2 The McGill University Health Centre, like hospitals elsewhere, has taken important steps toward engaging patients in self-care and quality-improvement initiatives, offering a first taste of the rewards that come when managers, providers and patients work together toward better processes and outcomes. This conference aims to tap into the wealth of experience developed here and abroad, equip care and management teams with the inspiration and tools they need to promote patient engagement, and help shape policies that will support their efforts. Ms. O Connor is an Associate Professor in the Ingram School of Nursing at McGill, a Certified Health Executive, and Past President of the Academy of Canadian Executive Nurses. She has completed fellowships with the Canadian Foundation for Healthcare Improvement (EXTRA program), the Canadian College of Health Leaders (CCHHL) and the U.S. Commonwealth Fund. Her current efforts are focused on engaging patients and frontline staff in the re-design of care delivery systems, and in supporting evidence-informed practice and administration. In 2012, Ms. O Connor was recognized as one of the Top 8 Women in Business in the Healthcare Sector in Quebec, and in 2014, she was awarded the CCHL National Nursing Leadership Award. Normand Rinfret is a seasoned executive who began his career in the education field but seized an opportunity over thirty years ago to work in health care. Passionate about improving patient care, making the hospital workplace a stimulating, effective environment and supporting research, Mr. Rinfret continues to use his business acumen, creativity and strong work ethic to build and maintain relationships and processes. Before being appointed Director General and CEO by the Board of Directors of the McGill University Health Centre in 2012, Mr. Rinfret held the Interim Director General position in addition to his role as Associate Executive Director and Chief Operating Officer. He leverages the vast knowledge and experience he has developed in administration and governance, human resources, labour relations and government negotiations, as well as his service on a number of boards of directors, to influence and shape positive changes at the MUHC and, more generally, in the health sector. Mr. Rinfret is a Professor at McGill University and sits on the Board of La Capitale General Insurance Inc. Ross Baker Professor, Institute for Health Policy, Management and Evaluation, University of Toronto Ross Baker, PhD, teaches and carries out research at the University of Toronto on patient safety, quality improvement, and leadership and organizational change. Director of the university s MSc Program in Quality Improvement and Patient Safety, his recent research projects include a study of patient engagement in leading organizations in North America and the UK; an assessment of governance in Ontario primary care; a study of contributing causes for medication and fall incidents in home care; and a synthesis of literature on physician engagement in healthcare organizations. In 2009, Dr. Baker led a study of effective governance practices to improve quality and patient safety that became the basis for the governance toolkit (Effective Governance in Quality and Patient Safety) developed by the Canadian Patient Safety Institute (CPSI) and Canadian Foundation for Healthcare Improvement (CFHI). He co-led the 2004 Canadian Adverse Events study, winner of the CFHI Health Services Research Advancement Award, and has been a board member of Saskatchewan s Health Quality Council since In 2011, he chaired the Advisory Panel on Avoidable Hospitalizations for the Ontario Ministry of Health and Long-Term Care. Dr. Baker has also served as a member of Great Britain s King s Fund Commission on Leadership and Management in the UK National Health Service (NHS), and is a member of the Improvement Science Development Group of the Health Foundation in London, England. Alain Biron Assistant to the Director, Quality, Patient Safety and Performance, MUHC Alain Biron, RN, PhD, is Assistant to the Director, Quality, Patient Safety and Performance at the McGill University Health Centre (MUHC) and Assistant Professor at the Ingram School of Nursing, McGill University. His research interests focus on ways high-performing healthcare organizations use performance measurement to drive improvement at the microsystem level. He has been involved in patient engagement and patient experience measurement projects at the local, national and international level since Dr. Biron s published articles concern quality improvement and patient engagement and safety. Jean Bourbeau Director, Respiratory Epidemiology and Clinical Research Unit, MUHC Jean Bourbeau, MD, FRCPC, is a