2 North Help Coalition North HELP Healthcare SME Coalition The New York City Department of Health and Mental Hygiene, Office of Emergency Preparedness and Response (OEPR), Core Program Budget Period 3 Elective Deliverable 1: The North HELP Coalition was established in 2010 to increase the level of emergency preparedness for vulnerable populations with special medical needs. Currently the Coalition focuses on the needs of dialysis patients and the facilities that serve them.. Services Provided The goals of the North HELP Coalition are to improve the delivery of care to vulnerable populations with special medical needs during disasters. This is accomplished by providing strategies and tools for providers and patients. Activities that support these goals include: o On-going engagement of Dialysis Centers in emergency preparedness initiatives o Conducting baseline emergency management assessments, and identifying and sharing best practices o Education and Training including: Incident Command System, Infection Control, and NIMS o Providing technical assistance in developing and enhancing emergency management plans and protocols o Developing and conducting drills and exercises Hospitals selecting to work with the North HELP SME Coalition must meet at least one of the following criteria: o Be able to commit administrative and/or clinical staff with working knowledge of ESRD and dialysis OR o Have at least one (1) stand-alone dialysis center within their catchment area OR o Provide services to a patient population which includes diagnosis of ESRD OR o AND Remain a member in good standing throughout the contract period by: o Attending at least 75 % of coalition meetings o Participating in the exercise o Participating in a minimum of one deliverable work group Point of Contacts Kevin Chason, DO, Director of Emergency Management, The Mount Sinai Hospital Center- Kevinchason@mountsinai.org Donald Cardone, EMT-P, CIC, Associate Director, Emergency Management, The Mount Sinai Hospital Center- Donald.cardone@mountsinai.org

3 NYC Geographic Coalitions NYCHCC \ Geographic Coalitions The New York City Department of Health and Mental Hygiene, Office of Emergency Preparedness and Response (OEPR), Core Program Budget Period 3 Elective Deliverable 1: Geographic Coalitions are defined by borough boundaries and are often administratively anchored by local/neighborhood hospitals. They are a collaborative network of health care organizations and their respective public and private sector partners. Geographic Coalitions share an overarching goal of improving the level of emergency preparedness in their communities, thereby assuring higher quality health care outcomes in a disaster. Opportunities Provided There are many benefits to joining a Geographic Coalition including: o Participation in local, borough-wide and regional trainings and exercises o Enhanced communication and situational awareness o Professional networking and mentoring opportunities o Sharing subject matter expertise and best/promising practices o Accreditation compliance o Joint hazard vulnerability analysis o Cost sharing Geographic Coalition membership is open to groups and organizations with an active interest in promoting emergency preparedness to their constituencies and communities. Typically, coalition members include healthcare providers (e.g., hospitals, nursing homes), community based organizations, local responders (e.g., NYPD, FDNY), and governmental organizations (e.g., OEM, DOHMH). Each Geographic Coalition has its own Charter and rules governing membership and expected levels of participation in coalition activities. 4 PCEPN Healthcare SME Coalition Primary Care Emergency Preparedness Network The New York City Department of Health and Mental Hygiene, Office of Emergency Preparedness and Response (OEPR), Core Program Budget Period 3 Elective Deliverable 1: The Primary Care Emergency Preparedness Network (PCEPN) is a partnership between the Community Health Care Association of New York State (CHCANYS) and the Primary Care Development Corporation (PCDC). PCEPN works to increase the level of emergency preparedness for the primary care sector in NYC. Services Provided : The goal of PCEPN is to strengthen emergency preparedness among NYC primary care sites. 5 NYC PEDIATRIC DISASTER COALITION PICU Surge Capacity Functional Exercise Series Hospitals meeting the following criteria may fulfill the Core Hospital Contract Deliverable E1: Healthcare Coalition Participation by participating in this exercise series. To qualify The hospital must have a PICU Surge Plan The hospital will be responsible for designating a hospital planning team of trusted agents who will also be participants in the design of coalition exercise. The planning team or designated representatives are required to attend 80% of all PDC exercise planning meetings Likely to be monthly based on the PDC contract but may include additional workshop planning meetings The hospital is expected to participate in all planning events in Year 1 to be eligible for the exercise in Year 2. The hospital must have their own internal and external communications systems The hospital may be asked to provide meeting and conference space for events that needs to be scheduled there Hospitals will be responsible for all plan revisions, but will be provided with tools and guidance by the PDC personnel How Does this Benefit Your Hospital? It may meet Joint Commission exercise requirements for 2 years The hospital will have an opportunity to test their Pediatric Critical Care Surge Plan and to collaborate with a coalition within the DOH with SME in pediatrics (PDC). The hospitals that work with the PDC will be assisting the city agencies test their plans for a major pediatric disaster. The hospitals would be paid for their work $3000 for this year.
