OCHS, an independent nonprofit corporation governed by a consumer majority board as a Federally Qualified Health Center, has a long standing non-exclusive affiliation with Beth Israel Deaconess Medical Center in Boston and refers most of its patients requiring acute care to Cape Cod Hospital in Hyannis. 2 Its health centers are located on the Outer and Lower Cape along the National Seashore, starting on the tip of Cape Cod in Provincetown, extending down the arm of the Cape to Wellfleet, and wrapping up past the Cape's elbow in Harwich. A fourth health center is planned in Brewster on land purchased in 2014 for this purpose. In addition to its three health centers, it also operates retail pharmacies in Wellfleet and Provincetown, a Dental Clinic within its OCHS Provincetown Health Center, and three Women, Infants and Children (WIC) sites in Harwich, Orleans, and Provincetown. The organization has grown rapidly, from $6.4 M in FY10 to $21Min FY 2016 and from serving 10,500 to over 16,000 unique patients, in the past six years as a result of increased access to health insurance and new executive leadership team. Re-branding of the organization may be required as OCHS plans to expand to serve the 70% of its population that are not yet within 20 minutes of its health centers. It hopes to achieve a sustainable scale of operations and serve its mission within its entire catchment area. Ensuring access to quality primary and episodic care to area residents and visitors is priority one! The motto at Outer Cape Health is we treat you well. OCHS s Provincetown site is a model of what it hopes to build at its other sites, OCHS Wellfleet, Harwich and a future OCHS Brewster. OCHS is licensed by the Massachusetts Department of Public Health for medical clinic and mental health clinic licensure, Board of Registration in Pharmacy for its two pharmacies and a satellite site at Seashore Point Skilled Nursing Facility, certified to provide Urgent Care at its OCHS Provincetown site, licensed by Mass. Dept. Public Health to provide medical radiology including Computerized Radiology and Mammography and Accredited by The Joint Commission for ambulatory care as well as Patient Centered Medical Home. 3 The Chief Development Officer role is an ideal opportunity for a seasoned development professional to encourage and inspire donors and community members to deepen their connections to Outer Cape Health Services commitment to their communities. Position Overview Chief Development Officer As the leader of the Development effort, the Chief Development Officer (CDO) will work in close partnership with the Chief Executive Officer and is expected to contribute broadly to all aspects of the fulfillment of the Health Center s mission and growth, by establishing a diversified fundraising program, determining and then meeting fundraising goals. The incumbent will be responsible for the strategic development, solicitation and management of the Health Center s efforts to substantially increase both unrestricted and capital support from foundations, corporations, government agencies, grateful patients, and the larger community of donors (Program Management solicits and manages program and service grants and contracts). S/He will oversee all major gifts, planned giving, and annual giving fundraising activities at the Health Center, including application of a moves management approach to a portfolio of major gift donors and prospects. S/He will lead activities pertaining to the identification, cultivation, solicitation, and stewardship of gifts from individual, corporate and foundation contributors, donors, patients, philanthropic organizations, and all other sources. S/He will develop and implement an annual giving program, including donor cultivation and solicitations, signature special events, patient/membership events, and other cultivation opportunities. The new Chief Development Officer will also be responsible for launching a successful campaign to build and expand health facilities. Essential Job Functions: Works with the Chief Executive Officer to strengthen and build successful programs for: annual giving, major giving, planned giving, corporate giving, special events, foundation grants (corporate, public, private), and multiple capital campaigns. Identifies, cultivates and solicits all appropriate unrestricted and capital funding sources. Responsible for the organization of a near-term future capital campaign, its strategy, implementation, infrastructure, leadership, training, timeline, and execution leading to achievement of campaign goals. Cultivates and maintains long-term relationships with corporations and public and private foundations, and develops strategies for philanthropic support. Cultivates and maintains relationships with Federal, State, and municipal governments and their funding agencies for capital and unrestricted gifts, but not program development awards. 4 Prepares written materials, including correspondence, solicitation materials, acknowledgements, and regular communications as part of the stewardship program. Works with Marketing and Communications staff to prepare materials, including brochures, articles, ads, and on-air announcements, to promote the concept of major giving to OCHS. Works with appropriate clinical and management staff, develops, prepares and monitors competitive proposals to foundations to achieve annual capital and unrestricted support goals for foundation giving, and to legislators and other government agencies, and guides these through the appropriate channels. Strengthens and expands government support for Health Center s visibility and stature within government arenas, in conjunction with the Health Center s programs. Identifies, qualifies and builds relationships with corporations, creates and manages strategic annual giving opportunities for corporations for sponsorship of events, workshops, programs, and publications, and develops and manages sponsored projects. Promotes a collegial atmosphere within the development department, working cooperatively with all fundraising officers to help them and the department attain their overall financial and advancement goals. Oversees and manages all aspects of the planned giving program, supervising gifts made through bequests, trusts, charitable gift annuities, and other planned giving vehicles. Ensures an accurate and up to date database of all current and potential donors, and reconciliation with Accounting Department. Creates and oversees donor recognition system to ensure timely expression of thanks, and activities to encourage continued support. Participates in community meetings and organizations to represent and promote the interests of Outer Cape Health in the community. Provides training, feedback, recognition and motivation to Board members, key staff, and volunteers in fundraising and solicitation techniques. In conjunction with the Chief Executive Officer, maintains an awareness of trends in legislation affecting funding in community health. Directs the production of all print and non-print development and collateral materials. Regularly reports development program status and public relations progress to Chief Executive Officer. Works collaboratively with Board President and Chair, Development Committee and Capital Campaign Committees. Other Duties and Responsibilities: Other related duties as assigned. 