Our mission is to provide hope, trust, and acceptance to all individuals who request services. Patience and acceptance are mandatory in receiving care and services from this Authority. We acknowledge that mental health and physical well-being are fundamental to the quality of life and productivity of individuals, families and communities. 71 ENROLLEE RIGHTS and RESPONSIBILITIES By State requirement; Recipients, Members and Enrollees have a Right to the following: 1. To be treated in a caring, respectful, culturally sensitive and professional manner. 2. To be treated with respect and recognition of your dignity and right to privacy. 3. To receive the Member Handbook, Provider Directory, and information about DWMHA, our services, our providers, and enrollee rights and responsibilities at the time of enrollment, intake, annually and upon request. 4. To participate with practitioners in making decisions about your care. 5. Enrollees have the responsibility to follow plans and instructions for care that you have agreed to with your practitioners. 6. To have a candid discussion of appropriate or medically necessary treatment options for your conditions, regardless of cost or benefit coverage. 7. To know the names and titles of all healthcare professionals involved in your care. 8. Enrollees have the responsibility to supply information (to the extent possible) that DWMHA, your MCPN, provider and its practitioners need in order to provide care. 9. Enrollees have the responsibility to understand your health problems and participate in developing mutually agreed-upon treatment goals, to the degree possible. 10. To voice complaints, grievances and appeals about DWMHA, your MCPN, or provider or the care provided to you. 11. To make recommendations regarding DWMHA s member rights and responsibilities policy. 12. 72 Children s Waiver Programs (CWP) - The Children s Home and Community Based Waiver Program (CWP) provides services that are enhancements or additions to regular Medicaid coverage to children up to age 18 who are enrolled in the CWP. The Children s Waiver is a fee-for-service program administered by the Community Mental Health Service Provider (CMHSP). The CMHSP is responsible for assessment of potential waiver candidates.
