roles, functions, and accountabilities that nurse practitioners are educated, competent, and authorized to perform. 1. Provide patients with relevant information and counselling on pharmaceutical therapy e. Provide patients with cost effective alternative choices for pharmaceutical therapy, as applicable f. Consult/collaborate with pharmacists as appropriate g. Write prescriptions accurately and completely h. Monitor patient response to pharmaceutical therapy i. Consult with a physician about prescribing j. Report adverse events associated with pharmaceuticals and other therapies to patients, relevant health care providers, and national or provincial reporting agencies k.