5 Preparation, Knowledge, Skills and Abilities: Bachelor s Degree and/or Master s Degree in related field preferred, with 10 years experience in corporate, foundation, major gifts and annual giving, program planning, and solicitation. Fundraising professional with a proven track record of success managing and directing a sophisticated development operation and playing a leadership role in a capital campaign. Budget management experience required. Capital campaign experience essential. Must have an understanding and commitment to working in a nonprofit environment. Demonstrated commitment to and experience with healthcare preferred. Exceptional leadership qualities to include: initiative, drive, motivation, creativity, presentation skills, time management, collaborative working style, superior organization, and strong, persuasive writing and communication skills. Must be able to develop both internal Development and Management team engagement as well as develop effective collaborations with leaders in Cape Cod and Boston based charitable and corporate foundations. Must be relentless in the pursuit of major gifts. Ease and speed in conceptualizing and executing proposals, correspondence, and other associated communications. Must have strong prospect research and cultivation skills with the ability to relate to and work with prospective donors and community leaders; experience with foundation proposal writing. Demonstrated track record in leveraging gifts and grants in a health center environment preferred. Must be competent using computers and technology to support fundraising efforts. Must be passionate about the mission of the organization and must be highly skilled at making the ask. 6 Development Overview Outer Cape Health Services is currently raising $500,000 through annual appeals, events, and individual, corporate, and foundation support. OCHS has generated $900,000 in Restricted, mostly capital campaign gifts. Separately, a grants program raises $2.7 million annually. There is a need to develop and launch a significant capital campaign to raise critically need funds for new health center buildings in Harwich, Wellfleet, and Brewster, and administrative offices. The goal of this effort will be to raise $10 - $15 million over the next five to seven years. Client Overview Nothing is more important than the health and wellbeing of families, friends, and neighbors. Outer Cape Health Services recognizes that we live in unique communities where people care about and depend on one another. Access to quality healthcare for all is a critical part of that caring. As a licensed community health center, OCHS provides primary care to patients of all ages. No one will be denied access to services due to an inability to pay. Discounts are available based on family size and income. Integration of mental health, dental health, and vision care has been achieved at its OCHS Provincetown site and is anticipated in Wellfleet and Harwich upon the completion of replacement health center facilities. Specialty care services are also available to Outer Cape Health Services primary care patients. In addition to primary care, OCHS offers same-day episodic care to residents and visitors year round. Outer Cape Health Services operates three centers (Provincetown, Wellfleet, and Harwich), two pharmacies and dozens of services and programs serving the eight towns of the Lower and Outer Cape. In 2015, approximately 16,500 people turned to the organization for care. Outer Cape Health Services is accredited as a Patient Centered Medical Home by The Joint Commission and also as a Licensed Medical and Mental Health Clinic of Massachusetts Department of Public Health. OCHS is a Federally Qualified Health Center, as designated by the Bureau of Primary Health Care of Health Resources Services Administration, that provides cash and non-cash resources to support FQHC's through its 330 funding program. 7 Clinical Lab Services at the Provincetown and Wellfleet Health Centers Attending physician care and pharmacy services at Seashore Point Skilled Nursing Facility in Provincetown OCHS Specialty Care Services, such as Infectious Disease through its well established HIV Care Management program, Pulmonary, Dermatology, Cardiology, Upper Extremity Orthopedics, and Office-Based Opiate Treatment Program Referral Services for Specialists at Beth Israel Deaconess Medical Center, Cape Cod Healthcare and Other Specialty Practices Health Insurance Enrollment Program, called Healthy Connections, provided on a free and confidential basis, that serves over 10,500 unique individuals each year, at three sites, to many who are not otherwise served as patients, also provides referrals to enabling services such as food pantries, supported housing, and transit. Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966), Outer Cape Health has a long history in pursuit of its mission. In 1996, Outer Cape Health signed a non-exclusive affiliation agreement with the Beth Israel Deaconess Medical Center, a teaching hospital of Harvard Medical School. Outer Cape Health Services, Inc. (OCHS) is governed by a Board of Directors, the majority of whom are patient consumers of its health services and representative of its seasonal and year round residential community. Their backgrounds include finance, community affairs, medicine, health services management, community-based applied research, health disparities advocacy, nursing, computer engineering, and business management. Services: Outer Cape Health Services provides comprehensive primary care, urgent, walk-in care, and specialty care, along with a range of supportive services, to residents, workers, and visitors, including internal medicine, family medicine, pediatrics, nurse midwifery, and osteopathy. Its highly qualified team of providers encourages preventive screenings, such as regular check-ups, inoculations, blood pressure screenings, nutritional counseling, and well-baby/child care. In 2015 OCHS achieved second top quality scores among the 21 pods and 2,700 physicians composing Beth Israel Deaconess Care Organization, despite having the highest average patient acuity scores and the highest proportional Medicare Pioneer bonus for managing quality and cost for its patients. 