3 BUSINESS AND ADMINISTRATION ACCOUNTANT Prepare income and balance sheet statements, consolidated statements and various other accounting statements and reports. Review and verify the accuracy of journal entries and accounting classifications assigned to various records. May analyze financial reports and records. In the absence of RN, may assume Charge Nurse responsibilities LICENSED PRACTICAL NURSE - POOL On call for in-house LPN duties MEDICATION ASSISTANT Under supervision of a licensed nurse (RN), prepare, administer and record select oral, nasal, rectal, inhalant, ophthalmic and transdermal scheduled medications to residents. Measure and record blood pressure and radial pulse. May also perform duties as nursing assistant. Requires successful completion of State of Wisconsin approved Medication Assistant Training Program CERTIFIED NURSING ASSISTANT Perform a variety of personal care, feeding, ambulation and special services for residents; keep nursing unit and residents' rooms in order. Other applicable job titles include: Certified Nurse Aide; Resident Assistant CERTIFIED NURSING ASSISTANT - POOL On call for in-house CNA duties CNA / UNIVERSAL CARE WORKER - RESIDENT ASSISTANT Provide assistance to residents in Assisted Living with their basic essential activities of daily living and instrumental activities (telephone, hearing and vision adaptive devices; mobility devices; light housekeeping; laundry). Remind residents to self-administer medications. Observe and report changes in residents physical/cognitive/emotional status to supervisor. Assist residents in dining area and activity programs SCHEDULER, NURSING SERVICES Responsible for all nurse, nurse aide and medical records scheduling. Collaborate with payroll for accuracy; arrange for replacement of call-ins. Perform other clerical tasks as assigned by nursing administration UNIT COORDINATOR Member of the medical records and supply staff who supervises unit clerks. Audit charts; coordinate medical supply purchases and inventory; transcribe orders. Reports to the Director of Nursing UNIT CLERK Perform clerical duties in a nursing unit related to admissions, transfers, discharges, deaths; chart / record required data; order supplies; complete census reports. May perform light housekeeping, meal preparation, and simple shopping. May transport client to doctor s office, stores, church, etc. Report any changes in client s condition to Supervisor. 10 NURSING SERVICE - continued HOME HEALTH AIDE Provide personal care services that facilitate a client s care at home but do not require performance by a RN or LPN. May assist client with personal care, transferring and ambulation, prescribed exercises, and selfadministration of medications. May perform simple nursing procedures, prepare meals and assist client in eating, and do other essential household services. Report changes in client s condition and needs to Supervisor. Complete appropriate charting and records HOSPICE RN / CASE MANAGER Plans, coordinates, documents and provides care to assigned caseload for hospice patients.

11 SPECIAL SERVICES SOCIAL WORKER (MSW) Perform a variety of resident and family services. Help residents adjust to medical, social and emotional problems; interpret social and emotional needs of residents to family and staff; provide information and referral services and serve as liaison with other agencies; process applications. May serve as department head and act as consultant to administrator and other departments SOCIAL WORKER (BSW) Process applications for admittance. Help residents and families adjust to the new environment; provide on-going help, information and referral services; serve as liaison with other agencies. May serve as department head SOCIAL WORKER DESIGNEE Perform social work services in a program set up by a Social Worker Consultant. Position does not require a social services degree, but is designated. This position is usually found in facilities with less than 110 beds ACTIVITIES DIRECTOR As department head / manager, plan and implement recreational, therapeutic and/or social activities to provide mental, physical and social stimulation for residents - both in group settings and one on one. Diagnose and treat speech, swallowing and language disorders as directed by physician; document treatments and progress. Another applicable job title is Speech Therapist, who functions under direction of Speech Pathologist OCCUPATIONAL THERAPIST Graduate of accredited OT program with OTR designation. Provide occupational therapy services and treatment to assist in the restoration of articular and muscular functions, develop coordination and tolerances, improve physical conditions and aid in mental rehabilitation; assist clients to perform the activities of daily living. May supervise COTA s and Therapy Aides CERTIFIED OCCUPATIONAL THERAPY ASSISTANT Graduate of accredited COTA program, with COTA designation. Under direction of OTR, provide occupational therapy services and treatments as prescribed. May conduct hobby, craft and recreational activities. Provide prescribed physical therapy services and treatments to maximize rehabilitation potential of client; document treatments and progress. May supervise PTA's and Therapy Aides.