respirologist and Professor in the Departments of Medicine and Epidemiology & Biostatistics at McGill University. He directs the Pulmonary Rehabilitation Unit at the MUHC s Montreal Chest Institute, and the chronic obstructive pulmonary disease (COPD) axis of the Respiratory Network of the Fondation de Recherche en Santé du Québec (FRSQ). He is President of the Canadian Thoracic Society, and Chair of the Dissemination & Implementation committee, as well as a member of the executive committee of the Global Initiative for Chronic Obstructive Lung Disease (GOLD). His areas of research and expertise are in COPD rehabilitation, disease and self-management (Living Well with COPD livingwellwithcopd.com), and in health, behaviour and clinical evaluative research. His research includes the large epidemiologic study of COPD (Canadian Cohort of Obstructive Lung Disease; CanCOLD) and evaluative research projects on new approaches in disease management. Sean P. Clarke Professor and Associate Dean, Undergraduate Program, Connell School of Nursing, Boston College Sean Clarke, RN, PhD, was the Director of the McGill Nursing Collaborative for Education and Innovation in Patient- and Family-Centred Care, a donor-supported initiative enhancing collaboration between McGill s Ingram School of Nursing, the MUHC and the Jewish General Hospital. While in Montreal, he also served as Chair of the Board of Directors of the Conseil québécois d agrément. A nursing services researcher in hospital care quality and safety, Dr. Clarke has extensive university teaching experience in Canada and the United States. He has also managed research groups and been involved in academia-practice partnerships, including an endowed clinical research chair at the University of Toronto and University Health Network (UHN). A fellow of the American Academy of Nursing, Dr. Clarke holds adjunct appointments at the Université de Montréal and the University of Hong Kong. He actively participates in research collaborations and training and the peer review of proposals and journal submissions, and speaks and consults internationally. Mario Di Carlo Member, Board of Directors and Executive Member, Central Users Committee, MUHC Mario Di Carlo s other MUHC roles include Patient Representative on the Transforming Care at the Bedside (TCAB) program and Co-chair of the Users Committee at the Montreal Neurological Institute and Hospital. He is a board member of the Regroupement provincial des comités des usagers (RPCU) and of the Association québécoise de la douleur chronique (AQDC). Mr. Di Carlo is Founder and President of the Butterfly Wings Foundation helping children with neuromuscular conditions, and is Top Trainer for the Stanford University Chronic Disease and Chronic Pain Self-Management Programs, operated at the MUHC as My Tool Box. Vincent Dumez Co-director, Office of Collaboration and Patient Partnership, Faculty of Medicine, Université de Montréal Vincent Dumez, MSc, was the inaugural director of the Université de Montréal medical faculty s Office of Patient Partner Expertise (OPPE) in In 2013, the OPPE was reorganized, and Mr. Dumez became Co-director of the new Office of Collaboration and Patient Partnership with Dr. Paule Lebel. Prior to 2010, Mr. Dumez was an associate at one of Montreal s most influential consulting firms, where he specialized in organizational design and worked for 13 years as a strategic consultant helping to change large health sector enterprises. The sufferer of three chronic diseases for decades, Mr. Dumez has been heavily involved in the development of patient partnerships In Quebec. He first explored the idea in his MSc dissertation on the province s tainted blood scandal and has since developed training for patients with chronic diseases, advanced the concept on the boards of various community organizations, and spoken on the subject in many workshops and conferences for healthcare professionals. Dan Florizone Former Deputy Minister of Health, Saskatchewan Dan Florizone, MBA, held the position of Deputy Minister of Health in Saskatchewan from 2008 to 2013, when he was appointed Deputy Minister of Education and Deputy Minister Responsible for the Lean Initiative. Presently a Policy Fellow with the Johnson-Shoyama Graduate School of Public Policy, Mr. Florizone has participated in many national and international initiatives, committees and projects, serving as Chairperson of the Saskatchewan Health Quality Council, and sitting on the boards of the Canadian Patient Safety Institute and Canada s Mental Health Commission of Canada. Prior to joining the Saskatchewan Ministry of Health, Mr. Florizone served as CEO of the province s Five Hills Health Region and Moose Jaw-Thunder Creek and South- East