2 The Dialysis Center "Alle Vele" can be found in Lignano Sabbiadoro, Via Centrale 29. It can be reached by car in the following manner: - from the motorway A4 Venice-Trieste, you must get off at Latisana thus following the directions for Lignano Sabbiadoro (20 KM.), at the end of which you then reach a roundabout with a fountain at its entrance and there, you must proceed turning left towards Lignano Pineta; - once arrived at the GETUR EFA Village, go to the reception asking for directions to reach the "Alle Vele" Residence. 2. The Dialysis Center "Alle Vele" carries out dialytic treatments for chronic, haemodialytic patients coming from Nephrologic Services and Dialysis, which are operative both on national and foreign territories. The Dialysis Center "Alle Vele" is a C.A.L. (Center of Limited Assistance) These services are carried out during the Summer period, starting from May to September. 3. The Center of Dialysis carries out two different types of dialytic services: 1. Bicarbonate dialysis; 2. Haemodiafiltration on line. Pag 2 di 5

The Center disposes of dialytic devices supplied by the Fresenius Firm together with a reverseosmosis demineralizator of recent manufacture both equipped with all technical requisitions which guarantee a perfect functioning and maximum safety in respect of the European standards now in act. The maintenance of both the reverse-osmosis device and the dialytic machinery is guaranteed by a full-risk assistance contract stipulated with the Fresenius Firm. 5. The Center of Dialysis disposes of: a dressing-room area with separated restrooms for each sex and then further divided between patients and assistant personnel, with respect of privacy standards; a waiting room which is also used for administrative acts; a small infusion room; a room for the observation of patients; a dialytic room supplied with 7 dialytic positions of which 2 are dedicated to HCV positive patients. However, the Center does not dispose of contumacial areas for HBV positive patients therefore they can not be accepted in the dialysis center. All the positions are supplied with scale beds in order to constantly monitor weight variations of the patients. Moreover, each bed also disposes of personal headphones to listen to and watch radio/tv programmes. The structure is also equipped with: a cart with all emergency and rianimation equipment; a haemogasanalyzer for the evaluation of major electrolytes (Na, K, Cl, Ca), of the acid base equilibrium (PH, PC02, P02, HC03) and of some haemato-chemical parameters such as haemoglobin, haematocrits, glicaemia, ect. 6. Regarding the organization of the Dialysis Center, a Sanitary Director will be nominated, who will be responsible for the administration, the inspection, the programming and the verification of all the activities carried out annually. The constant presence of a doctor, a specialist in nephrology together with an adequate number of professional, nurse personnel per each dialysis position (1 nurse for every 3 patients), however always on the basis of the typology of service to be carried out and at the conditions of the single patients (staff on the site). 7. At the Center a person is responsible for the pharmacy, supplying medicines and dialytic material, the administration and control of the emergency cart and moreover with the duty of addressing and organizing schedules and functions of all the nurse personnel. All the working personnel in the Dialysis Center receives a personal copy and an adequate formation course regarding all the major guide lines, protocols and instructions established by the Sanitary Direction of the C.A.L. thus the personnel is strictly committed to observe them. These procedures are up-dated annualy (in the same period of the yearly opening of the center) and so is the personnel in the case of variations. 9. All the person of the staff is highly qualified in order to work in the Dialysis Center. For newly hired persons, a differentiated training will be carried out, according to duties and to their degree of preperation in relation to their acquired experience thanks to former working tasks. 10. During the opening period, two turns of dialysis will be performed: a morning one, starting from 7:00 a.m. to 12:00p.m. then an afternoon session, from 1:00p.m. to 6:00p.m. In the programming of dialytic turns, the demands of the users are taken into consideration as much as possible and therefore eventual variations of schedules\turns are always communicated to the users opportunely. 11. The client may book for the dialytic services in advance at the C.A.L. "Alle Vele" (Director of administrative ) by handing in the patient's medical cards issued by the Center where he\she habitually undergoes the dialytic treatment and by specifying the kind of dialysis carried out, the heparinization system used, the duration of the treatment, hepatitis markers, vascular access and intra\extra dialytic therapy. 12. Reservations can be carried out during the solar year by means of a specific answering machine service, which has been activated at the EFA head office in Udine during office hours (8:30a.m. - 12:30p.m. ; 2:30 p.m. - 5:30 p.m., tel.number: ; fax: mail: info@efafondazione.com). The reservations may be done in different ways : via telephone, fax, letter, or At the moment of the reservation, the following information will be communicated: 1. the type of services carried out ( haemodialysis in standard bicarbonate and on-line haemodiafiltration); 2. the service rates carried into effect on behalf of the Dialytic Center in accordance with the scale of charges of the F.V.G. Region, if it is the case of a private customer; 3. the confirmation time of the reservation (maximum 15 days from the reservation). Pag 4 di 5

5 14. The fee due for the dialysis session is fixed by the regional official price list (Friuli Venezia Giulia) and the same amount is given by the others italian regions, as forseen in agreements. For foreign guests, if the sum given by their insurance institute is lower then the above mentioned fee, the difference is charged to the single user. 15. Regarding the criteria for the selection of patients, they will have to hand in, in due time, all the adequate documentation written by their personal doctor or by the structure they have been followed by. The Sanitary Direction, make the final decision about accepting the patient or not in the structure, always in accordance with the original center of dialysis of the patients. 16. Before starting any kind of dialytic session, each patient must undergo a nephrologic check-up and hand in all his\her anamnestic data. 17. Each patient must sign an informative form of agreement together with a form regarding the treatment of personal data which must be carried out before the start of the first dialytic session. 18. The record-charts of all patients will be kept under lock and key in specific archives, to which only the Sanitary Director and the Head of the Dialytic Department have access. 19. The Sanitary Direction has also established that all patients, at the end of their stay in the Center, must fill in an evaluation questionnaire regarding the service quality carried out on behalf of the C.A.L. "Alle Vele" together with eventual complaints\suggestions which will be gathered in a specific register. 20. The deliberation of the Regional Council n. 297 of February 8, 2008 has established a formal union of the C.A.L. "Alle Vele" with the Nephrology and Dialysis Operative Unit of the ASS. N. of Palmanova, Regarding this matter, the Operative Unit of reference, upon previous, agreed conditions, is committed to: 1. the treatment of cases which can not be resolved at a local level; 2. the management of problematic, vascular accesses; 3. the management of medical- surgical complications, by means of an emergency unit always at disposition, to be carried out at the head office, during the hours which are not covered by C.A.L. medical personnel O Pag 5 di 5
