4 From Our Montana Region Presidents and CEOs Dear Community Members, It was because of a call to respond to critical health needs that a small group of courageous young women traveled to the Montana territory to open a Sisters of Charity of Leavenworth hospital in The pioneering spirit they brought lives on in the extraordinary people who continue to serve at Holy Rosary Healthcare in Miles City, St. James Healthcare in Butte and St. Vincent Healthcare in Billings. While the details of the challenges we face today are different, the mission of responding to identified community health needs has remained the same. SCL Health s Montana Region remains committed to meeting the needs of our communities. Together we are working to increase access to health services, educate the future healthcare workforce, enhance the health of the community through community partnerships, and address the underlying conditions which profoundly impact health outcomes in our state. Throughout this report we highlight stories illustrating the impact of our community benefit in the State of Montana. 7 Social Determinants of Health Our individual health choices such as not smoking, being physically active and eating healthy foods are important for good health, but we also know that our health is influenced by the conditions of the neighborhoods and communities where we live, learn, work, play, and pray. These factors such as Economic Stability, Education, Social and Community Context, Health Care, and Neighborhood make up the social determinants of health. Increasingly social determinants of health are being used as another tool to guide strategies for community health improvement interventions and investments. 8 CHNA Overview Every three years, Holy Rosary Healthcare, St. James Healthcare and St. Vincent Healthcare conduct formal community health needs assessments (CHNA) to understand the priority health needs of our communities. This process includes review of primary and secondary data to determine what health needs exist in each of our communities. With community input, we prioritize the greatest health needs of each community. As we focus on the identified areas of greatest need, we recognize clinical care, health behaviors, the physical environment and socioeconomic factors all influence health. A community health improvement plan (CHIP) is developed with strategies to provide the greatest impact on health and well-being for the communities we serve, especially those who are most vulnerable. 9 Equipping the Workforce St. Vincent Healthcare Trains, Attracts New Physicians Through the Montana Family Practice Residency Program Economic stability and education are important social determinants of health. SCL Health s Montana hospitals are providing training and educational opportunities to ensure a well-equipped workforce and economic opportunities for Montanans. For over 20 years St. Vincent Healthcare has participated in the Montana Family Practice Residency Program, welcoming eight residents from various medical schools across the nation. Each commits three years to the Billings community as part of this residency and is able to practice at different care sites as they gain experience in various fields. In this immersive and interactive learning environment, residents have the opportunity practice in any area relevant to their education, from pediatrics to trauma and virtually any focus in between. Currently Montana has the fewest number of residents per capita in the United States; as such, this program adds tremendous benefits to the community of Billings as well as St. Vincent Healthcare. It gives the hospital the potential to retain residents who want to establish their practices in the area and find physicians interested in practicing in rural areas across the state of Montana. The retention rate of these highly qualified students within our state is approximately 62 percent. This program brings a lot of enthusiasm to our staff, and creates a positive atmosphere. - Dr. Michael Bush According to Dr. Michael Bush, Chief Medical Officer of St. Vincent Healthcare, the Montana Residency Program offers other benefits. Those benefits include keeping staff current on all training curriculum and methods, as well as bridging gaps between St. Vincent Healthcare and Billings Clinic and Riverstone Health, all three of which are participating in the program. This program brings a lot of enthusiasm to our staff and creates a positive atmosphere, says Dr. Bush It ensures the focus remains on providing the highest quality care for the Billings community. Dr. Bush goes on to note that while there are no major changes occurring within the residency program, if an opportunity to increase funding presents itself, eventually the program may extend to accept more than eight residents per session Montana Community Benefit Report 8