Under direction of PT, perform PT services and treatments as prescribed; document treatments and progress THERAPY AIDE Assist therapists in providing rehabilitation therapy services and treatments; prepare and clean treatment areas and equipment; transport clients. May also do clerical tasks. Other applicable job titles include: Physical Therapy Aide; Occupational Therapy Aide; Rehabilitation Aide; Rehabilitative Aide; Restorative Aide RECREATIONAL THERAPIST Plan and organize medically approved recreation program for residents. Regulate content of program in accordance with residents capabilities, needs and interests. Counsel and encourage residents to develop leisure activities. Organize and coordinate special outings and accompany residents on outings. Prepare progress charts and periodic reports for medical staff reflecting residents reactions and progress or regression. Train volunteers in techniques of recreation therapy QUALIFIED MENTAL RETARDATION PROFESSIONAL Responsible for coordinating and monitoring of assessment, planning and implementing individual developmentally disabled programs while advocating for the resident throughout the process. Typically has specialized training in mental retardation DIETITIAN Graduate of accredited program with Registered Dietitian designation. Plans menus and all special diets; reviews charts; acts as consultant to the Food Service department and organization DIETETIC TECHNICIAN Assess resident nutritional status, develop care plans and may supervise food operations in accordance with all applicable regulations, codes, policies and procedures. Conduct quality assurance audit studies. Maintain resident nutrition records. Write weekly modified menus. Conduct orientation and training programs. May develop meal assembly / service and cleaning procedures. Requires graduate of a two year Dietetic Technician program with current ADA registration preferable PHARMACIST Graduate of accredited Pharmacy program with Registered Pharmacist designation. Set up department policies and procedures; dispense medications according to physicians' orders, following state and federal guidelines. Review residents' charts and medications sheets. Design and implement an inventory control system PHARMACY TECHNICIAN Trained and certified as a Pharmacy Technician. Carry out procedures under the directions of Pharmacist. 14 Executive Compensation Survey EXECUTIVE POSITIONS PART I Administrator The Administrator is responsible for administering, directing and coordinating all activities of the nursing home to carry out its objectives in the provision of nursing home care and related services. Responsible for the overall operation of the nursing home, formulates plans and policies and allocates resources for the achievement of the objectives Assistant Administrator Direct, supervise and coordinate functions and activities of one or more major departments. Consult with and advise Administrator on problems related to the operation of the facility. Recommend changes in administrative policies to achieve the objectives of the facility more effectively. During absence of the Administrator is responsible for planning, directing and coordinating all activities to ensure full patient care in accordance with accepted medical standards. (May be called: Associate Administrator) Senior Financial Officer Direct and coordinate activities concerned with financial administration, general accounting, patient business services, and financial and statistical reporting. Devise and install new and/or modified accounting systems to provide complete and accurate records of assets, liabilities, and financial transactions. Prepare detailed analyses of financial statements to reflect variances in income, expenditures, and capital asset values. Recommend to administration various means of reducing operating costs and increasing revenues. Generally reports to Senior Executive. (May be called: Business Manager, Controller, etc.) Senior Human Resources Executive Responsible for the entire range of personnel administration and human resources activity in the organizational unit. Develops, recommends and implements policies and programs in such areas as recruitment and employment, training, wage, salary and benefits administration, employee relations, communications and safety. Provides guidance and technical assistance in these areas to other staff and operating functions of the organization. (May be called: V. P. Human Resources, Director of Personnel, Human Resources Manager) Senior Nursing Services Executive Responsible for planning, organizing and administering the department of nursing. Develop, recommend and implement objectives. Interpret and put into effect administrative policies. Prepare and administer budgets. Select, train, and direct the rehabilitation staff in accordance with established guidelines. Assure compliance with applicable State and Federal codes and regulations. Organize and implement in-service staff training and community oriented education programs. Administer rehabilitation services within approved budget allowances. Maintain working relationships with community agencies. Monitor trends in rehabilitation services Director of Pastoral Care Plan, organize, and direct religious and spiritual activities to meet needs of residents, families, and staff.
