1 At QPA, we value general practice. We are GPs. We understand how hard had it is to be a GP. One minute you can be seeing a child with a sore throat, explaining the need or otherwise for antibiotics, the next managing a child with severe behavioural problems and the complex family needs. One minute you can be seeing a patient with an ankle injury, discussing whether they need an Xray or not, the next managing a patient with complex chronic disease. As GPs, we understand that general practice isn t just about our patient s problems but about their stories, where and how they live. We make a difference to people s lives in small or large steps every day. At QPA, we don t just accredit general practices: we understand them. Oxford Dictionary: A situation in which a difficult choice has to be made between two courses of action, either of which entails transgressing a moral principle. 6

3 Examples of Ethical Dilemmas Patient-Practitioner Relationships Professional Differences Patients giving gifts to the Practitioner Emotionally charged clinical situations - such as unwanted pregnancy, terminal illness, euthanasia Reporting requirements such as state driver licensing authority that a patient is unfit to drive Patient request for medical certificate when practitioner does not believe patient s condition warrants one 7 Standards Requirements Mandatory to document situations that present ethical dilemmas and the actions taken Practitioners may discuss with colleagues Practitioners may discuss with medical defence organisation Generic ethical dilemma discussions recorded in medico-legal file Specific individual dilemmas to be recorded in patient health record 8 Criterion QI3.2 - Open Disclosure Indicator A - Our practice follows an open disclosure process that is based on the Australian Open Disclosure Framework. 9

4 Open Disclosure The Australian Open Disclosure Framework (the Framework) is designed to enable health service organisations and clinicians to communicate openly with patients when health care does not go to plan. Source: Australian Open Disclosure Framework 10 Open Disclosure in General Practice Indicator lists that you could: Maintain an open disclosure process Develop and implement policies and guidelines that align with Australian Open Disclosure Framework Keep a record of any discussions Implement quality improvement initiatives Record any incidents in the patient s record Educate your practitioners Discuss open disclosure at practice team meetings Incorporate open disclosure within onboarding process 11 Question time To ask a question please use the Question function in your GoToWebinar control panel.