10 Training the Next Generation of Radiology Technicians at St. James Healthcare St. James Healthcare is helping aspiring radiology professionals by offering a radiology technology residency at their facilities. The two-year program provides students the opportunity to get hands-on experience as they complete the curriculum. Many students are from the Butte area originally and have made the decision to return, says Jerri Doyle, Imaging Manager at St. James Healthcare. The hospital retains at least one student after each session, and has been able to retain as many as five. It s a good way to ensure we are never without radiology technicians. The curriculum is broken up into quarterly segments to allow study topics to be covered comprehensively. The 10 to 12 students who participate work various hours, ranging from part-time (three days a week) to full-time. According to Doyle, the radiology technology residency program is not only beneficial to students, but also to St. James Healthcare staff as they remain on the leading edge of radiology education. Healthcare support occupations along with healthcare practitioners and technical occupations are projected to be among the fastest-growing occupational groups through 2024, contributing about 1 in 4 new jobs. 11 Photographer: Bob Zellar Used with permission from the Billings Gazette From left to right: Rich Brant, Terry Hollister, Kathy Lombardozzi Clinical Pastoral Education Enhances Spiritual Care to Patients, Families & Communities As a faith-based institution, St. Vincent Healthcare is dedicated to providing pastoral care to patients, families and associates. As part of this effort, the organization has offered Clinical Pastoral Education (CPE) since 2014, one of the only hospitals throughout the region to do so. CPE is the primary method of training hospital and hospice chaplains and spiritual care providers in the United States. Over the course of one CPE session, a student is required to complete 400 hours of service, which is divided into 100 hours of education and 300 clinical hours. There are two different CPE sessions throughout the course of a year, and each session accepts a maximum of 15 students. Before St. Vincent adopted the CPE program there was no in-state program, shares Chaplain Terry Hollister, who coordinates the program. Students come from all over the region to complete their course. When they successfully complete it, they are able to provide better spiritual care overall. The curriculum in the program helps students provide the best spiritual care possible teaching them several aspects of providing care. They learn to be conscious of emotions, identify feelings within themselves and those they are serving. Through the program, participants realize it is a gift to be with people during vulnerable times, says Chaplain Hollister. 12 REACH Camps Encourage Youth to Explore Careers in Healthcare Throughout the year Holy Rosary Healthcare actively looks for ways to connect with and impact youth in Miles City and the surrounding area. One of the organization s most successful efforts has been through the hosting of one-day programs targeted specifically at high school students. REACH stands for Research and Explore Awesome Careers in Healthcare and is a program focused on helping students find healthcare careers that would interest them. As they visit various program stations throughout the day, students complete hands-on activities that give them an authentic look at a particular career. For example, teens may learn to take patient vitals as they learn the role of a clinic nurse, or learn to determine their own blood type as they learn more about phlebotomy. Students must apply for acceptance into the program, which accommodates approximately 15 to 20 students. REACH camps are held twice each year at Holy Rosary Healthcare. 13 Getting to a Healthy Weight Gladys Turnquist Diabetes and Prevention Programs Promote Healthy Lifestyles By addressing the social and community context and built environment, SCL Health s Montana hospitals are making it easier for our neighbors to live a healthy lifestyle. St. Vincent Healthcare, Holy Rosary Healthcare and St. James Healthcare are all working to improve community health by preventing diabetes and heart disease one person at a time. All of these organizations offer Diabetes and Heart Disease Prevention Program (DPP). This program is dedicated to changing the behavior of those at risk of developing type 2 diabetes or heart disease but who do not yet have it, says Bev McHugh, RD, LN with St. Vincent Healthcare. People who are eligible to participate are overweight and have one or more health factors that indicate they are at risk. Those factors, she says, include a diagnosis of prediabetes, a history of gestational diabetes or having a baby that weighed over nine pounds. Those with high blood pressure high cholesterol, or high triglycerides are also eligible, even if they are currently taking medication to control those levels. We want to offer resources to help promote healthy lifestyles in Miles City and other small communities, says Liane Vadhiem, RD, LN, CDE, the dietician and diabetes educator at Holy Rosary Healthcare. The DPP program helps us do that, working with readily available options Montana Community Benefit Report 12

14 The 16-week program consists of nutrition education and exercise classes with 6 subsequent monthly follow-up sessions. It is open to both men and women, and is focused on promoting healthy lifestyle adjustments. Each participant is encouraged to add 150 minutes of increased activity to their week. In addition, they are expected to keep a food journal as they focus on making good habits part of their lifestyle. Members learn the basics of healthy eating and receive tips such as how to manage stress without returning to poor eating habits. For participants lacking easy access to workout facilities because they live in rural areas or have work schedules which preclude it, at-home workouts are emphasized as part of the program as needed. St. Vincent Healthcare and St. James Healthcare have partnered with the local YMCA so that Billings and Butte-area participants may use those facilities as often as they wish throughout their time in the program. Bev says approximately 60 percent of participants who sign up for the program complete it, and with excellent results. On average, members lose approximately 12 pounds during the initial session, along with between 5 to 7 percent of their total body fat. Upon completion, they have the knowledge they need to continue to make the right choices for long-term wellness. DPP sessions begin twice annually, once at the beginning of the year and again mid-year. Thanks to the success seen by past participants, each session is usually full. Holy Rosary Healthcare has been recognized for its Diabetes PreventionProgram. Ann Albright of the Centers for Disease Control noted, The sustained success of this lifestyle change program makes an invaluable contribution to the prevention of type 2 diabetes, both in the community and nationally Montana Community Benefit Report

