1 Community Support Services Summit 2015 Presented in Partnership by the North East Regional Community Support Services (CSS) Network & North East Local Health Integration Network (LHIN) Speaker Bios Perry McLeod-Shabogesic, Director of Traditional Programing, the Shkagamik-Kwe Health Centre Perry McLeod-Shabogesic of the "Crane Clan" is an Ojibway Anishinabe from N'biising (Nipissing) First Nation, located on the north shores of Lake Nipissing. Presently he works for the Shkagamik-Kwe Health Centre as the Director of Traditional Programing. He is a husband and father of four children and loves these family roles. Perry has been a cultural resource person, cartoonist/artist, writer, traditional helper and medicine harvester for many years. His spirit name is "Andzoked," which means "Story Teller" in Anishinabie. He is also a recognized Traditional Helper/ Oshkabewis/Pipe Carrier assisting and facilitating ceremonies for community members and families upon request. Perry has also worked in the areas of addictions, holistic health, social services, education, fisheries and culture. With his background he has been able to facilitate a variety of workshops. Marie Leon, CEO of ICAN and Chair of the Regional Community Support Services (CSS) Network Marie Leon has been with ICAN for 20 years. Her commitment to the organization was borne out of the experiences she and her family had dealing with her father's stroke. Marie attended Cambrian College and earned diplomas in both the Law Clerk Program and Business Administration. She joined ICAN in 1995 as book-keeper and worked her way up through the organization. She became the Accounting/IT Assistant, then the Executive Coordinator, the Manager of Corporate Services and then CEO. Marie is known for her openness and her hands-on approach to leadership and team building. Marie is an active and enthusiastic participant on a number of Community Support Service committees including the Sudbury/Manitoulin/Parry Sound CSS Network and Assisted Living Committee. She is also a member of the Early Adopters Steering Committee which is leading the roll out of the changes to HCCSA legislation. She is the current Chairperson of the Regional Community Support Services Network, and is leading the way on two important regional initiatives, the Regional Marketing Material project and the Regional Health Human Resources project. Terry Tilleczek, Senior Director - Policy and Health System Planning, North East LHIN Terry Tilleczek has been involved in health system planning in Northeastern Ontario for 25 years, joining the NE LHIN in Terry worked within the District Health Council (DHC) system from 1990 until 2005, the last 9 years as Executive Director of the Manitoulin and Sudbury DHC and the Algoma, Cochrane, Manitoulin and Sudbury DHC. Prior to joining the NE LHIN in the fall of 2006, he spent a year at the North Region Office of the Ministry of Health and Long Term Care. Terry holds a Bachelor of Arts degree from Wilfred Laurier University and a Master of Arts degree from the University of Toronto.

2 Joe McReynolds, member of the Minister s Expert Panel on Home and Community Care and Associate Senior Consultant with tng Joe McReynolds is an experienced professional who focuses his consulting business on good governance, leadership and system planning and integration. His clients include the health care sector, the arts and social service communities. Joe has executive experience in community services, local government, economic developments and health care. Joe combines his own business with being an Associate Senior Consultant with tng, a small boutique firm that focuses on governance and planning across a spectrum of not-for-profit organizations and associations. Joe continues his passion for innovative community development and capacity building. Joe has been active in his community sitting on several community Boards and offering himself as a candidate for local government. Since leaving the public sector, he has led the Ontario Community Support Association as CEO, been Chair of the Halton Peel District Health Council, and was the founding Chair of the Central West Local Health Integration Network. He currently is a Director of two national service boards Health Nexus Sante and Focus Accreditation. Joe is a recognized community service leader, advocate for client/family-centered care, and a pioneer of system change. Joe has recently been an active participant of the Minister s Expert Panel on Home and Community Care whose report has been accepted for transformation of care at home. Jennifer Michaud, Senior Project Manager of Enabling Technologies, North East LHIN A registered nurse, Jen is passionate about helping clinicians adopt enabling technologies. She started