Health Districts. He received the Institute of Public Administration of Canada-Saskatchewan Lieutenant Governor s Gold Medal Award in 2008 and the Excellence through Evidence Award from the Canadian Foundation for Healthcare Improvement (CFHI) in Carolyn Freeman Physician Quality Champion, Department of Quality, Patient Safety and Performance, MUHC Carolyn Freeman, MD, FRCPC, is Professor of Oncology and Pediatrics and Mike Rosenbloom Chair of Radiation Oncology at McGill University. A radiation oncologist with more than 30 years of experience, Dr. Freeman is a well-known expert in the management of children with cancer. The author of over 120 peer-reviewed articles, she has played leadership roles in many national and international professional organizations, and is a founding member of the Canadian Association of Radiation Oncologists and the International Pediatric Radiation Oncology Society, where she is current President. Dr. Freeman s roles at the MUHC include Co-director of the Quality and Performance Improvement Office for the Cancer Care Mission, and Clinical Advisor for the new Cancer Centre at the Glen (open in 2015). Valerie Frunchak Associate Director of Nursing, Jewish General Hospital Valerie Frunchak, RN, M Sc(A), has provided leadership in a variety of clinical sectors during her career. An Assistant Professor at the Ingram School of Nursing at McGill University, her areas of expertise include women s health, mental health and professional development. 6 About the speakers continued nization he founded in 2007 (initially called the Patients Association of Canada) that now reaches more than 10,000 patients and providers across the country. He holds a PhD from Cornell University and has been an adjunct professor at Toronto, McGill and York universities, and has acted as a health system advisor in Canada and the UK. His interest in health care started when he became a family caregiver for his dying father in Montreal. He participated in the first multi-scenario strategic planning exercise for Ottawa-Carleton. He was the planner for the Centenary Wing of the MUHC s Royal Victoria Hospital in Montreal. He was a Fellow of the King s Fund in England for six years, returning to Canada to take up his role at Baycrest and also lead training programs in patient-centred care at Sunnybrook Hospital in Toronto. Dr. Glouberman has led research on health policy for the Canadian Policy Research Networks and directed a Masters program in health management at McGill University. In 2005, Dr. Glouberman underwent major surgery, became a patient and wrote the book, My Operation, a vivid account of the gap between patients experiences and institutional concerns. Maria Judd Senior Director, Patient Engagement and Improvement, Canadian Foundation for Healthcare Improvement Maria Judd, MSE, is responsible for developing and implementing strategies, programs, activities and business initiatives supporting the mission of the Canadian Foundation for Healthcare Improvement (CFHI). Prior to joining CFHI in 2005, Ms. Judd served in various health-related organizations as a community health centre board member, clinician, program manager and researcher. As the best practices program manager for the Canadian Physiotherapy Association, Ms. Judd designed a program of activities to support evidence-informed practices among members and their colleagues. She also coordinated the Cochrane Collaboration s Musculoskeletal Group at the University of Ottawa. Karen J. Kieley Product Development Specialist, Accreditation Canada Ms. Kieley, MHSA, has worked with Accreditation Canada as an Accreditation Product Development Specialist since In her role, she is responsible for the research and development of national standards of excellence and accreditation processes for health care services. Brenda MacGibbon Patient representative, McGill University Health Centre Ms. MacGibbon was formerly a member of the MUHC Board of Directors and Chair of the MUHC Review Committee. She is currently the Chair of the Royal Victoria Hospital (RVH) Patients Committee and a patient representative on the MUHC Quality and Risk Committee (COQAR), the Technology Assessment Unit (TAU) Policy Committee, the Transforming Care at the Bedside (TCAB) Measure and Evaluation Committee, the MUHC Cancer Mission Quality and Performance Improvement Office (QPIO) Committee, the MUHC PAINFREE* Project and the TCAB team on RVH Surgical 9W. Ms. MacGibbon is a statistician and Adjunct Professor in the Department of Mathematics at the Université de Québec à Montréal (UQAM). James Merlino Chief Experience Officer and colorectal surgeon, Cleveland Clinic James Merlino, MD, is Associate Chief of Staff of the Cleveland Clinic health system, as