15 Partnering for a Healthy Community with Healthy By Design St. Vincent Healthcare is a member of the Healthy By Design Coalition, a community coalition focusing on the community health needs assessment priorities of healthy weight status, mental health and access to care. The coalition has worked hard to make small changes in our community relating to these areas, says April Keippel, Mission and Community Benefit Programs Manager at St. Vincent Healthcare. That includes activities to help make the healthy choice the easy choice. One of those activities is a gardeners market which runs from June through October. Unlike a traditional farmers market, which often charge fees to sellers, this market does not charge vendors. That means community members have access to fresh produce at a lower price, and they can purchase them using government-issued funds. Another way Healthy By Design is encouraging the community to adopt healthy lifestyles is our message, says Keippel. That stands for 5 fruits or veggies, 2 hours or less of screen time, 1 hour of physical activity and 0 sugary drinks in a day. In addition to promoting greater activity levels and improved nutrition, the Healthy By Design Coalition is focusing on making the Billings community trauma-informed through training and providing resources to help individuals cope with stress in a positive manner Montana Community Benefit Report 14

16 Partnering with Schools Keeping Kids on the Move As part of an ongoing effort to promote health and wellness of Billings-area students, St. Vincent Healthcare and the St. Vincent Healthcare Foundation have partnered with Kohl s in the Kids on the Move program. Together, they work with elementary school children to add additional physical activity into their lives, emphasize healthy choices and take control of their own health. The program includes sponsoring running clubs at the Title I schools in the Billings area, engaging with kids and families as part of the Heart & Sole Run each summer and hosting the Yellowstone Elementary School (YES) Cross-Country Meet each fall. Some 1,000 fourth, fifth and sixth graders compete at the meet each year, which is held at Pioneer Park. One of most exciting aspects of the program is also the opportunity to help elementary age children stay active by ensuring they have quality footwear. While many may take having a pair of running shoes for granted, some families lack the financial resources to provide them. That s why each year, St. Vincent Healthcare and Kohl s distribute more than 200 pairs of shoes to students in need at Broadwater, Newman, Orchard and Washington Elementary Schools. When you have a good pair of shoes, you are more likely to go and be active, says volunteer program coordinator Chris Cook. We engage these kids to be active and turn off the TV. Helping Students Learn Every week at Broadwater Elementary School in Billings, students welcome some very special helpers to their school: volunteers from St. Vincent Healthcare. Associates from departments throughout the organization work with individual students as well as small groups, helping them with reading, writing and other skills. Tutoring and mentoring services are just the beginning, however. In the library, St. Vincent Healthcare volunteers help to bind books, while others work to provide school supplies for children who otherwise would not have them. One of the associates favorite activities is the annual chili night, which allows them to serve dinner to Broadwater Elementary students and their families. St. Vincent Healthcare is also a partner at the Career Center, where high school-aged students have access to vocational and technical education, including courses focused on careers in medicine Montana Community Benefit Report