with the North East LHIN in February 2013 with the Policy and Planning team and lead initiatives related to Chronic Disease Prevention and Management until transitioning to the Enabling Technologies team in August Prior to that, Jen worked for OTN for almost 10 years as the NE Regional Director and then Provincial Telehomecare Manager. Jen is looking forward to supporting clinicians and clients in the continued adoption of tools that are improving access to CSS sector in Northeastern Ontario. Michel O Connor, Officer of Enabling Technology, Project Management Office, North East LHIN Michel O Connor (Mike) has been with the North East LHIN almost since its inception in He has held various roles over the years with the LHIN ranging from being a Planning Officer working with the service providers in Parry Sound and Temiskaming to leading the Health Human Resources Planning file to the mobilization of the Long-Term Care Homes sector. As an Officer with the NE LHIN s Enabling Technology and Project Management Office he is currently focused on the development of knowledge management practices for the LHIN. Mike is also the LHIN s Lead on the Integrated Assessment Record where he was a member of the Provincial Steering Committee. Mike holds an Honors Bachelor s Degree in Commerce and Sports Administration (H.BComm (SPAD) along with a Master s degree in Business Administration (MBA) interests in Information Systems and Organizational strategy and behavior. 2

3 Rebecca Bowes, Regional Coordinator Post Stroke Transitional Care Program, ICAN Rebecca Bowes is a graduate of Laurentian University where her focus of study included Psychology and Neuroscience. Rebecca began the first 10 years of her career in the mental health field as a psychometrist and counsellor before working primarily in the acquired brain injury field. Five years ago, Rebecca became the region s first Stroke Community Navigator when she joined the team at the ICAN Independence Centre & Network in partnership with Health Sciences North. In February of 2014, Rebecca took on the role of Regional Coordinator of the Post Stroke Transitional Care Program as it began its expansion to five more sites across the North. She is a member of the Ontario Association of Counsellors, Consultants, Psychometrists, and Psychotherapists as well as a member of a Provincial Navigation Chapter and the North Eastern Ontario Stroke Network Steering Committee. Lyle Foreshew, Director of Care, Maison Vale Hospice Lyle Foreshew has been an RN for 23 years, he is a graduate of Cambrian College. He is a Certified Hospice Palliative Nurse. A majority of the work has been in community base health care, including the expansion of attendant care services in the Sudbury Manitoulin area. For the past five 5 years, Lyle has worked at Maison Vale Hospice, providing end of life care. Recently has taken on the role as Director of Care for the hospice. Trish Lafantaisie, RN, Clinical Lead, Maison Vale Hospice Trish is an RN with a BScN who has been working in healthcare for 25 years. She started her nursing career working on a Palliative Care Unit in Toronto (for 10 years) as well as working part time with a Pain Management physician consultant. Trish then worked with the CCAC in Toronto and Sudbury in the positions of Intake Care Coordinator, Palliative Care Coordinator and in program development providing assistance with the development of palliative programing in the North East. She sat on the panel with the RNAO and assisted with the Best Practice Guidelines for End-of-Life care. Trish has been working at Maison Vale Hospice for the last 1.5 years initially as a residential floor nurse and in the last four months taken on the position of Clinical Lead. Monica Bretzlaff, Manager: North East Behavioural Supports Ontario (BSO), Provincial BSO, Senior s Mental Health Monica has held various roles throughout the past thirteen years at North Bay Regional Health Centre (NBRHC) as Psychogeriatric Resource Consultant (PRC); Director of Kirkwood Place; and most recently as Provincial- Regional-District Manager for Behavioural Supports Ontario and Manager for Senior s Mental Health-Regional Consultation Service. Monica has over 17 years of experience working in the field of Geriatrics, acquiring vast competencies in diverse settings such as In-patient Geriatric Psychiatry, Longterm Care, Community Care, Alzheimer Society, College and University Educator; Provincial PIECES & U- FIRST Educator; GPA Master Coach and RNAO Best Practice Champion. Monica is an active advocate in fostering community partnerships and linkages focused on person-family centered care. She serves on several local, regional and provincial committees and has contributed to numerous publications and inter-collaborative program development. 3

