3 History of Medical Home Pediatrics -Started as a movement to have the medical record stored in the pediatrician s office. Developed as a way to provide quality care for children with special healthcare needs. In 2002, grew into a concept for medical care.

10 Joint Statement Definition The Patient-Centered Medical Home (PC- MH) is an approach to providing comprehensive primary care for children, youth and adults. The PC-MH is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient s family.