17 Helping Miles City Athletes Incredibly Rewarding SCL Health s Montana hospitals are increasing access to care and improving health literacy through partnerships with Montana schools. Whether stepping foot on the court, track, field or mat, Eastern Montana athletes always give their best to represent their schools, families and communities. That s why Holy Rosary Healthcare is pleased to have the opportunity to give them the best support and medical care available. Rob Brugger, director of the Holy Rosary Healthcare Therapy team, started offering sports medicine assistance in 1999 when he saw a need for it in the community. The program has continued to grow and now includes two dedicated licensed athletic trainers, Amy Tooke and Katie Patch of Miles City. Along with attending athletic events, as part of relationships with Custer County District High School (CCDHS) and Miles Community College (MCC), Tooke and Patch provide free athletic training sessions to public school and collegiate athletes from Miles City and surrounding areas through HRH s Training Room program. During the school year, the Training Room program allows student athletes access to HRH s athletic trainers for injury prevention, evaluation and treatment recommendations at no charge. Tooke, who has been a trainer in the community for 11 years, says giving athletes the support they need often means long hours, but her job is something she enjoys. Helping someone get better after they have an accident is incredibly rewarding, she says. You get to see athletes at their worst when they are not only physically strained, but emotionally strained as well. We work with them to get them back into their active lifestyle as quickly and as safely as possible. Though newer to the athletic training team, Patch says her time with them has been an excellent experience thus far. You hate seeing injuries but watching someone heal is incredibly fulfilling, she says. I can t stress enough how thankful I am to have two incredible women like Amy and Katie as part of our team, Brugger says. We truly could not do this without them. Their hard work and focused mentality have helped us keep this program going strong. 18 St. James Healthcare s Sports Medicine Program Benefits Area Athletes Keeping student athletes healthy and providing them the care they need if they are injured is at the heart of any sports medicine program. That includes the program at St. James Healthcare in Butte, which is led by the organization s Manager of Therapy and Orthopedic Services, Chris Heard. I ve been a part of this outreach program since 1997, he says. We have a team of physicians and athletic trainers in seven surrounding schools. These schools would not have the funding needed to support that level of coverage without this program. The schools impacted by the St. James Sports Medicine Program include high schools in Butte, Anaconda, Twin Bridges, Harrison and Sheridan. It also provides services at Montana Tech and the University of Montana Western. The program conforms to guidelines from the National Athletic Trainers Association (NATA). The program, which is offered in partnership with Montana Sports Medicine, offers community and peer education on sports-related injuries. Students who will be participating in high school sports also have the advantage of completing a sports physical either for free or for just $5, and the money earned goes back to each athlete s school s athletic program. Likewise, funds raised from student athlete EKGs as part of a physical is given to the St. James Healthcare Foundation to expand the athletic training program and fund much-needed materials. Chris Heard has been recognized by the NATA as an expert in his field. He presented the St. James Healthcare Medical Model of Athletic Training as a best practice in Boise, Idaho in March of Montana Community Benefit Report

19 Distracted Driving Course Targets Teens Behind the Wheel One of the biggest concerns parents deal with is allowing their teen drivers to get behind the wheel. Not only do they worry about other drivers on the road, but also the decisions their child may make in terms of distracted driving. To help teens understand the ramifications of distracted driving, the trauma team at St. Vincent Healthcare has developed a distracted driving course that provides an immersive, eye-opening experience. Students who are participating in a driver s education class have the chance to participate in distracted driving sessions. Students first don so-called drunk goggles, which simulate alcohol impairment by distorting their vision. They then attempt to drive a golf cart through an established course, being carefully monitored for safety. Following that exercise, they attempt to drive the course without the goggles, but while texting on a cell phone. Through the laughter which often ensues as these young drivers crush one orange cone after another comes an understanding of the impact these behaviors can have. As part of the course, students also commit to being a safe driver and avoiding distractions whenever they get behind a wheel. The trauma team s goal is to reduce the often life-changing impacts a car accident can have on teens, their friends and families. They also believe this class is a crucial part of a new driver s education process. This philosophy is shared by their colleagues at St. James Healthcare, who have also made a distracted driving course available to student drivers in Butte. Together, they hope that making these type of hands-on courses available will help to reduce the number of traumas and fatalities on Montana s roadways Montana Community Benefit Report 18