4 Heidi Resetar, Occupational Therapist and Advanced Clinician Practitioner in Arthritis Care, North Shore Tribal Council Heidi Resetar is an Occupational Therapist who has been providing community based services in a variety of settings in North Eastern Ontario since graduating from the University of Western Ontario in Heidi currently provides community based Occupational Therapy services to First Nation communities along the North Shore of Lake Huron as part of the Community Support Services Program within the North Shore Tribal Council. In response to the high incidence of arthritis experienced within the North Shore Tribal Council communities, Heidi completed the Advanced Clinician Practitioner in Arthritis Care (ACPAC) Program in June of The ACPAC program is a 10 month certificate-based program at the post licensure level that is hosted by St. Michael s Hospital and The Hospital for Sick Children in Toronto, Ontario in collaboration with the University of Toronto. She is currently in the process of developing an Arthritis Management Program within the North Shore Tribal Council First Nation Communities. This new model of care will utilize this new extended practice role to triage patients with osteoarthritis and inflammatory arthritis in order to improve access to specialized arthritis care in this time of limited resources. The Ontario Telemedicine Network (OTN) is one of the tools that will be utilized to implement interprofessional client-centred collaborative care. Mark Dowdall, Clinical Manager for Utilization, Health Sciences North Mark Dowdall is currently the Clinical Manager for Utilization with Health Sciences North. Prior to this, he was a medical social worker. Colleen Cleroux, Manager of Client Services, North East CCAC Colleen Cleroux manages the Access Team of Care Coordinators, who work at the Health Sciences North (HSN) Site. Her team collaborates with HSN staff, and various community partners, with the goal of facilitating the most appropriate plan of care/discharge destination, for each patient. Frankie Vitone, Senior Director, Client Services, North East CCAC Frankie Vitone holds a Bachelor of Science in Nursing and a Masters in Health Studies. She has enjoyed a varied career to date, sharing her nursing skills within acute care, public health and clinical education settings. In 1994, Frankie turned her focus to home care, joining the team at the Sudbury Home Care program, which later became the Manitoulin-Sudbury Community Care Access Centre. In 2007, Frankie played a key role in provincial amalgamation that united five CCACs under a single banner - the North East CCAC. Today, she leads a Care Coordination team who provide quality home and community care to 16,000 clients on any given day across the North East. Tracy Howson, Quality Improvement Specialist with Health Quality Ontario Prior to joining Health Quality Ontario (HQO) Tracy worked for Quality Improvement and Innovation (QIIP) with a focus on primary care. She has a Bachelor of Science and Master in Public Administration. In addition to Lean Six Sigma, Tracy completed the Improvement Advisor training at IHI. During her career Tracy has managed a wide range of projects across the age and health spectrum. Examples include programs such as a provincial maternal newborn demonstration project, the Ontario Chronic 4

5 Disease Prevention Alliance, The Heart Health Resource Centre, and the Durham Regional Diabetes Education Program. Her particular interests are in collaboration and complexity. Lynn Dionne RRT, Team lead, Quality Improvement Plans, Health Quality Ontario Lynn s keen interest in health care quality has been complemented with over 25 years of experience as a respiratory therapist. Prior to joining Health Quality Ontario, Lynn was a quality improvement consultant with the Centre for Healthcare Quality Improvement (CHQI) where she held various roles and responsibilities. At Health Quality Ontario, Lynn is Team Lead with Quality Improvement Plans. She has a MHSc (Masters of Health Science) in Health Administration from the Institute of Health Policy, Management and Evaluation at the University of Toronto, BSc in biology from Laurentian University, Diploma in Respiratory Therapy from the Michener Institute for Applied Health Sciences, and is Lean Six Sigma Black Belt candidate. Dawn Condon, Stand-Up Facilitator and Fitness Instructor Dawn Condon is a certified Lifestyle Management Coach, Personal Trainer, Wellness Coach, Group Fitness instructor, Ageless Grace facilitator and Yoga Teacher. Her specialties include weight management and nutrition, exercise and active lifestyles, seniors and seniors fall prevention, stress management, yoga and yoga therapy. She is a long standing