The discussion will focus on how health care organizations and states are transforming into highly functioning, health IT-enabled practices that can coordinate patient care across the medical neighborhood, engage patients in their care, and manage populations to improve health outcomes. Panelists representing innovative models such as Accountable Care Organizations, the Comprehensive Primary Care Initiative, State Innovation Models, and other programs at the state level will discuss challenges and successes in their implementation of these CMS initiatives, as well as share their experiences and explore how other states and communities can learn from, iterate on, and build upon their roadmap for transforming health care.