8 In addition, Outer Cape Health provides extensive outreach and support services through the WIC (Women, Infants, Children) nutrition program and through Healthy Connections, which helps uninsured people with publicly funded health insurance and social services programs. Primary care providers work closely with Specialists who are available either at OCHS sites or at Beth Israel Medical Center, Cape Cod Hospital, or other specialty practices. Once a patient registers as an Outer Cape Health patient the organization can provide the quality care needed. As a registered patient, its on-call physician will be available to help 24 hours a day. Leadership: Sally Deane, MPH Chief Executive Officer Sally Deane, M.P.H., began as Chief Executive Officer at OCHS, January, Ms. Deane s accomplishments in health services leadership are both extensive and remarkable. Most relevant to OCHS, Ms. Deane served as Executive Director of Fenway Community Health Center, , achieving both a financial turn-around, renovation, initial clinic licensure and response to the HIV crisis including founding of a research program originally focusing on the etiology, treatment and prevention of HIV that has grown to become the largest global LGBTQ health research institute. In her role as Assistant Director of Massachusetts Medicaid Program for Ambulatory Care she introduced case mix reimbursement rates and resource based relative value rates for professional services to achieve policy goals through economic incentives. At Cambridge Management Group, Ms. Deane was Senior Manager serving hospitals and insurers with strategy formation and business development. As principal of Integrated Health Strategies consulting firm, she guided the success of a strategic development plan for a range of health center, hospital and health related organizations. As Vice President for Healthcare of Chadwick Martin Bailey, Ms. Deane oversaw the OUTER CAPE HEALTH SERVICES BACKGROUNDER 8

9 development of market research projects for a range of clients evaluating and re-positioning their health related services, products, insurances, national foundations and disease oriented consortiums for improved success in both domestic and international settings. Ms. Deane s results-oriented commitment to improve community health has guided her work as a management consultant domestically and internationally providing strategic and business plan development and market research studies for hospitals, health centers, health departments, foundations, advocacy organizations and insurers. She has served as Chair of the Board of many health-related organizations, ranging from Boston Women s Health Book Collective, authors of Our Bodies Ourselves to New England College of Optometry s New England Eye Institute. Ms. Deane received her M.P.H. in Health Policy & Management from Boston University School of Public Health where she remains as an adjunct faculty and Board of Advisors member. Previously she attended and completed all but her dissertation at the Boston University School of Education and attended the Graduate School of Management. Ms. Deane attended public schools in Seward, Fairbanks, and Anchorage Alaska before attending and graduating from Whittier College. Ms. Deane purchased her first Provincetown home in1993, where she now resides. Ms. Deane is passionate regarding the mission of Outer Cape Health Services and intends for her service to be a capstone of her career. Location Outer Cape Health Services eight towns are composed of nearly 50,000 year-round residents that swell to at least three times that with summer visitors to the National Seashore. Full of history, beautiful and fragile environmental conditions, and known as an international tourist destination, it has its main health centers in the three following locations with a fourth planned in Brewster: Wellfleet is uniquely characterized by its geography, culture and people. Located 75 miles out into the Atlantic Ocean, it's only two miles wide, with beautiful beaches on both the bay and Atlantic sides. Despite its narrow width, strangely enough, it's endowed with beautiful, clear spring-fed ponds, or kettle holes, which were created by glaciers several thousand years ago. 10 Wellfleet Harbor is an actively changing panorama of fishing boats, yachts, trawlers, and, at low tide, flats of shellfish. The site of the Marconi Wireless Station in South Wellfleet, Wellfleet also boasts some of the best nature trails in the Cape Cod National Seashore, the Atlantic White Cedar Swamp and Great Island. Harwich is a vibrant town composed of eight villages, located on the warm side of the Cape peninsula, with an extensive shoreline on Nantucket Sound. The year-round population is approximately 12,677 with a seasonal increase to 37,000. With miles of rivers and marshes, and a coastline of sandy beaches dotted with the town's four picturesque harbors, Harwich has the unique ability to provide every form of aquatic activity available: quiet canoeing through the great marshes of the Herring River, water skiing on Long Pond, deep sea fishing out of the harbors, fly fishing in several of the smaller ponds, or swimming and sunbathing on the sandy Nantucket Sound and Pleasant Bay beaches. Provincetown is located at the extreme tip of Cape Cod and is a small coastal resort town with a yearround population of just under 3,000 and a summer population of as high as 60,000. After landing in its bay in1620, the Pilgrims wrote the Community Compact before sailing to Plymouth Rock where there was more land and conditions for long-term growth of the settlement. It has some of Cape Cod s best beaches (Race Point Beach and Herring Cove Beach are favorites). Plenty of cool boutiques, restaurants, theatre, and art galleries give this town a fun and unique vibe.