fitness instructor and personal trainer at the Sudbury YMCA. She has been teaching senior s yoga and chair yoga at the Parkside Older Adult Centre for eight years. Dawn has also been a Stand-Up Facilitator with the Sudbury and District Health Unit for over four years. Most recently, she has partnered with the NEOMO Medical Group to become the Wellness Coordinator. Monique Rocheleau, Directrice générale adjointe/ Associate Executive Director, Réseau du mieux-être francophone du Nord de l Ontario A member in good standing with the Ontario College of Social Workers and Social Service Workers, Monique Rocheleau holds a Master s Degree in Social Work from Laurentian University. She has over 20 years experience within the health and social service sectors, having worked as a Senior Officer with the North East LHIN, Program Supervisor with the Ministry of Community and Social Services, and as a hospital consultant with the Ministry of Health and Long-Term Care. Monique is currently the Associate Executive Director with the Réseau du mieux-être francophone du Nord de l Ontario. Monique has spent several years volunteering with various organizations such as HANDS the Family Help Network, Northeastern Ontario Stroke Network, and the Timmins Learning Centre. Monique is also a past Chair of the Board of Directors for Collège Boréal. Monique Lapalme, North East LHIN French Language Services Officer Monique Lapalme started her career in the health care sector as a medical radiation technologist and has worked in the hospital system in Sudbury, Moncton and Quebec City. She then worked for over 20 years as French Language Services Coordinator, including a role as senior coordinator for Northern Ontario and, since 2010, as FLS coordinator with the North-East LHIN. 5

6 Angela Recollet, Executive Director, Shkagamik-Kwe Health Centre Angela Recollet is an Ojibwe woman from the Wikwemikong Unceded Reserve and a member of the Wahnapitae First Nation. A mother of two and a grandmother of four, she resides in Sudbury. Angela has held her current position at Shkagamik-Kwe Health Centre since March of Previous to this post, Angela was the lead in Aboriginal Affairs at Laurentian University from Cumulatively, she has extensive experience working with the aboriginal community both as an advocate in education and in improving the general living and health conditions of aboriginal populations. Some of her experience at the governance level includes: the Northern Ontario School of Medicine Board of Directors, Social Planning Council Board of Directors, Board of Directors for the Association of Ontario Health Centres and is a member of the Greater City of Sudbury s Healthy Communities Cabinet. More recently she has been appointed to the Ministry of Long Term Care s Expert Advisory committee on Primary Care. She also participates as and External Advisor to the Ministry of Corrections and Community Safety. Sylvie Gaskin, Manager of Privacy and Risk, Ontario Telemedicine Network (OTN) A Social Worker with over 20 years experience in healthcare leadership in Ontario, Sylvie holds a Master s Degree in Social Work from Laurentian University. Sylvie is also a Certified Information Privacy Professional/Canada CIPP(C) through the International Association of Privacy Professionals (IAPP). In addition to providing leadership for the privacy function, Sylvie leads OTN s enterprise risk, records and policy management processes. Nancy Andrews, Health Information Custodian, Health Sciences North Nancy Andrews is responsible for managing privacy and information security for Health Sciences North corporately, in its role as a Health Information Custodian, and as a Health Information Network Provider for shared information systems. With her over 15 years of information technology experience in the health care sector, she has provided privacy leadership during the implementation of the Integrated Assessment Record for the NE LHIN and is currently providing the same for the Connecting North East Ontario project. Nancy is a Certified Information Privacy Professional/Canada and has also earned her Certification in Risk and Information Systems Control. Stephanie Lefebvre, Manager, Community Mobilization Sudbury/ Gestionnaire, Mobilisation communautaire Sudbury Prior to joining the Canadian Mental Health Association Sudbury/Manitoulin, Stephanie had been working in the field of Public Health for over 10 years. With a focus on health equity and the social determinants of health, she has contributed to programs and policy at the local, provincial and national levels. She is passionate about creating opportunities for all members of our community to be well and live life to the fullest. Stephanie has guided the Community Mobilization Sudbury program since its launch in May 2014 watching it grow from eight founding agencies to 24 community partners. Stephanie holds a Masters in Social Work in Community Development and Social Planning from Wilfrid Laurier University. 6

