There are different areas of this assessment. The white boxes are for you to write in. The grey shaded areas are for the assessor to write in. Please do not write in these boxes. 3 Instructions for the trainee Definitions of terms Abbreviations Your answers to the assessment tasks need to show the assessor that you have a full understanding of the topic. The assessor may require you to discuss your responses. This assessment is open book, which means that you can use any information you wish when you are completing this assessment. This information may include learning materials, books, the internet and your organisation s policies and procedures. Your answer for Task One is a practical demonstration. Your assessor or verifier will observe you and complete an observation checklist. The assessor/verifier may require you to perform the task on more than one occasion. For any documents you supply, remove all identifying details of the people receiving services or support. You need to show that your work complies with your organisation's policies and procedures. If you require assistance with any aspect of the assessment, please contact your assessor. The word trainee in this assessment refers to the person being assessed. Other terms that may be used are candidate, student or employee. To report is to pass on information about a person's condition to the appropriate authority, as specified in the organisation s policies and procedures. Service plan is a general term that describes the individual or group plan for service delivery. This plan is developed by service providers with the person or people receiving support and their families/whānau. A service plan may include a care plan or a rehabilitation plan. An organisation s policies and procedures are the policies and procedures of the trainee s employer and include ethical codes, standards and other organisational requirements. Workplace assessor or assessor means the person who will assess the trainee. A verifier is a workplace supervisor or manager who understands the assessment, works closely with the trainee, and can confirm that the trainee has performed the task competently. ER refers to the evidence requirements of the unit standard the evidence (descriptions, explanations, documents etc) that you must provide and/or the actions that you must do or demonstrate. 4 Task One Observe, report, and document changes in condition in a person receiving care or support in a health or disability setting (demonstration) Outcomes 1, 2 and 3 For this task you must demonstrate that you have: 1 Observed two changes in the condition of either one person or two different people. These changes can be physical, behavioural, environmental, or emotional. Your observation must relate to: Conditions documented in the person s service plan. Observations requested in the progress reports. Changes in condition recorded in the progress reports. Any changes in condition not previously reported. 2 Reported these two changes: Following your organisation s policies and procedures. Following legislative requirements and the Code of Rights. 3 Documented these two changes: Describing, in your notes, the impact of these changes on the achievement of the person s independence goals. Making sure that your change notes are accurate and effective. They must be legible, factual, written in permanent ink, be dated, give the time, be signed, describe condition changes, describe the response to care, and follow legislative requirements. Following your organisation s policies and procedures. You will be observed demonstrating this task and your observer will complete the observation checklist. You might be observed by the assessor or by a verifier, such as your manager, supervisor or team leader who can verify that they have observed you carrying out this type of work in your day-to-day work. If so, the assessor may contact this person to discuss this form and the responses made. Your manager/supervisor will give feedback about your involvement in observing, reporting and documenting changes in the condition of a person/two people receiving care or support. The assessor may contact your manager/supervisor to discuss this form and the responses made. If no is checked on the form, a brief comment must be given. Your observer and/or assessor may also ask you questions to clarify your understanding of the tasks you are carrying out. 5 Observation checklist (to be completed by an observer, such as your manager, supervisor, team leader or assessor) As you carry out this practical task an observer will fill in this checklist. The observer may be your assessor or a verifier such as your manager, supervisor or team leader, who can verify that you have competently performed the task. When doing this practical task you must work within your organisation s policies and procedures. You must respect the rights and culture of the person you are supporting. Observe, report, and document changes in condition in a person receiving care or support in a health or disability setting The trainee observed two changes in the condition of either one person or two different people. These changes can be physical, behavioural, environmental, or emotional. The trainee s observation related to conditions documented in the person s service plan.
