3 Access Department All Washtenaw County Residents Engagement, Connection, Crisis Management Triage, Assessment, Crisis Services Hours of operation- 24/7, 365 days per year Contact number Access received 55, 855 calls & completed 1,408 assessments

4 Access Department Triage Team- All Washtenaw County residents, information and referral to community resources, links to immediate support for mental health and substance use emergencies Crisis / Assessment Team- A mobile team, crisis stabilization services, information and linkage, navigation to mental health system, preadmission screening process, hospital diversion planning, and eligibility determination process to determine CSTS eligibility. Crisis Residential Services- Recovery oriented, person centered, 6 beds, short term crisis stabilization home.

5 Integrated Health Provide limited mental health services (consultation, short term therapy, co-visits) and bridging to CSTS if necessary at Packard Health. The staffing model is 1 FTE Mental Health Professional and 4 hours a week of Prescriber time. Washtenaw Health Initiative (WHI) member of the Safety Net committee which includes other Integrated Health partners.

6 Nursing, Health & Wellness, OBRA Nursing Services- Provides medication reconciliation and administration, checking vitals and education regarding medication and health conditions and care coordination with outside providers Health and Wellness- Provide disease management/health wellness services- care coordination, health education, limited transportation, nutrition services, social support to CMH clients with co-morbid health conditions Current Health Home Pilot Project- 535 enrolled consumers OBRA-provides screening for persons with a mental health diagnosis or developmental disability seeking admission to nursing facilities.

7 Youth and Family Services We provide a broad range of services to support the social and emotional health needs of children and their families from infancy through 18 years of age Children may have a mental health diagnosis or a developmental disability Currently serve approximately 800 families annually Use evidence based interventions supported by the American Academy of Pediatrics Philosophy is family centered, youth driven and strength based Services are provided in the community, foster independence, choice, and address the issues that are important to the family