20 Improving Access to Care A woman can look up the schedule, find a location close to her, schedule an appointment and receive same care and technology she would receive anywhere. - Eric Pollard, DIrector of Radiology & Telehealth Services Mobile Mammography Coach Continues to Provide Convenient Screenings to Montana Women SCL Health is committed to providing Montana s rural communities access to key health screenings and specialty care. These innovative programs ensure access to high quality healthcare regardless of location. Mobile Mammography is not a new service for St. Vincent Healthcare; however, it is one that continues to deliver tremendous benefits to women in Billings and in surrounding rural areas. The mammography bus has been active for six years, says Eric Pollard, Director of Radiology & Telehealth Services at St. Vincent Healthcare. It s typically in action five to six times within any given week, visiting various locations across the state. Pollard says that there has been an outstanding response to the concept of mobile mammography. Many women who would have potentially postponed yearly checkups due to time or travel issues are now able to complete their screenings. This is significant, as it increases the likelihood breast cancer will be detected early and leads to faster treatment. The process for having a checkup on the mobile mammography coach is comparable to scheduling an appointment at a breast center, says Pollard. A woman can look up the schedule, find a location close to her, schedule an appointment and receive the same care and technology she would receive anywhere. St. Vincent Healthcare s mobile mammography coach is one of only two in the state of Montana. Screening takes as little as 15 minutes, and all screenings completed on the coach are transmitted to a boardcertified radiologist for review. Results are then mailed within approximately a week to each patient and her care provider to ensure rapid follow up if any concerns are noted Montana Community Benefit Report

21 eemergency Program Increases Access to Care in Miles City & Surrounding Areas Montanans living in rural locations surrounding Miles City now have the opportunity to receive the highest levels of emergency care thanks to Holy Rosary Healthcare s eemergency program. This program, made possible by a grant from the Leona M. and Harry B. Helmsley Charitable Trust, provides enhanced 24-hour access to emergency room specialists. Using technology specifically designed for hospital use, board-certified and emergency trained physicians and nurses at larger hospitals are providing remote assistance to Holy Rosary Healthcare providers in real time. This ensures real time consultations on trauma cases as well as patients suffering a heart attack, stroke or other critical condition. Because Holy Rosary Healthcare serves as a trauma center for outlying communities such as Ekalaka and Jordan, care sites in those areas are connected via the eemergency network. Doing so ensures critically injured patients receive care as rapidly and efficiently as possible Montana Community Benefit Report 20

22 Addressing Mental Health Needs Specially Trained Nurses Meet Unique Physical, Emotional Needs of Rape Victims By addressing the social and community context, SCL Health is responding to our communities mental health needs. The victims of rape suffer not only from physical injuries, but also mental and emotional injuries brought on by such an attack. These patients often feel vulnerable not only during and immediately after an assault, but also for weeks, months and even years to come. In an effort to meet the unique needs of these victims, Holy Rosary Healthcare has added three trained Sexual Assault Nurse Examiners to their staff. These nurses have undergone extensive training which allows them to provide an enhanced response to rape survivors. Without judging, they listen to the victim s story of what happened, carefully record what they hear and offer medical care as needed. Sexual Assault Nurse Examiners are also trained to conduct a forensic examination that may be two hours or more in length. Following strict protocols and with emphasis on the privacy and dignity of the patient, they gather vital evidence needed for law enforcement to investigate the assault evidence which could also allow attorneys to prosecute offenders. If needed, those nurses may also testify in any legal proceedings relating to that victim. These dedicated nurses are a valuable asset to both a victim in crisis and to the coordinated community response to sexual assault. They also assure victims are fully informed about their options following a rape and provide valuable resources to the victim that can assist that person in the healing process, including referral to an advocate at the Custer Network Against Domestic Abuse & Assault, or CNADA, in Miles City Montana Community Benefit Report

23 Community Crisis Center Serves Needs of Those With Mental Health, Substance Abuse Concerns As Montana s largest city, Billings is frequently a draw for travelers looking for shopping entertainment and yes, healthcare. It has also become a hub of sorts for those who are indigent, including those with issues relating to mental health and substance abuse. The Community Crisis Center was established in 2006 as part of a partnership between St. Vincent Healthcare, Riverstone Health and Billings Clinic. Funding is also provided by the Billings community via a public safety mill levy as well as the State of Montana and other grant resources. It is open 24 hours a day to provide help to those in crisis, offering assessment, stabilization and referral services. A key benefit the Community Crisis Center provides is the diversion of many individuals out of the criminal justice system and giving law enforcement the resources needed to handle this population. Center staff members have provided training to police officers, sheriff s deputies and other first responders who may be called to manage situations involving those with mental illness or substance abuse concerns. The most rewarding part of my job is building relationships with people who need help and guiding them towards the right path, says MarCee Neary, who has been the program s director since Our team knows we are doing the right thing by assisting them Montana Community Benefit Report 22