well as a practising staff colorectal surgeon at the Digestive Disease Institute in Cleveland, Ohio. He is the Founder and current President of the Association for Patient Experience. As a member of the Clinic s executive team, Dr. Merlino leads initiatives to improve patient experience across the Cleveland Clinic s health system and heads efforts to improve physician-patient communication, patient access, relations with referring physicians and employee engagement. In 2013, Dr. Merlino was named to HealthLeaders magazine s list of 20 people who make healthcare better in recognition of his role as a world leader in the field of patient experience. Angela Morin Patient and Family Experience Advisor, Kingston General Hospital Angela Morin, BA, has partnered with healthcare professionals and provided input into quality improvement, program development, and policy and facility design at Kingston General Hospital since She currently sits on the Kingston General Hospital and Southeast Regional Cancer Centre Patient and Family Advisory Councils, as well as Accreditation Canada s Client and Family-Centred Care Advisory Council. She is a faculty member for the Canadian Foundation for Healthcare Improvement s (CFHI) Partnering with Patients and Families for Quality Improvement collaborative. Ms. Morin s experience supporting a close friend through her breast cancer journey continues to motivate her to bring the patient s and family s voice into health care. Ed Percy Vice President, CGI Global Healthcare Ed Percy. B Eng(Hons), is responsible for health care across CGI s worldwide operations, including managing the company s strategic portfolio, promoting its thought leadership, and providing strategic support for 400 offices in 40 countries from his base in Manchester, England. Trained as an engineer and a keen advocate of using technology to improve clinical efficacy and safety, Mr. Percy has worked in the field of health care for most of his career and been instrumental to the success of many critical ehealth projects around the globe. A respected thought leader in Europe and the Middle East, he served on the ehealth Advisory Board of Saudi Arabia s Ministry of Health and is a regular presenter at conferences. Deborah Radcliffe-Branch Director, My Tool Box Chronic Disease Self-Management Program, MUHC Dr. Radcliffe-Branch joined the Montreal Neurological Institute and Hospital (MNI/MNH) in July 2007 as Assistant Professor in the Faculty of Medicine, and was subsequently appointed Director of My Tool Box, the MUHC s Chronic Disease Self-Management program (Stanford model). Dr. Radcliffe-Branch implemented this unique program in English and French across five MUHC hospital sites and has spearheaded the expansion and development of the Stanford CDSMP in select McGill RUIS territories, several regions across Quebec, and overseas. Prior to joining the MUHC, Dr Radcliffe-Branch completed a post-doctoral fellowship at Université de Montréal in the areas of patient-physician communication and older women s health. She is a member of the Advisory Board of the Patient Resource Centre of the MNI/MNH and is the recipient of grants and awards from the Association des paraplegiques du Québec, Fondation pour la recherche sur la moelle épiniere, the Rick Hansen Institute, and most recently the McGill Centre of Excellence on Aging and Chronic Diseases (CEACD). Graeme Rocker Head, Division of Respirology, Capital Health Nova Scotia Graeme Rocker, MA, MHSc, DM FRCP, FRCPC, is a Professor of Medicine at Dalhousie University in Halifax, Nova Scotia. He leads the INSPIRED (Implementing a Novel and Supportive Program of Individualized care for patients and families living with REspiratory Disease) outreach program for patients and families living with advanced chronic obstructive pulmonary disease (COPD). The holder of medical degrees, a Masters in bioethics and a research Doctorate from Oxford, Dr. Rocker received the 2009 Roger Bone Award from the American College of Chest Physicians for advances in end-of-life care. In 2013, Dr. Rocker was appointed Clinical Improvement Advisor to the Canadian Foundation for Healthcare Improvement (CFHI). Jean Rodrigue Interim Director of Public Health, Montérégie Department of Public Health Jean Rodrigue, MD, MScCH, began his career as a family doctor at the CLSC des Etchemins and later practised in Montreal at the CLSC des Faubourgs and the family medicine unit at Hôpital Notre-Dame. He then moved into healthcare administration and has since held many positions, serving most notably as Head of the Department of Family Medicine at Hôpital Notre-Dame; Associate Director General (planning and regionalisation) of the Fédération des médecins omnipraticiens du