7 Lynelle Hamilton, Senior Practice Advisor to PSNO, has over 30 years experience in health and human services. She has worked as a consultant to the Ministry of Health and the OCSA on the development of the Personal Support Worker Training Standards. She is the author of Being a Personal Support Worker. She has delivered over 500 workshops and programs. Her specialities include Training Needs Assessments, Prior Learning Assessments, Adult Education Personal Support Programme development, PSW Scope of Practice and Policy Issues. Portia Machiwenyika, Training and Marketing Coordinator for Capacity Builders. For the last two years, Portia has been involved in planning the OCSA Conference as part of the Planning Committee. She is heavily involved in her local church and understands first hand what it is like to juggle work and life issues. Portia holds a Bachelor of Commerce degree and a Bachelor of Theology degree. Diane Lajambe, District Branch Manager, Canadian Red Cross Diane Lajambe has beeendistrict Branch Manager, Canadian Red Cross since 2004, managing several Community Support Services including transportation, meals on wheels, home maintenance, PATH, friendly visiting, telephone assurance, assisted living for high risk seniors in addition to other core programs of the CRC. She is also a member of the North East Community Support Services Regional Network, Algoma CSS Network; Member of the HCCSA Early Adopter Workgroup. Diane has participated in several responses of vulnerable people including the evacuations of Kashechwan residents to Sault Ste. Marie and, in 2012, she took part of the response team for the Thunder Bay Floods. She was involved in the initial implementation of the Vulnerable Persons Registry (VPR) in Sault Ste. Marie and is currently working on the implementation of VPR in Thunder Bay. Wendy Carew, North East LHIN Regional Coordinator of the Stay on Your Feet fall prevention strategy Wendy Carew brings many years of experience in community engagement, communication, research, planning and evaluation. With over 20 years working within the public health system, she contributes her solid knowledge and experience in population health approaches to the prevention of injury and disease. Anik Proulx, Public Health Nurse, Sudbury & District Health Unit Anik has been a Registered Nurse for the past eight years. She started her career in Oncology at Health Sciences North, worked as an Occupational Health Nurse in the mining sector in Sudbury, and as a Nurse Manager responsible for occupational health services and infection control at the Sensenbrenner Hospital in Kapuskasing (her home town). She has been active, as a Public Health Nurse, in the area of Falls Prevention at the Sudbury and District Health Unit for the past two years and is the chair of the local Stay On Your Feet Falls Prevention Coalition working in partnership with many agencies in the community. She has also worked in the area of control of infectious diseases, sexual health and comprehensive tobacco control. In the fall of 2014, Anik is also a Canadian Falls Prevention Curriculum trainer in the North East and has since trained many members in the community. 7

8 Terri Lazinski, Public Health Nurse, Sudbury & District Health Unit Terri is a Registered Nurse who started her career in the area of Critical Care nursing and as a Critical Care Co-ordinator. As a public health nurse, she has worked in the area of high risk community home visiting, sexual health, comprehensive tobacco control and has recently joined the Falls Injury Team. She recognizes the benefits to our communities when we work collaboratively to benefit the health of our citizens. Rita Mannarino, Nurse Practitioner, Palliative Care Program, North East Community Care Access Centre, Rita Mannarino, RN(EC), MScN, PHC-NP has worked in various settings including, ICU, geriatrics/ltc, oncology, palliative care and primary care. Rita has held various roles including front line nursing, educator, pain specialist and management. Rita is now working in end of life/palliative care helping patients living with terminal illness and their loved ones to ensure dignity and quality of life by ensuring that pain & other symptoms are addressed, and that issues including advance care planning, spiritual needs and their desired place of death is met. Rita works with an interdisciplinary team to ensure that patients at the end of their life, receive holistic care and that their mind, body and spirit is cared for during their final journey in life. 8