Québec; Director of Medical and University Affairs at the Montérégie Health and Social Services Agency (ASSS); and Assistant Deputy Minister (Health Services and Academic Medicine) with the Quebec Ministry of Health and Social Services. Dr. Rodrigue has always combined his clinical duties with teaching family medicine students and residents. The holder of a Master s degree in Community Health, he has collaborated on research into health service organization, interdisciplinary collaboration and patients as partners in care. Eleanor Rivoire Executive Vice President and Chief Nurse Executive, Kingston General Hospital Eleanor Rivoire, RN, MSc, has more than 35 years of experience in the clinical, education and administrative domains of professional practice. 7 on patient engagement, patient- and family-centred care, and models of interprofessional practice and education. She is currently engaged in leading innovative changes at Kingston General bringing about improved hospital quality and safety outcomes. Janice Selemba Vice President, Customer Strategy and Value, Roche Canada Janice Selemba, B Com(Hons), is responsible for planning and leading the development of future Roche Canada patient-centric business models and translating strategy across functions and business units. As a member of Roche s Canadian leadership team, she helps set strategic directions, and has driven company-wide patient and customer centricity shifts, led business transformational initiatives, and been actively engaged in talent management. Previously, Ms. Selemba was with Sanofi Canada, holding a number of positions including Vice President, Marketing, Customer Strategy & Solutions & Access. During her career of over 20 years, she has worked in pharmaceutical marketing, sales, product development, reimbursement, continuing education and training, and business and financial analysis. Bill Tholl President and CEO, HealthCareCAN Mr. Tholl serves as the Founding President and CEO of HealthCareCAN (formerly the merged Association of Canadian Academic Healthcare Organizations and the Canadian Healthcare Association). The mandate of the new organization is to speak with one unified voice on behalf of Canada s healthcare community to advance organizational and health system performance. Prior to joining HealthCareCAN, he was the Founding Executive Director of the Canadian Health Leadership Network ( ). This network was formed to increase leadership capacity throughout Canada s health and healthcare systems. Mr. Tholl also served as CEO and Secretary General of the Canadian Medical Association ( ) and the Heart and Stroke Foundation of Canada ( ). The Globe and Mail has described him as Medicare s Mr. Fix-it. Mr. Tholl is the recipient of numerous national awards and has written on many topics, most recently as the co-author of Bringing Leadership to Life in Health (Springer, January 2014). Ruth Thorlby Senior Fellow, Health Policy, Nuffield Trust Ruth Thorlby, MSc, is a Senior Health Policy Fellow at the Nuffield Trust, an independent British institute based in London conducting healthcare research and policy analysis. Ms. Thorlby s research interests include UK National Health Service (NHS) reform, integrated care, GP commissioning, accountability, international comparisons and health inequalities. Her recent publications include a study of the impact of Britain s Francis Inquiry into poor NHS care on the behaviour of senior hospital management, and studies of strategies to improve population health management in general practice and the effects of budget cuts on older adults social care services in England. Before joining the Nuffield Trust. Ms. Thorlby was a senior fellow at The King s Fund and a Harkness Fellow at Harvard Medical School, where she researched how US physicians and healthcare organizations understand and tackle racial inequities in health service quality. A graduate of the London School of Economics (LSE), Ms. Thorlby was a broadcast journalist before moving into health policy, and worked for BBC News and the BBC World Service. Brigitte Vachon Assistant Professor, School of Rehabilitation, Université de Montréal Brigitte Vachon, PhD, is Assistant Professor of Occupational Therapy at the Université de Montréal s School of Rehabilitation. She is a researcher at the Centre de recherche Fernand-Seguin of Montreal s University Institute of Mental Health, and collaborator on the COMPAS project with the Public Health Agency of Montérégie. Her research focuses on factors and strategies changing professional practice in primary care and rehabilitation, and examines the evaluation of interventions promoting interprofessional collaboration and patient involvement in health services quality improvement.
